A letter report issued by the Government Accountability Office with an abstract that begins "The Medicare Improvements for Patients and Providers Act of 2008 directed the Secretary of Health and Human Services to develop a program to give physicians confidential feedback on the Medicare resources used to provide care to Medicare beneficiaries. GAO was asked to evaluate the per capita methodology for profiling physicians--a method which measures a patient's resource use over a fixed period of time and attributes that resource use to physicians--in order to assist the Centers for Medicare & Medicaid Services (CMS) with the development of a physician feedback approach. In response, this report examines (1) the extent to which physicians in selected specialties show stable practice patterns and how beneficiary utilization of services varies by physician resource use level; (2) factors to consider in developing feedback reports on physicians' performance, including per capita resource use; and (3) the extent to which feedback reports may influence physician behavior. GAO focused on four medical specialties and four metropolitan areas chosen for their geographic diversity and range in average Medicare spending per beneficiary. A letter report issued by the Government Accountability Office with an abstract that begins "The Food and Drug Administration (FDA) oversees the clinical investigators who conduct research involving new drugs, biologics, and medical devices to ensure that their conduct does not compromise the safety of clinical trial participants or the integrity of clinical trial data. FDA can debar or disqualify investigators who have engaged in misconduct such as submitting fraudulent data. Debarred or disqualified investigators cannot engage in certain activities related to clinical research. GAO was asked to review FDA's debarment and disqualification processes. GAO examined the length of time debarment and disqualification processes have taken and factors for those time frames, and the statutory and regulatory limitations of debarment and disqualification. GAO reviewed laws, regulations, and FDA files through November 5, 2008, for (1) all investigators, study coordinators, and sub-investigators for whom FDA pursued debarment since receiving debarment authority in 1992; and (2) all clinical investigators for whom FDA pursued disqualification since FDA adopted its current process for initiating proceedings in 1998."

Correspondence issued by the Government Accountability Office with an abstract that begins "Since 2001, Congress has provided the Department of Defense (DOD) with $893 billion in supplemental and annual appropriations, as of June 2009, primarily for Overseas Contingency Operations (OCO).1 DOD's reported annual obligations2 for OCO have shown a steady increase from about $0.2 billion in fiscal year 2001 to about $162.4 billion in fiscal year 2008. For fiscal year 2009, Congress appropriated $151 billion in war-related requests. A total of $89.1 billion has been obligated through the third quarter of fiscal year 2009 through June 2009. The United States' commitments to OCO will likely involve the continued investment of significant resources, requiring decision makers to consider difficult trade-offs as the nation faces an increasing long-range fiscal challenge. The magnitude of future costs will depend on several direct and indirect cost variables and, in some cases, decisions that have not yet been made. DOD's future costs will likely be affected by the pace and duration of operations, the types of facilities needed to support troops overseas, redeployment plans, and the amount of equipment to be repaired or replaced. After 7 years, DOD has incrementally fielded some capabilities, and, through fiscal year 2008, reported obligating about $96.5 million. GAO was asked to review the program including the extent that DOD has (1) effectively managed and overseen DRRS acquisition and deployment and (2) implemented features of DRRS consistent with legislative requirements and DOD guidance. GAO compared DRRS acquisition disciplines, such as requirements development, test management, and DRRS oversight activities, to DOD and related guidance, and reviewed the system's current and intended capabilities relative to legislative requirements and DOD guidance. A number of technical requirements and performance criteria can drive stellarator costs, e.g., tight tolerances, accurate coil positioning, low aspect ratio (compactness), choice of assembly strategy, metrology, and complexity of the stellarator coil geometry. With the completion of a seven-year design and construction effort of the National Compact Stellarator Experiment (NCSX) it is useful to interject the NCSX experience along with the collective experiences of the NCSX stellarator community to improving the stellarator configuration. Can improvements in maintenance be achieved by altering the stellarator magnet configuration with changes in the coil shape or with the combination of trim coils? Can a mechanical configuration be identified that incorporates a partial set of shaped fixed stellarator coils along with some removable coil set to enhance the overall machine maintenance? Are there other approaches that will simplify the concepts, improve access for maintenance, reduce overall cost and improve the reliability of a stellarator based power plant? Using ARIES-CS and NCSX as reference cases, alternative approaches have been studied and developed to show how these modifications would favorably impact the stellarator power plant and experimental projects. DeSlover ABSTRACT An outstanding problem that contributes considerable uncertainty to Global Climate Model (GCM) predictions of future climate is the characterization of ice particle sizes in cirrus clouds. Recent parameterizations of ice cloud effective diameter differ by a factor of three, which, for overcast conditions, often translate to changes in outgoing longwave radiation (OLR) of 55 W m-2 or more. Much of this uncertainty in cirrus particle sizes is related to the problem of ice particle shattering during in situ sampling of the ice particle size distribution (PSD). Ice particles often shatter into many smaller ice fragments upon collision with the rim of the probe inlet tube. These small ice artifacts are counted as real ice crystals, resulting in anomalously high concentrations of small ice crystals (D < 100 µm) and underestimates of the mean and effective size of the PSD. Half of the cirrus cloud optical depth calculated from these in situ measurements can be due to this shattering phenomenon. Design of 10 GeV laser wakefield accelerator stages with shaped laser modes

