Health Reform Implementation Analysis: A Guide to Policy Development for Geriatric Care Planning, Integration and Evaluation

In the context of health care delivery for senior citizens, this research utilizes three studies that examine the development and implementation of health policy and the factors that can directly or indirectly impact the effective delivery of health services to senior citizens. It utilizes three essays employing mixed methods (qualitative and quantitative) methods including semi structured interviews, multiple regression and partial least squares structural equation modelling to examine the extent to which the implementation of health services delivered attributes of primary and integrated care to seniors. The two essays identified methods, approaches and strategies of integrated care relevant to the development of policy that can be successfully implemented when the contextual issues that older people consider to be important in maintaining their functional capabilities and their motivation to improve health as perceived by them are addressed. Consequent upon the results from these studies, the third essay examines the methodological issues on integrated geriatric care implementation when guidelines for effective policy development identified were not followed. Stimulus Control Effects of Changes in Schedules of Reinforcement

Sometimes, changes in consequences are accompanied by a clear stimulus change explicitly arranged by the experimenter. Other times when new consequences are in effect, there is little or no accompanying stimulus change explicitly arranged by the experimenter. These differences can be seen in the laboratory as multiple (signaled) schedules and mixed (unsignaled) schedules. The current study used college students and a single-subject design to examine the effects of introducing signaled and unsignaled schedules, and the transitions between them. In one phase, a card was flipped from purple to white every time the schedule was switched from VR-3 to FT-10. In another phase, the schedule still changed periodically, but the card always remained on the purple side. Results showed that the participants' responding was controlled by the schedule of reinforcement, by the color of the card, or both. These results suggest that changes in patterns of reinforcement lead to changes in stimulus control. In addition, the stimulus control for a behavior can come from several different sources. During teaching, it may facilitate the development of stimulus control to change the environment when a new behavior is required.

Impact of Anti-S2 Peptides on a Variety of Muscle Myosin S2 Isoforms and Hypertrophic Cardiomyopathy Mutants Revealed by Fluorescence Resonance Energy Transfer and Gravitational Force Spectroscopy

Myosin subfragment-2 (S2) is an intrinsically unstable coiled coil. This dissertation tests if the mechanical stability of myosin S2 would influence the availability of myosin S1 heads to actin thin filaments. The elevated instability in myosin S2 coiled coil could be one of the causes for hypercontractility in Familial Hypertrophic Cardiomyopathy (FHC). As hypothesized FHC mutations, namely E924K and E930del, in myosin S2 displayed an unstable myosin S2 coiled coil compared to wild type as measured by Fluorescence Resonant Energy Transfer (FRET) and gravitational force spectroscopy (GFS). To remedy this, anti-S2 peptides; the stabilizer and the destabilizer peptides by namesake were designed in our lab to increase and decrease the stability of myosin S2 coiled coil to influence the actomyosin interaction. Firstly, the effectiveness of anti-S2 peptides were tested on muscle myosin S2 peptides across MYH11 (smooth), MYH7 (cardiac), and MYH2 (skeletal) with GFS and FRET. The results demonstrated that the mechanical stability was increased by the stabilizer and decreased by the destabilizer across the cardiac and skeletal myosin S2 isoform but not for the smooth muscle isoform. Adolescent Self-Theories of Singing Ability within the Choral Hierarchy

The purpose of this study was to explore adolescent self-views of singing ability through both implicit theories and self-concept meaning systems. A secondary purpose of this study was to examine these self-views specifically in the context of a choral hierarchy. Using a researcher-designed survey instrument, I gathered data from middle- and high-school students currently enrolled in a choir program organized in a hierarchical structure. I analyzed descriptive statistics of survey responses to items designed to measure implicit theories of singing ability, singing self-concept, and goal orientation. I also examined differences among participants by ensemble placement in implicit theory and self-concept scores, correlation between implicit theory and self-concept, and whether implicit theory, self-concept, goal orientation, or current enrollment could predict future enrollment decisions. In addition to these quantitative measures, I coded open-ended responses to two failure scenarios and examined participant responses by ensemble and gender. Both implicit theory and self-concept scores were higher for participants at the top of the choral hierarchy than at the bottom. Open-ended responses, however, did not align with the implicit theory scale and a number of students presented a false growth mindset. Exploring the Impact of Decentralization of Decision Making and Complexity on Supply Chain Resilience

