Although only three account holders are actively earning credits and two have begun to use them, in the 12-month period ending November 19, 2012, the number of credits OTEXA issued increased to about 42 million SMEs from approximately 8 million SMEs during the same period 1 year earlier. Similarly, over this period, the number of credits redeemed, or used by account holders increased to about 25 million SMEs from approximately million SMEs in 2011. These 1,964 hospitals, which represented 39 percent of the 5,013 eligible hospitals, were awarded a total of $1.7 billion in Medicaid EHR incentive payments for 2011. While the amount of Medicaid EHR incentive payments awarded to each hospital ranged from $7,528 to $7.2 million, the median payment amount was $613,512. Participation rates, as well as total payments, were higher for hospitals in the Medicaid EHR program when compared to the Medicare EHR program, though the median payment amount in the Medicaid EHR program was less than half as large. For example, both USAID and USDA have objectives that address financial services, infrastructure, agricultural productivity, agribusiness development, and child and maternal nutrition needs in food insecure countries. Some of these shared objectives are the result of authorizing legislation, through which Congress outlines nonemergency food aid objectives, while others are included in presidential initiatives and agency strategies. We also found that USAID and USDA nonemergency food aid programs shared common geographic focus areas in which they implemented similar activities. For example, in fiscal year 2011, both USAID and USDA had nonemergency food aid programs in Guatemala and Uganda and both programs were providing agricultural training. As a result, Medicare may have paid more than necessary for dialysis care in 2011 because the bundled payment rate in that year was based on 2007 utilization levels. We estimated that Medicare expenditures on dialysis would have been about $650 million to $880 million lower in 2011 if the bundled payment rate were rebased to reflect the 2011 utilization level of ESRD drugs. Furthermore, this estimate of potential savings could be larger in future years if the level of ESRD drug utilization at the end of 2011 declines further, as preliminary data suggest. Rebasing the bundled payment rate to account for changes in ESRD drug utilization could help ensure that Medicare pays appropriately for dialysis services and also yield savings to Medicare. However, CMS officials indicated that they did not have immediate plans to rebase the rate and that the statute does not provide CMS with explicit authority to do so. Our review of open sources published from January 2012 through September 2012 indicated that only 1 of these 17 firms--Sytrol--was reported to have sold refined petroleum products to Iran during that same time period. In addition, our open source review and communication with firms indicated that 12 of the 17 firms were reported to have stopped selling these products to Iran before September 30, 2012; however, our review did not identify sufficient information to indicate either that the remaining 4 firms had continued to sell or ceased selling refined petroleum products to Iran at some point between January 1, 2012 and September 30, 2012."

GAO's preliminary evaluation indicates that the cost estimates are comprehensive in that they include major components of construction and operating costs. However, they are not based on a complete set of assumptions, such as how the Authority expects to adapt existing high-speed rail technology to the project in California. The cost estimates are accurate in that they are based on the most recent project scope, include an inflation adjustment, and contain few mathematical errors. In both the Baseline Extended and Alternative simulations, debt held by the public grows as a share of gross domestic product (GDP) over the long term. While the timing and pace of growth varies depending on the assumptions used, neither set of assumptions achieves a sustainable path. In the Baseline Extended simulation, which assumes current law, including the discretionary spending limits and other spending reductions contained in the Budget Control Act (BCA) of 2011 and expiration of certain tax cuts enacted in 2001 and 2003, debt as a share of GDP declines in the short term before turning up again.