We present particle-in-cell simulations, using the VORPAL framework, of 10 GeV laser plasma wakefield accelerator stages. Scaling of the physical parameters with the plasma density allows us to perform these simulations at reasonable cost and to design high performance stages. In particular we show that, by choosing to operate in the quasi-linear regime, we can use higher order laser modes to tailor the focusing forces. This makes it possible to increase the matched electron beam radius and hence the total charge in the bunch while preserving the low bunch emittance required for applications.

Pacific Northwest National Laboratory (PNNL) has been tasked by Bechtel National Inc. (BNI) on the River Protection Project-Hanford Tank Waste Treatment and Immobilization Plant (RPP-WTP) project to perform research and development activities to resolve technical issues identified for the Pretreatment Facility (PTF). The Pretreatment Engineering Platform (PEP) was designed, constructed, and operated as part of a plan to respond to issue M12, "Undemonstrated Leaching Processes," of the External Flowsheet Review Team (EFRT) issue response plan.( ) The PEP is a 1/4.5-scale test platform designed to simulate the WTP pretreatment caustic leaching, oxidative leaching, ultrafiltration solids concentration, and slurry washing processes. The PEP replicates the WTP leaching processes using prototypic equipment and control strategies. A simplified flow diagram of the PEP system is shown in Figure 1.1. Two operating scenarios are currently being evaluated for the ultrafiltration process (UFP) and leaching operations. By November of 1997, Fluor Hanford (Fluor) had been the site manager of the Hanford nuclear reservation for a year. The Hanford site had been established as part of the Manhattan Project in the 1940s that gave birth to the atomic bomb. Hanford produced two thirds of U.S. plutonium during the Cold War period. The Hanford site was half the size of Rhode Island and occupied 586 square miles in southeastern Washington State. The production of plutonium for more than 40 years left a huge legacy of chemical and radiological contamination: 80 square miles of contaminated groundwater; 2,300 tons of spent nuclear fuel stored in underwater basins; 20 tons of plutonium-laced contaminated materials; and 500 contaminated facilities. The cleanup involved a challenging combination of radioactive material handling within an infrastructure constructed in the 1940s and 1950s. The cleanup that began in 1988 was expected to take 30 years or more. Improving safety at Hanford had already proven to be a significant challenge. As the new site manager at Hanford, Fluor Hanford inherited lower- and mid-level managers and thousands of unionized employees, many of whom were second or third generation Hanford employees. This technical report documents the results of laboratory geochemical and hydrologic measurements of sediments collected from new borehole 299-E13-65 (C7047) and comparison of the results with those of nearby borehole 299-13E-62 (C5923) both drilled in the BC Cribs and Trenches Area. The total and water-leachable concentrations of key contaminants will be used to update contaminant-distribution conceptual models and to provide more data for improving baseline risk predictions and remedial alternative selections. Improved understanding of subsurface conditions and methods to remediate these principal contaminants can be also used to evaluate the application of specific technologies to other contaminants across the Hanford Site.