The purpose of this three-essay dissertation is to synthesize and extend the effects of decentralization in decision-making and supply chain complexity in the context of supply chain resilience (SCRES).First essay contributes to theory and practice by expanding resilience thinking into including supply chain orientation and organizational structure and their implications and also responds to prior research arguing for the importance of identifying organizational factors that improve supply chain resilience. Second essay contributes to the supply chain organizational structure and SCRES literature by not just providing empirical support for decentralization of decision making in times of disruptions but more precisely by showing the factors that either impede or facilitate decentralization at the organizational level. Understanding the interplay among these factors is critical to explaining the lack of success for decentralization in the context of SCRES. Third essay contributes to practice by reviewing some of the major complexity drivers present in the supply chains and providing strategies along with a four-step process that practitioners can use to manage complexity.

Multifactorial Determinants of Change in Mental Disorder and Happiness among Older Americans

Mental health is an intrinsic capability that constitute an essential component of healthy aging. Mental health is constituted by positive constructs such as happiness and negative constructs (mental disorders) such as depression. As people grow older, they become more prone to developing mental disorders which are linked to poorer quality of life, increased disability, increased utilization and cost of health services, and higher rates of suicide. This dissertation involved three studies that focused on factors that predict change in mental disorders and happiness of older Americans over a period of five years. Two waves of publicly available national representative data from the National Social Life Health and Aging Project (NSHAP) collected in 2010 and 2015 were used. A total of 2210 older adults within the ages 62-90 years were used in the analyses. Mental health measures were CES-Depression scale, HADS anxiety scale, and self-rated happiness. Essay 1 aimed to identify the important aspects of older adults' interaction with their neighborhood that predict the presence of mental disorder and happiness. The Role of Eigenvalues of Parity Check Matrix in Low-Density Parity Check Codes

The new developments in coding theory research have revolutionized the application of coding to practical systems. Low-Density Parity Check (LDPC) codes form a class of Shannon limit approaching codes opted for digital communication systems that require high reliability. This thesis investigates the underlying relationship between the spectral properties of the parity check matrix and LDPC decoding convergence. The bit error rate of an LDPC code is plotted for the parity check matrix that has different Second Smallest Eigenvalue Modulus (SSEM) of its corresponding Laplacian matrix. It is found that for a given (n,k) LDPC code, large SSEM has better error floor performance than low SSEM. The value of SSEM decreases as the sparseness in a parity-check matrix is increased. It was also found from the simulation that long LDPC codes have better error floor performance than short codes. This thesis outlines an approach to analyze LDPC decoding based on the eigenvalue analysis of the corresponding parity check matrix.

Identification, Characterization and Engineering of UDP-Glucuronosyltransferases for Synthesis of Flavonoid Glucuronides

Flavonoids are polyphenolics compounds that constitute a major group of plant specialized metabolites, biosynthesized via the phenylpropanoid/polymalonate pathways. The resulting specialized metabolites can be due to decoration of flavonoid compounds with sugars, usually glucose, by the action of regiospecific UDP-glycosyltransferase (UGT) enzymes. In some cases, glycosylation can involve enzymatic attachment of other sugar moieties, such as glucuronic acid, galactose, rhamnose or arabinose. These modifications facilitate or impact the bioactivity, stability, solubility, bioavailability and taste of the resulting flavonoid metabolites. The present work shows the limitations of utilizing mammalian UDP-glucuronosyltransferases (UGATs) for flavonoid glucuronidation, and then proceeds to investigate plant UG(A)T candidates from the model legume Medicago truncatula for glucuronidating brain-targeted flavonoid metabolites that have shown potential in neurological protection. We identified and characterized several UG(A)T candidates from M. truncatula which efficiently glycosylate various flavonoids compounds with different/multiple regiospecificities. Biochemical characterization identified one enzyme, UGT84F9, that efficiently glucuronidates a range of flavonoid compounds in vitro. In addition, examination of the ugt84f9 gene knock-out mutation in M. truncatula indicates that UGT84F9 is the major UG(A)T enzyme that is necessary and sufficient for attaching glucuronic acid to flavonoid aglycones, particularly flavones, in this species. Traffic-Generated Air Pollution-Exposure Mediated Expression of Factors Associated with Progression of Multiple Sclerosis in a Female Polipoprotein E Knockout Mouse Model

