This report describes the Future Combat System (FCS) program, which was to be the Army's major research, development, and acquisition program consisting of 14 manned and unmanned systems linked by an extensive communications and information network. This report describes the proposed restructuring of the FCS program, announced by Secretary of Defense Gates in April 2009, which includes modernizing outdated equipment and re-evaluating vehicle design strategy. This report will be superseded by a report on the Army's BCT (Brigade Combat Team) Modernization Strategy when sufficient details are available.

A letter report issued by the Government Accountability Office with an abstract that begins "Concerns over unsuitable insurance products and inappropriate sales practices directed at servicemembers have prompted legislative and regulatory changes to address these concerns. Congress asked GAO to assess oversight efforts by the Department of Defense (DOD), the National Association of Insurance Commissioners (NAIC), and state insurance regulators. In this report, GAO (1) describes actions to improve oversight of life insurance sales to servicemembers and the extent to which they have been completed, and (2) identifies and analyzes factors that contribute to apparent ongoing inappropriate insurance sales. To complete this work, GAO visited five military facilities, analyzed DOD pay records, reviewed DOD and state enforcement actions, and spoke with DOD, state regulatory, and insurance industry officials."

Tax Gap: Requiring Information Reporting for Charitable Cash Contributions May Not Be an Effective Way to Improve Compliance

A letter report issued by the Government Accountability Office with an abstract that begins "Individual taxpayers who misreport charitable cash contributions they deduct on their tax returns contribute to the tax gap, the difference between tax amounts taxpayers report and pay voluntarily and on time and the amounts they should pay under the law. The Internal Revenue Service (IRS) most recently estimated a gross tax gap of $345 billion for tax year 2001. One approach that tends to result in high levels of taxpayer compliance is information reporting to IRS by third parties on taxpayer transactions. GAO was asked to (1) provide information on characteristics of individual taxpayer misreporting of charitable cash contributions, (2) provide information on actions that IRS takes to address misreporting, and (3) evaluate potential benefits and challenges associated with requiring information reporting for charitable cash contributions. To meet its objectives, GAO used data from IRS's tax year 2001 National Research Program (NRP) compliance study of individual taxpayers, reviewed IRS guidance and enforcement data, and interviewed IRS officials and representatives from charities or organizations that represent charities. A letter report issued by the Government Accountability Office with an abstract that begins "To reduce the number of crashes involving commercial motor carriers, the Federal Motor Carrier Safety Administration (FMCSA) within the Department of Transportation orders unsafe carriers out of service. To help keep these carriers off the road, FMCSA's voluntary Performance and Registration Information Systems Management (PRISM) grant program, a small program funded at $5 million per year, helps states establish information systems connections between state vehicle registration and FMCSA's safety databases. These connections provide states with up-to-date information on carriers' safety status when carriers try to register or renew registrations with the state. For states to deny, suspend, or revoke registrations to out-of service carriers, states must pass legislation enabling them to do so. As directed by a congressional committee, GAO examined (1) PRISM's effectiveness and (2) the potential to fully implement the program nationally. Financial Management: Achieving Financial Statement Auditability in the Department of Defense