One year into the Hanford contract, Fluor had learned a number of hard lessons very quickly. Although the Hanford remediation contract was in many ways a new endeavor for Fluor and a different kind of contract, the organization moved quickly to increase communication with all employees, attack head-on what it considered unsafe and inappropriate safety practices, and strongly inject its own corporate cultural beliefs into the Hanford organization. It wasn't easy, and it didn't happen overnight. From the beginning, Fluor established processes and programs to drive down injury rates. For example, whereas the previous contractor's approach to injuries had been passive, Fluor took a much more aggressive approach to worker injuries. The previous contractor had established a practice of sending injured workers home with the basic directive 'to come back when you are well'. Instead of using outsourced medical assessment, Fluor internalized it and evaluated all claims aggressively. Legitimate claims were quickly settled, and management moved to identify 'repeat offenders' when it came to reportable safety incidents. In the first year of Fluor's management, reportable injuries dropped from 5.37 to 2.99 per 200,000 man-hours. Despite the drop in injury rates, the safety record at Fluor Hanford was not at a …

Device-scale perpendicular alignment of colloidal nanorods

The self-assembly of nanocrystals enables new classes of materials whose properties are controlled by the periodicities of the assembly, as well as by the size, shape and composition of the nanocrystals. While self-assembly of spherical nanoparticles has advanced significantly in the last decade, assembly of rod-shaped nanocrystals has seen limited progress due to the requirement of orientational order. Here, the parameters critically relevant to self-assembly are systematically quantified using a combination of diffraction and theoretical modeling; these highlight the importance of kinetics on orientational order. Through drying-mediated self-assembly we achieve unprecedented control over orientational order (up to 96percent vertically oriented rods on 1cm2 areas) on a wide range of substrates (ITO, PEDOT:PSS, Si3N4). This opens new avenues for nanocrystal-based devices competitive with thin film devices, as problems of granularity can be tackled through crystallographic orientational control over macroscopic areas.

Small Business Demand Response with Communicating Thermostats: SMUD's Summer Solutions Research Pilot

This report documents a field study of 78 small commercial customers in the Sacramento Municipal Utility District service territory who volunteered for an integrated energy-efficiency/demand-response (EE-DR) program in the summer of 2008. The original objective for the pilot was to provide a better understanding of demand response issues in the small commercial sector. Early findings justified a focus on offering small businesses (1) help with the energy efficiency of their buildings in exchange for occasional load shed, and (2) a portfolio of options to meet the needs of a diverse customer sector. To meet these expressed needs, the research pilot provided on-site energy efficiency advice and offered participants several program options, including the choice of either a dynamic rate or monthly payment for air-conditioning setpoint control. An analysis of hourly load data indicates that the offices and retail stores in our sample provided significant demand response, while the restaurants did not. Thermostat data provides further evidence that restaurants attempted to precool and reduce AC service during event hours, but were unable to because their air-conditioning units were undersized. On a 100 F reference day, load impacts of all participants during events averaged 14%, while load impacts of office and retail …

Final Report for "Modeling Electron Cloud Diagnostics for High-Intensity Proton Accelerators"

Electron clouds in accelerators such as the ILC degrade beam quality and limit operating efficiency. The need to mitigate electron clouds has a direct impact on the design and operation of these accelerators, translating into increased cost and reduced performance. Diagnostic techniques for measuring electron clouds in accelerating cavities are needed to provide an assessment of electron cloud evolution and mitigation. Accurate numerical modeling of these diagnostics is needed to validate the experimental techniques. In this Phase I, we developed detailed numerical models of microwave propagation through electron clouds in accelerating cavities with geometries relevant to existing and future high-intensity proton accelerators such as Project X and the ILC. Our numerical techniques and simulation results from the Phase I showed that there was a high probability of success in measuring both the evolution of electron clouds and the effects of non-uniform electron density distributions in Phase II.