Environmental air pollution is one risk factor associated with the onset and progression of multiple sclerosis (MS). In this project, we investigated the effects of ubiquitous traffic-generated pollutants, namely a mixture of gasoline and diesel vehicle exhaust (MVE), on signaling pathways associated with the pathophysiology of MS in the central nervous system (CNS) of either ovary intact (ov+) or ovariectomized (ov-) female Apolipoprotein (Apo) E-/-. Specifically, we investigated whether a subchronic inhalation exposure to MVE (200 PM μg/m3; 6 hr/d, 7d/wk, 30d) vs. filtered air (FA) controls altered myelination, T cell infiltration, blood-brain barrier (BBB) integrity, or production of reactive oxygen species (ROS) and expression of neuroinflammation markers in the CNS ov+ and ov- Apo E-/- mice. Our results revealed that inhalation exposure to MVE resulted in increased demyelination and CD4+ and CD8+ T cell infiltration, associated with alterations in BBB integrity. Disruption of the BBB was evidenced by decreased tight junction (TJ) protein expression, increased matrix metalloproteinase (MMPs) activity, and increased permeability of immunoglobin (Ig) G, which were more pronounced in the MVE ov- group. Military Religio: Caesar's Religiosity Vindicated by Warfare

Gaius Julius Caesar remains one of the most studied characters of antiquity. His personality, political career, and military campaigns have garnered numerous scholarly treatments, as have his alleged aspirations to monarchy and divinity. However, comparatively little detailed work has been done to examine his own personal religiosity and even less attention has been paid to his religion in the context of his military conquests. I argue that Caesar has wrongly been deemed irreligious or skeptical and that his conduct while on campaign demonstrates that he was a religious man. Within the Roman system of religion, ritual participation was more important than faith or belief. Caesar pragmatically manipulated the Romans' flexible religious framework to secure military advantage almost entirely within the accepted bounds of religious conduct. If strict observance of ritual was the measure of Roman religiosity, then Caesar exceeded the religious expectations of his rank and office. The evidence reveals that he was an exemplar of Roman religio throughout both the Gallic Wars (58-51BC) and the subsequent Civil Wars (49-45BC).

Analysis of the Cytochrome P450 and UDP-Glucuronosyltransferase Families and Vitamin D3- Supplementation in Anoxia Survival in Caenorhabditis elegans

Alteration in diet and knockdown of detoxification genes impacts the response of C. elegans to oxygen deprivation stress. I hypothesized that feeding worms a vitamin D3-supplementation diet would result in differential oxygen deprivation stress response. We used a combination of wet lab and transcriptomics approach to investigate the effect of a vitamin-D3 supplemented diet on the global gene expression changes and the anoxia response phenotype of C. elegans (Chapter 2). C. elegans genome consists of 143 detoxification genes (cyp and ugt). The presence of a significant number of genes in these detoxification families was a challenge with identifying and selecting specific cyp and ugt genes for detailed analysis. Our goal was to understand the evolution, phylogenetic, and expression of the detoxification enzymes CYPs and UGTs in C. elegans (Chapter 3). We undertook a phylogenetic and bioinformatics approach to analyze the C. elegans, detoxification family. Phylogenetic analysis provided insight into the association of the human and C. elegans xenobiotic/endobiotic detoxification system. In light of incidents and concerns regarding the vulnerability of the global positioning system (GPS), the main purpose of the thesis is to look at alternative systems for radio guidance and to put up a serious study on such alternatives with receive and transmit antenna. There is also the need to design such antennas with multiple frequencies to offer robustness in the unlikely event that such adversarial attacks on the GPS happen. The basis on which such alternative antennas are designed is a slotted microstrip. The characteristics of the slot or slots on the microstrip are analyzed by mapping their exact locations on the patch and then noting the resultant center frequencies, the return losses, and the bandwidth. The activities associated with this also focus on the design, fabrication, validation, and characterization of one or more slotted antennas prototypes. The measurement of the antenna prototypes does confirm several frequencies that coexist to see applications, in aeronautical radionavigation, fixed-mobile radionavigation, and radiolocation. Intersecting Identities and Conflict as Moderators of the Relationship Between Discrimination and Mental Health in Emerging Adulthood