A letter report issued by the Government Accountability Office with an abstract that begins "Since the first financial statement audit was attempted at a major Department of Defense (DOD) component over 20 years ago, GAO and DOD auditors have continued to report significant weaknesses in the department's ability to provide timely, reliable, consistent, and accurate information for management analysis, decision-making, and reporting. DOD has undertaken a number of initiatives over the years, such as the Financial Improvement Initiative in 2003, to improve the department's business operations, including financial management, and achieve clean financial statement audit opinions. However, these initiatives have met with limited success. In 2005, the DOD Comptroller established the DOD Financial Improvement and Audit Readiness (FIAR) Directorate to manage DOD-wide financial improvement efforts and to integrate those efforts with transformation activities, such as those outlined in the Enterprise Transition Plan, across the department. The components report accomplishments and progress against planned corrective actions to the FIAR Directorate for reporting in the FIAR Plan. In December 2007, the Office of Management and Budget (OMB) issued guidance aimed at improving the use of award fee contracts. GAO was asked to (1) identify agencies' actions to revise or develop award fee policies and guidance to reflect OMB guidance, (2) assess the consistency of current practices with the new guidance, and (3) determine the extent agencies are collecting, analyzing, and sharing information on award fees. Although until recently deployed in Iraq and regarded favorably, it has not performed the full range of missions anticipated, and how well it can do so is in question. In view of concerns about the V-22 program, you asked us to determine if the V-22 will perform as promised, and if it will, at what cost. A letter report issued by the Government Accountability Office with an abstract that begins "Foreign investors in U.S. companies or assets include individuals, companies, and government entities. One type of foreign investor that has been increasingly active in world markets is sovereign wealth funds (SWF), government-controlled funds that seek to invest in other countries. As the activities of these funds have grown they have been praised as providing valuable capital to world markets, but questions have been raised about their lack of transparency and the potential impact of their investments on recipient countries. GAO's second report on SWFs reviews (1) U.S. laws that specifically affect foreign investment, including that by SWFs, in the United States and (2) processes agencies use to enforce them. GAO reviewed policy statements, treaties, and U.S. laws, and interviewed and obtained information from agencies responsible for enforcing these laws. GAO also interviewed legal experts and organizations that track state foreign investment issues."

The initiative enables DOD to leverage private sector resources to construct or renovate family housing. As of March 2009, DOD had awarded 94 projects and attracted over $22 billion in private financing. DOD plans to privatize 98 percent of its domestic family housing through 2012. Since GAO's last housing privatization report in 2006, major force structure initiatives have placed new demands on DOD for housing. GAO was asked to assess (1) the progress of DOD's housing privatization program, (2) the occupancy rates of the housing projects, (3) the impact of various force structure initiatives and DOD's efforts to mitigate any challenges, and (4) the effect of financial market turmoil on some projects. A letter report issued by the Government Accountability Office with an abstract that begins "The Environmental Protection Agency (EPA) has estimated that a potential gap between future needs and current spending for wastewater infrastructure of $150 billion to $400 billion could occur over the next decade. A number of entities are involved in planning, financing, building, and operating this infrastructure. Some of these stakeholders have suggested a variety of approaches to bridge this potential gap. One such proposal is to establish a clean water trust fund. In this context, GAO was asked to (1) obtain stakeholders' views on the issues that would need to be addressed in designing and establishing a clean water trust fund and (2) identify and describe potential options that could generate about $10 billion in revenue to support a clean water trust fund. To assist in addressing DOD's business system modernization challenges, the Ronald W. Reagan National Defense Authorization Act for Fiscal Year 2005 (the Act) contains provisions that require the department to take certain actions and to annually report to its congressional committees on these actions. A letter report issued by the Government Accountability Office with an abstract that begins "Tens of thousands of women die each year from breast or cervical cancer. While screening and early detection through mammograms and Pap tests--followed by treatment--can improve survival, low-income, uninsured women are often not screened. In 1990, Congress authorized the Centers for Disease Control and Prevention (CDC) to fund screening and diagnostic services for such women, which led CDC to establish the National Breast and Cervical Cancer Early Detection Program. The Breast and Cervical Cancer Prevention and Treatment Act of 2000 was also enacted to allow states to extend Medicaid eligibility to women screened under the Early Detection Program and who need breast or cervical cancer treatment. Screened under the program is defined, at a minimum, as screening paid for with CDC funds. GAO examined the Early Detection Program's screening of eligible women, states' implementation of the Treatment Act, Medicaid enrollment and spending under the Treatment Act, and alternatives available to women ineligible for Medicaid under the Treatment Act. It provided nearly $575 million in assistance in fiscal year 2008. This assistance is provided through the U.S. Agency for International Development (USAID) and through contributions to international organizations, primarily the United Nations Relief and Works Agency for Palestine Refugees in the Near East (UNRWA). The Department of State (State) oversees U.S. contributions to UNRWA. To help ensure that U.S. funds for these programs are not provided to individuals or entities engaged in terrorist activities, USAID and State must comply with restrictions under U.S. law. End-Stage Renal Disease: CMS Should Monitor Effect of Bundled Payment on Home Dialysis Utilization Rates

