Private Health Insurance: Research on Competition in the Insurance Industry

Correspondence issued by the Government Accountability Office with an abstract that begins "Health care providers and members of Congress have raised concerns that consolidation in the private health insurance industry may be resulting in less competitive markets and contributing to rising health insurance rates paid by consumers and employers. However, measuring the extent of changes in market competition over time or the effects of changes is challenging. In particular, reliable, longitudinal data to measure concentration, that is, the number of competitors and their relative market share, are only available on health maintenance organizations (HMO) but not on preferred provider organizations (PPO) or other insurance products that may comprise the market. Further, data on health insurers are not available at all geographic levels. Despite these challenges, researchers have used the data available to study competition in health insurance markets, typically using one of two measures of competition: (1) HMO market concentration or (2) the number of HMOs in a market. Researchers acknowledge that market concentration and the number of competitors are not perfect measures of competition in private health insurance markets and that there are limits to the conclusions to be drawn from studies that rely on the available data. Testimony issued by the Government Accountability Office with an abstract that begins "The American Recovery and Reinvestment Act of 2009 (Recovery Act) included more than $48 billion for the Department of Transportation's (DOT) investment in transportation infrastructure, including highways, rail, and transit. This testimony--based on GAO report GAO-09-829, issued on July 8, 2009 and updated with more recent data, in response to a mandate under the Recovery Act--addresses (1) the uses of Recovery Act transportation funding including the types of projects states have funded, (2) the steps states have taken to meet the act's requirements, and (3) GAO's other work on transportation funding under the Recovery Act. In GAO-09-829, GAO examined the use of Recovery Act funds by 16 states and the District of Columbia (District), representing about 65 percent of the U.S. population and two-thirds of the federal assistance available through the act. Participants included representatives from U.S. government agencies, foreign embassies, nongovernmental and multilateral organizations, policy institutes, the private sector, and academia. GAO was asked to conduct a forensic audit and related investigations to (1) determine whether GAO could substantiate the allegation of lost and stolen property at IHS and identify examples of wasteful purchases and (2) identify the key causes of any loss, theft, or waste. GAO analyzed IHS property records from fiscal years 2004 to 2007, conducted a full physical inventory at IHS headquarters, and statistically tested inventory of information technology (IT) equipment at seven IHS field locations in 2007 and 2008. Consequently, it is important that projects be managed effectively to ensure that public resources are wisely invested. To this end, the Office of Management and Budget (OMB), which plays a key role in overseeing the federal government's IT investments, identifies major projects that are poorly planned by placing them on a Management Watch List and requires agencies to identify high-risk projects that are performing poorly (i.e., have performance shortfalls). Having accurate and transparent project cost and schedule information is also essential to effective oversight. At times, changes to this information--called a rebaselining-- are made to reflect changed development circumstances. These changes can be done for valid reasons, but can also be used to mask cost overruns and schedule delays. GAO has previously testified on the Management Watch List and high risk projects. GAO was asked to (1) provide an update on these projects, (2) identify OMB's efforts to improve the identification and oversight of these projects, and (3) summarize the results of GAO's IT project rebaselining report being released today. Though not required to do so, the Coast Guard has established a similar program. This statement addresses the extent to which DOD and the Coast Guard (1) have developed and implemented policies and programs to prevent, respond to, and resolve sexual assault incidents involving servicemembers; (2) have visibility over reports of sexual assault; and (3) exercise oversight over reports of sexual assault. This statement draws on GAO's preliminary observations from an ongoing engagement examining DOD's and the Coast Guard's programs to prevent and respond to sexual assault. In conducting its ongoing work GAO reviewed legislative requirements and DOD and Coast Guard guidance, analyzed sexual assault incident data, and obtained through surveys and interviews the perspective on sexual assault matters of more than 3,900 servicemembers stationed in the United States and overseas. The results of GAO's survey and interviews provide insight into the implementation of the programs but are nongeneralizable. Correspondence issued by the Government Accountability Office with an abstract that begins "In October 2001, spores of the deadly bacterium anthrax were found in mail sent to members of the news media and congressional leaders. In all, 22 people were infected with anthrax and 5 people died, including 2 postal workers. The United States Postal Service (Service) took a variety of steps to protect people from biohazards in the mail. For example, the Service began contracting for the irradiation of mail to recipients at the Congress, the White House, and federal agencies with specific ZIP Codes (20201 through 20597) in the Washington, D.C., area (D.C. federal mail). The irradiation process uses either a high-energy electron beam or X-rays to penetrate pieces of mail (mailpieces) and kill harmful organisms, such as anthrax. The Service initially hired two contractors to irradiate the mail, one of which operated between November 2001 and April 2002. The other contractor has been irradiating mail since November 2001. In addition, the Service hired contractors to transport the mail for irradiation and to oversee the current irradiation contractor. The large percentage of procurements recorded as miscellaneous obligations in fiscal year 2007 raised questions about whether proper controls were in place over the authorization and use of billions of dollars. GAO's testimony provides preliminary findings related to (1) how VHA used miscellaneous obligations during fiscal year 2007, and (2) whether the Department of Veterans' Affairs (VA) policies and procedures were designed to provide adequate controls over their authorization and use. GAO recently provided its related draft report to the Secretary of Veterans Affairs for review and comment and plans to issue its final report as a follow-up to this testimony. GAO obtained and analyzed available VHA data on miscellaneous obligations, reviewed VA policies and procedures, and reviewed a nongeneralizable sample of 42 miscellaneous obligations at three case study locations. GAO's related draft report includes four recommendations to strengthen internal controls governing the authorization and use of miscellaneous obligations, in compliance with applicable federal appropriations law and internal control standards."