Individuals with a minority sexual identity, such as lesbians, gay men, and bisexuals (LGB) face increased risk for stigmatization surrounding their sexual identities and subsequent psychological distress. Sexual minorities of color (SMOC) face the same difficulties faced by White sexual minorities, often compounded with stigma and discrimination linked to their racial/ethnic identities. However, because SMOC remain underrepresented in research on LGB issues, empirically-driven knowledge about these groups is lacking, even among outcomes where noted disparities exist, such as depression. Emerging adulthood may be a particularly important period for understanding effects of intersectional identities and discrimination among SMOC, who often navigate identity-related milestones and experiences independently for the first time within this developmental period. This study examined the relationships between discrimination based upon racial/ethnic and sexual intersecting identities and depression symptoms among emerging adults, as well as ways that group identity factors (ethnic identity, sexual identity, conflicts in identity allegiances) moderated this relationship. Findings indicated that experience of intersectional discrimination was strongly, positively related to depression symptoms. Ethnic identity negatively related to depression independently, but not in the regression model accounting for other variables. Resurrection Attempts: Essays

This dissertation is composed of a critical preface, "Reconciling Art and Account in the Creative Essay," and the essay collection Resurrection Attempts: Essays. The preface situates the following essay collection within the genre of contemporary creative nonfiction. Specifically, it argues that genre-bending or genre hybridity are inherent and unavoidable features of creative nonfiction writing and should be celebrated, rather than denied or lamented. It points to other writers who deliberately challenge the bounds of genre, and discusses some of the collection's innovations in form and other ways it offers experimentation, such as use of unusual or borrowed points of view, disruption of chronology, and adoption of elements from other genres of writing, including fiction, poetry, and academic. Ultimately, embracing the artistic side of creative nonfiction (as opposed to its "purely" journalistic side) allows for heightened intimacy with the reader, a much wider breadth of storytelling, and a more vulnerable—and therefore more truthful—interrogation of legacy and the human experience. Novel Approaches for Enhancing Resistance to Fusarium graminearum in Arabidopsis and Wheat by Targeting Defense and Pathogenicity Factors

Fusarium head blight (FHB) is an important disease of small grain cereals including wheat that affects grain quality and yield. The fungus Fusarium graminearum (Fg) is the major agent of this disease. Lack of natural resistance has limited ability to control wheat losses to this disease. Developing new approaches is critical for increasing host plant resistance to this fungus. This work has identified four processes that can be targeted for enhancing host plant resistance to FHB. The first involves targeting the pattern-triggered immunity mechanism to promote host plant resistance. Two other approaches involved reducing activity of susceptibility factors in the host to enhance plant resistance. The susceptibility factors targeted include accumulation of the phytohormone jasmonic acid and the 9-lipoxygenase pathway that oxidizes fatty acids. Besides suppressing host defenses against Fg, jasmonic acid also directly acts on the fungus to promote fungal growth. 9- lipoxygenases similarly suppress host defenses to promote fungal pathogenicity. Another approach that was developed involved having the plant express double stranded RNA to target fungal virulence genes for silencing. Traffic Forecasting Applications Using Crowdsourced Traffic Reports and Deep Learning

Intelligent transportation systems (ITS) are essential tools for traffic planning, analysis, and forecasting that can utilize the huge amount of traffic data available nowadays. In this work, we aggregated detailed traffic flow sensor data, Waze reports, OpenStreetMap (OSM) features, and weather data, from California Bay Area for 6 months. Using that data, we studied three novel ITS applications using convolutional neural networks (CNNs) and recurrent neural networks (RNNs). The first experiment is an analysis of the relation between roadway shapes and accident occurrence, where results show that the speed limit and number of lanes are significant predictors for major accidents on highways. The second experiment presents a novel method for forecasting congestion severity using crowdsourced data only (Waze, OSM, and weather), without the need for traffic sensor data. The third experiment studies the improvement of traffic flow forecasting using accidents, number of lanes, weather, and time-related features, where results show significant performance improvements when the additional features where used.