A letter report issued by the Government Accountability Office with an abstract that begins "Medicare covers dialysis--a process that removes excess fluids and toxins from the bloodstream--for most individuals with end-stage renal disease (ESRD), a condition of permanent kidney failure. Most patients with ESRD receive dialysis in a facility, while some patients with ESRD are trained to self-perform dialysis in their homes. The Centers for Medicare & Medicaid Services (CMS)--the agency that administers the Medicare program--has taken steps to encourage home dialysis and is in the process of changing the way it pays for dialysis services. Effective 2011, CMS will pay for dialysis services using an expanded bundled payment. The Tax Relief and Health Care Act of 2006 required GAO to report on the costs of home dialysis treatments and training. GAO examined (1) the extent to which the costs of home dialysis differ from the costs of dialysis received in a facility, and (2) CMS's plans to account for home dialysis costs in the expanded bundled payment. A letter report issued by the Government Accountability Office with an abstract that begins "The western United States, including vast stretches of federal land, is home to more than a third of the 1,317 species listed under the Endangered Species Act. Under section 7 of the act, federal agencies must ensure that any actions they authorize, fund, or carry out, whether on federal or private lands, do not jeopardize listed species. To fulfill this responsibility, the agencies often must formally consult with the Department of the Interior's U.S. Fish and Wildlife Service (Service), which issues a biological opinion assessing whether an action is likely to "take," or harm, a listed species. The Service may require the agencies to monitor and report on the action's effects on listed species, including take. For listed species subject to formal consultations in 11 western states, GAO was asked to examine the extent to which the Service tracks (1) required monitoring reports and (2) cumulative take. GAO reviewed the act, regulations, and policy and interviewed Service staff in all western states, reviewed 128 consultation files in five offices, and analyzed 23 listed species in detail."

Personnel Security Clearances: An Outcome-Focused Strategy Is Needed to Guide Implementation of the Reformed Clearance Process

A letter report issued by the Government Accountability Office with an abstract that begins "Personnel security clearances are used to verify that national security information--which in some cases could cause exceptionally grave damage to national security if disclosed--is entrusted only to those who have proven reliability and loyalty to the nation. In response to long-standing problems with timeliness and backlogs, Congress mandated clearance reforms as part of the Intelligence Reform and Terrorism Prevention Act of 2004 (IRTPA), and since 2005 the Department of Defense's (DOD) clearance program has remained on GAO's high-risk list despite improvements in timeliness. In 2007, a Joint Reform Team, led by the Office of Management and Budget (OMB), was established to improve the clearance process across the government. GAO was asked to review the extent to which reform efforts (1) align with key practices for organizational transformations and (2) address identified factors for reforming the personnel security clearance process. To assess these objectives, GAO compared joint reform reports to key transformation practices and essential factors for reform."

Aviation Security: TSA Has Completed Key Activities Associated with Implementing Secure Flight, but Additional Actions Are Needed to Mitigate Risks

A letter report issued by the Government Accountability Office with an abstract that begins "To enhance aviation security, the Department of Homeland Security's (DHS) Transportation Security Administration (TSA) developed a program--known as Secure Flight--to assume from air carriers the function of matching passenger information against terrorist watch-list records. In accordance with a mandate in the Department of Homeland Security Appropriations Act, 2008, GAO's objective was to assess the extent to which TSA met the requirements of 10 statutory conditions related to the development of the Secure Flight program. GAO is required to review the program until all 10 conditions are met. In September 2008, DHS certified that it had satisfied all 10 conditions. To address this objective, GAO (1) identified key activities related to each of the 10 conditions; (2) identified federal guidance and best practices that are relevant to successfully meeting each condition; (3) analyzed whether TSA had demonstrated, through program documentation and oral explanation, that the guidance was followed and best practices were met; and (4) assessed the risks associated with not fully following applicable guidance and meeting best practices."