Past disclosures of terrorists' plans for smuggling liquids onboard aircraft to construct a bomb in flight highlighted the continued need to examine this key aspect of homeland security. One layer of the aviation security system involves the ability of the federal government to respond to actual or potential security threats while a commercial aircraft is in flight. These security threats can include the following: (1) Passengers considered to be security risks to aviation are found to be onboard flights bound for or leaving the United States. (2) Situations develop while the aircraft is in flight--for example, a passenger becomes disruptive or acts suspiciously, a bomb threat is received, or an unidentified package is found onboard the aircraft. (3) A commercial aircraft transmits a signal designed to alert authorities that a hijacking is in process. Procedures for addressing these in-flight security threats involve a wide range of federal agencies and entities. Testimony issued by the Government Accountability Office with an abstract that begins "Since the June 27, 2007, Capitol Visitor Center (CVC) hearing, the project's construction has progressed, and according to the latest schedule, Architect of the Capitol (AOC) is still projecting a June 27, 2008, completion date and a September 22, 2008 opening date. Work has advanced on the project's heating, ventilation, and air-conditioning (HVAC) system, interior wall stone and ceiling installation, and other interior and exterior construction work. However, some delays have occurred in activities on the project's critical path (i.e., the work on the fire alarm system) and on most of its near-critical paths, and further delays are possible. AOC was able to mitigate the delay in the project's critical path by reducing the time available for future fire alarm testing. This action may not produce the desired results, though, given the complexity of the requirements for fire alarm testing. Delays in near-critical activities such as the ceiling close-ins and the House and Senate expansion spaces have reduced the time reserved for contingencies along those near-critical paths, but have not yet affected the critical path. The Act stipulated major changes to FEMA that were intended to enhance its preparedness for and response to catastrophic and major disasters. As GAO has reported, FEMA and DHS face continued challenges, including clearly defining leadership roles and responsibilities, developing necessary disaster response capabilities, and establishing accountability systems to provide effective services while protecting against waste, fraud, and abuse. Correspondence issued by the Government Accountability Office with an abstract that begins "In September 2004, the Air National Guard (ANG) discovered inaccuracies in its personnel strength data for recruits with no prior service assigned to pay groups F (attending Initial Active Duty Training, or IADT) and P (awaiting IADT). Specifically, these data did not reflect an accurate accounting of the number of recruits in pay groups F and P. Those recruits attending IADT work full-time and are paid accordingly (pay group F), while recruits waiting to attend IADT receive pay on a part-time basis at the rate of four drills per month (pay group P). The pay for those attending IADT is much higher than for those waiting to attend IADT, so the effect of miscoding the pay groups on ANG's budget estimates can be considerable. In May 2007, we reported to Congress that although ANG has taken reasonable steps to correct the errors in its personnel strength data, some problems still exist with a key quality assurance tool (the Status Report Card process) and with the reliability of the alternate data being reported while the inaccuracies are addressed. Correspondence issued by the Government Accountability Office with an abstract that begins "About 111,000 women covered by the Department of Defense's (DOD) TRICARE program gave birth during 2006. During their pregnancies, about half of these women received obstetric care from physicians and other providers practicing at military hospitals and clinics called military treatment facilities (MTF), while half received their care from civilian physicians and other civilian providers. In recent years, the use of civilian obstetric care has increased among TRICARE beneficiaries. In 2004, 51 percent of TRICARE beneficiaries delivered their babies at civilian hospitals; by 2006, 54 percent delivered at civilian hospitals. However, through 2005, some TRICARE beneficiaries reported difficulties obtaining obstetric care from civilian physicians. At the same time, some civilian physicians contended that TRICARE payment rates for obstetric care were too low. TRICARE reimburses physicians for most obstetric care using two global payments, one for uncomplicated vaginal delivery and the other for uncomplicated cesarean delivery, each of which is a single amount that covers a defined set of related services.