Transforming Art Education in Saudi Arabia: Inclusion of Social Issues in Art Education

The purpose of this study was to describe in-service Saudi Arabian art education teachers' (a) philosophies of education before and after an issues-based art education (IBAE) workshop, (b) perceptions of the IBAE workshop and its products, (c) perception of the importance of IBAE approach, reasons, topics, and challenges after the workshop, and (d) perception of IBAE within the Saudi Arabian context. A mixed-methods approach was followed. The qualitative portion of the study utilized a post-workshop questionnaire and reflective essay completed by 37 participants, and personal teaching journal protocol and focus group discussion from 18 participants who attended the workshop and implemented the IBAE lesson in their classrooms. Analysis of the data confirmed that art teachers' perspectives towards IBAE positively increased after attending the IBAE workshop. Specifically, in the quantitative findings, participants indicated a positive attitude toward the teaching philosophy of social reconstruction after the IBAE workshop. These results demonstrate that the tenets of social reconstruction align with the IBAE approach. In addition, the quantitative data suggests that teacher participants' overall perception of the importance of addressing social issues in the art curriculum was positive, with 91.9% of respondents agreeing. Achievement Motivation Theory as a Model for Explaining College Athletes' Grit

This study examined the direct and indirect associations of the perceived coach-created motivational climate, athletes' implicit theory, and athletes' achievement goal orientation in relation to their levels of grit. Five hundred twenty-three Division I and II collegiate student-athletes (male = 246, female = 277) from five institutions across the south-central and southeast areas of the United States completed self-report measures assessing the previously described constructs. The measurement model fit the data well (SRMR = .055; CFI = .938; RMSEA = .067) and demonstrated invariance across the male and female athletes. The structural model demonstrated adequate fit (SRMR = .088; CFI = .918; RMSEA = .068). All direct and indirect paths in the model were significant in the expected direction, with the exception of the direct path from entity theory to ego goal orientation and the indirect path from ego-involving climate to ego goal orientation through entity theory, which were both non-significant. The mastery-incremental constructs accounted for 65% of the variance in grit. Results indicate that achievement motivation theory is an appropriate framework through which to examine grit, and achievement motivation constructs may be significant antecedents of grit's development. Limitations and future directions are discussed.

Role of 5.8S rRNA in Zebrafish and Human Blood Coagulation

Hemolytic disorders are characterized by hemolysis and are prone to thrombosis. Previously, it has been shown that the RNA released from damaged blood cells activates clotting. However, the nature of RNA released from hemolysis is still elusive. We found that after hemolysis, the red blood cells from both zebrafish and humans release 5.8S rRNA. This RNA activated coagulation in zebrafish and human plasmas. Using both natural and synthetic 5.8S rRNA and its synthetic truncated fragments, we found that the 3'-end 26 nucleotide-long RNA (3'-26 RNA) and its stem-loop secondary structure were necessary and sufficient for clotting activity. Corn trypsin inhibitor (CTI), a coagulation factor XII (FXII) inhibitor blocked 3'-26 RNA-mediated coagulation activation of both zebrafish and human plasma. CTI also inhibited zebrafish coagulation in vivo. 5.8S rRNA monoclonal antibody inhibited both 5.8S rRNA- and 3'-26 RNA-mediated zebrafish coagulation activity. Both 5.8S rRNA and 3'-26 RNA activates normal human plasma but did not activate FXII-deficient human plasma. Taken together, these results suggested that the activation of zebrafish plasma is via FXII-like protein. Immigrant Children's Perspectives of Books that Share Stories of Early School Experiences

Guided by the importance of children's voices and perspectives, this study aims at finding the immigrant children's perspectives of books that share stories of early school experiences of immigrant children. Before working with children, there was a careful selection process and analyzing of the three picture books chosen for the study using critical content analysis and childism lenses. The participants are three Arab immigrant children at the age of 6 who are bilingual and attended school in the U.S for one year, at least. With acknowledgement to reader-response theory, the data collection process started with an introductory home visit, followed by three individual interactive read-aloud sessions using interviews, audio records, and observations. The data collection involved field notes of non-verbal responses of the participants and these notes supported analysis of the eight transcripts. Thematic analysis is used in analyzing the data of each story, followed by identifying finding themes across all three stories. A Genetic Approach to Identify Proteins that Interact with Eukaryotic Microtubule Severing Proteins via a Yeast Two Hybrid System