Technosocial Modeling for Determining the Status and Nature of a State’s Nuclear Activities

The International Atomic Energy Agency State Evaluation Process: The Role of Information Analysis in Reaching Safeguards Conclusions (Mathews et al. 2008), several examples of nonproliferation models using analytical software were developed that may assist the IAEA with collecting, visualizing, analyzing, and reporting information in support of the State Evaluation Process. This paper focuses on one of the examples a set of models developed in the Proactive Scenario Production, Evidence Collection, and Testing (ProSPECT) software that evaluates the status and nature of a state’s nuclear activities. The models use three distinct subject areas to perform this assessment: the presence of nuclear activities, the consistency of those nuclear activities with national nuclear energy goals, and the geopolitical context in which those nuclear activities are taking place. As a proof-of-concept for the models, a crude case study was performed. The study, which attempted to evaluate the nuclear activities taking place in Syria prior to September 2007, yielded illustrative, yet inconclusive, results. Due to the inconclusive nature of the case study results, changes that may improve the model’s efficiency and accuracy are proposed.

CH2M Hill Plateau Remediation Company (CHPRC) requested the services of the Pacific Northwest National Laboratory (PNNL) to provide technical support for the Remediation Decision Support (RDS) activity within the Soil & Groundwater Remediation Project. A portion of the support provided in FY2009, was to extend the soil unsaturated hydraulic conductivity using an alternative approach. This alternative approach incorporates the Brooks and Corey (1964), van Genuchten (1980), and a modified van Genuchten water-retention models into the interfacial-area-based relative permeability model presented by Embid (1997). The general performance of the incorporated models is shown using typical hydraulic parameters. The relative permeability models for the wetting phase were further examined using data from literature. Results indicate that the interfacial-area-based model can describe the relative permeability of the wetting phase reasonably well.

Total aerosol effect: forcing or radiative flux perturbation?

Uncertainties in aerosol forcings, especially those associated with clouds, contribute to a large extent to uncertainties in the total anthropogenic forcing. The interaction of aerosols with clouds and radiation introduces feedbacks which can affect the rate of rain formation. Traditionally these feedbacks were not included in estimates of total aerosol forcing. Here we argue that they should be included because these feedbacks act quickly compared with the time scale of global warming. We show that for different forcing agents (aerosols and greenhouse gases) the radiative forcings as traditionally defined agree rather well with estimates from a method, here referred to as radiative flux perturbations (RFP), that takes these fast feedbacks and interactions into account. Thus we propose replacing the direct and indirect aerosol forcing in the IPCC forcing chart with RFP estimates. This implies that it is better to evaluate the total anthropogenic aerosol effect as a whole.

Under funding from the Department of Energy Office of Nuclear Energy’s Materials, Protection, Accounting, and Control for Transmutation (MPACT) program (formerly the Advanced Fuel Cycle Initiative Safeguards Campaign), Pacific Northwest National Laboratory (PNNL) and Los Alamos National Laboratory (LANL) are collaborating to study the viability of lead slowing-down spectroscopy (LSDS) for spent-fuel assay. Based on the results of previous simulation studies conducted by PNNL and LANL to estimate potential LSDS performance, a more comprehensive study of LSDS viability has been defined. That study includes benchmarking measurements, development and testing of key enabling instrumentation, and continued study of time-spectra analysis methods. This report satisfies the requirements for a PNNL/LANL deliverable that describes the objectives, plans and contributing organizations for a comprehensive three-year study of LSDS for spent-fuel assay. This deliverable was generated largely during the LSDS workshop held on August 25-26, 2009 at Rensselaer Polytechnic Institute (RPI). The workshop itself was a prominent milestone in the FY09 MPACT project and is also described within this report.