Microtubules (MT) are regulated by multiple categories of proteins, including proteins responsible for severing MTs that are therefore called MT-severing proteins. Studies of katanin, spastin, and fidgetin in animal systems have clarified that these proteins are MT-severing. However, studies in plants have been limited to katanin p60, and little is known about spastin or fidgetin and their function in plants. I looked at plant genomes to identify MT-severing protein homologues to clarify which severing proteins exist in plants. I obtained data from a variety of eukaryotic species to look for MT-severing proteins using homology to human proteins and analyzed these protein sequences to obtain information on the evolution of MT-severing proteins in different species. I focused this analysis on MT-severing proteins in the maize and Arabidopsis thaliana genomes. I created evolutionary phylogenetic trees for katanin-p60, katanin-p80, spastin, and fidgetin using sequences from animal, plant, and fungal genomes. I focused on Arabidopsis spastin and worked to understand its functionality by identifying protein interaction partners. The yeast two-hybrid technique was used to screen an Arabidopsis cDNA library to identify putative spastin interactors. Interorganizational Collaboration in Implementing Urban Greening Policies in Saudi Arabia: An Institutional Collective Action Framework

This dissertation aims to examine the relationship between interorganizational collaboration and the implementation of urban greening policy. Specifically, it discusses bonding and bridging relationships that explain a successful interorganizational collaboration, and to what extent these factors explain the perception of success in the implementation of public programs. The effects of risks of collaboration on the implementation of urban greening policy are also studied. To frame the analysis, this dissertation uses Feiock's institutional collective action (ICA) framework, which aims to understand successful interorganizational collaboration and policy implementation across sectors. The collection of data was carried out in Riyadh City in Saudi Arabia, which is located in the center of Saudi Arabia. In this study, the unit of analysis is the networks of relationships among organizations that work with the Royal Commission for Riyadh City (RCRC). The data were obtained from 44 organizations collaborating to implement urban greening projects in Riyadh City and were collected over 17 days from June 15, 2019 to July 2, 2019. Third-Party Perception: Implications for Governance and Communication of Health Risks during the Umrah in Saudi Arabia

The current study projects the third-person perception phenomenon into the area of emergency management, specifically regarding risk communication in the context of religious gatherings. This study utilized the Umrah religious gathering in Makkah, Saudi Arabia, during summer 2019 as a case study (N = 257). This study aimed to investigate whether pilgrims perceive there was a greater effect of health information on others than on themselves. Survey results were translated and then coded and analyzed statistically using SPSS software. The findings indicated that third-person perception existed among pilgrims. Specifically, the perception of pilgrims that the influence of news about MERS-CoV, believed to be undesirable in its effect on themselves, was greater on others than on themselves was found statistically significant. Further, the findings indicated that the more pilgrims watched, listened to, or read news about MERS-CoV, the larger the effect of the news they perceived on themselves and others was. Thus, exposure to MERS-CoV news did not increase, but rather decreased the perception of difference between self and others. Also, the empirical findings indicated that pilgrims who were knowledgeable about MERS-CoV could relate to the coverage. Thermal Transport Modeling in Three-Dimensional Pillared-Graphene Structures for Efficient Heat Removal

Pillared-graphene structure (PGS) is a novel three-dimensional structure consists of parallel graphene sheets that are separated by carbon nanotube (CNT) pillars that is proposed for efficient thermal management of electronics. For microscale simulations, finite element analyses were carried out by imposing a heat flux on several PGS configurations using a Gaussian pulse. The temperature gradient and distribution in the structures was evaluated to determine the optimum design for heat transfer. The microscale simulations also included conducting a mesh-independent study to determine the optimal mesh element size and shape. For nanoscale simulations, Scienomics MAPS software (Materials And Processes Simulator) along with LAMMPS (Large-scale Atomic/ Molecular Massively Parallel Simulator) were used to calculate the thermal conductivity of different configurations and sizes of PGS. The first part of this research included investigating PGS when purely made of carbon atoms using non-equilibrium molecular dynamics (NEMD). The second part included investigating the structure when supported by a copper foil (or substrate); mimicking production of PGS on copper. The micro- and nano-scale simulations show that PGS has a great potential to manage heat in micro and nanoelectronics.
