Federal Employees Health Benefits Program: First-Year Experience with High-Deductible Health Plans and Health Savings Accounts

A letter report issued by the Government Accountability Office with an abstract that begins "The Federal Employees Health Benefits Program (FEHBP) recently began offering high-deductible health plans (HDHP) coupled with tax-advantaged health savings accounts (HSA) that enrollees use to pay for health care. Unused HSA balances may accumulate for future use, providing enrollees an incentive to purchase health care prudently. The plans also provide decision support tools to help enrollees make purchase decisions, including health care quality and cost information. Concerns have been expressed that HDHPs coupled with HSAs may attract younger, healthier, or wealthier enrollees, leaving older, less healthy enrollees to drive up costs in traditional plans. Because the plans are new, there is also interest in the plan features and the decision support tools they provide to enrollees. GAO was asked to evaluate the experience of the 14 HDHPs coupled with an HSA that were first offered under the FEHBP in January 2005. GAO compared the characteristics of enrollees in the 14 HDHPs to those of enrollees in another recently introduced (new) plan without a high deductible and to all FEHBP plans. Information on False Claims Act Litigation

Correspondence issued by the Government Accountability Office with an abstract that begins "The False Claims Act (FCA) is one of the government's primary weapons to fight fraud against the government. The Act, as amended in 1986, provides for penalties and triple damages for anyone who knowingly submits or causes the submission of false or fraudulent claims to the United States for government funds or property. Under the FCA's qui tam provisions, a person with evidence of fraud, also known as a whistle blower or relator, is authorized to file a case in federal court and sue, on behalf of the government, persons engaged in the fraud and to share in any money the government may recover. The Department of Justice (DOJ) has the responsibility to decide on behalf of the government whether to join the whistle blower in prosecuting these cases. From fiscal years 1987 through 2005, settlements and judgments for the federal government in FCA cases have exceeded $15 billion, of which $9.6 billion, or 64 percent, was for cases filed by whistle blowers under FCA's qui tam provisions. This report discusses (1) the processes and events that surrounded the Air Force's development of its 20-year force structure plan, including the involvement of key stakeholders and the development of the Guard's Vanguard initiative, and (2) the extent to which the Air Force is utilizing key results-oriented management tools to guide its effort to identify new missions for the Air National Guard and integrate active and Guard forces as part of its Future Total Force effort."

A letter report issued by the Government Accountability Office with an abstract that begins "The Medicare Modernization Act of 2003 directed that hospitals lose 0.4 percent of their Medicare payment update if they do not submit clinical data for both Medicare and non-Medicare patients needed to calculate hospital performance on 10 quality measures. The Centers for Medicare & Medicaid Services (CMS) instituted the Annual Payment Update (APU) program to collect these data from hospitals and report their rates on the measures on its Hospital Compare Web site. For hospital quality data to be useful to patients and other users, they need to be reliable, that is, accurate and complete. GAO was asked to (1) describe the processes CMS uses to ensure the accuracy and completeness of data submitted for the APU program, (2) analyze the results of CMS's audit of the accuracy of data from the program's first two calendar quarters, and (3) describe processes used by seven other organizations that assess the accuracy and completeness of clinical performance data."

FreedomCAR and vehicle technologies heavy vehicle program FY 2006. Benefits analysis : methodology and results - final report.

This report describes the approach to estimating benefits and the analysis results for the Heavy Vehicle Technologies activities of the Freedom Car and Vehicle Technologies (FCVT) Program of EERE. The scope of the effort includes: (1) Characterizing baseline and advanced technology vehicles for Class 3-6 and Class 7 and 8 trucks, (2) Identification of technology goals associated with the DOE EERE programs, (3) Estimating the market potential of technologies that improve fuel efficiency and/or use alternative fuels, (4) Determining the petroleum and greenhouse gas emissions reductions associated with the advanced technologies. In FY 05 the Heavy Vehicles program activity expanded its technical involvement to more broadly address various sources of energy loss as compared to focusing more narrowly on engine efficiency and alternative fuels. This broadening of focus has continued in the activities planned for FY 06. These changes are the result of a planning effort that occurred during FY 04 and 05. Summary of DOE/PERF water program review.

For many years, the U.S. Department of Energy (DOE) has supported and sponsored various types of water research relating to the oil and gas industry through its Office of Fossil Energy and its National Energy Technology Laboratory (NETL). In early 2005, the Petroleum Environmental Research Forum (PERF) submitted a proposal to DOE for funding an upcoming PERF meeting that would feature water research in the petroleum industry. PERF is a nonprofit organization created in 1986 to provide a stimulus to and a forum for the collection, exchange, and analysis of research information related to the development of technology concerning the petroleum industry, and a mechanism for establishing joint research projects in that field. Additional information on PERF can be accessed at http://www.perf.org. DOE agreed to provide funding to hold a review of its water research program in conjunction with the fall 2005 PERF meeting. Argonne National Laboratory (Argonne) was asked to coordinate and host the meeting, which was referred to as the DOE/PERF Water Program Review. Causes of Ocean Surface temperature Changes in Atlantic andPacific Topical Cyclogenesis Regions

Previous research has identified links between changes in sea surface temperature (SST) and hurricane intensity. We use climate models to study the possible causes of SST changes in Atlantic and Pacific tropical cyclogenesis regions. The observed SST increases in these regions range from 0.32 to 0.67 C over the 20th century. The 22 climate models examined here suggest that century-timescale SST changes of this magnitude cannot be explained solely by unforced variability of the climate system, even under conservative assumptions regarding the magnitude of this variability. Model simulations that include external forcing by combined anthropogenic and natural factors are generally capable of replicating observed SST changes in both tropical cyclogenesis regions.

On September 8-9, 2005, representatives of the Kansas Department of Health and Environment (KDHE), the Commodity Credit Corporation of the U.S. Department of Agriculture (CCC/USDA), and Argonne National Laboratory met at the KDHE's offices in Topeka to review the status of the CCC/USDA's environmental activities in Kansas. A key CCC/USDA goal for this meeting was to obtain KDHE input on the selection of possible remedial approaches to be examined as part of the Corrective Action Study (CAS) for this site. As a result of the September meeting, the KDHE recommended several additional activities for the Everest site, to further assist in selecting and evaluating remedial alternatives for the CAS. The requested actions included the following: (1) Construction of several additional interpretive cross sections to improve the depiction of the hydrogeologic characteristics affecting groundwater and contaminant movement along the apparent main plume migration pathway to the north-northwest of the former CCC/USDA facility, and in the vicinity of the Nigh property. (2) Identification of potential locations for several additional monitoring wells, to better constrain the apparent western and northwestern margins of the existing groundwater plume. Enhancement of Terrestrial Carbon Sinks through the Reclamation of Abandoned Mined Lands

This project will determine the optimal forest management method to employ for each of the major commercial tree species so that profitability of timber production only or the combination of timber production and carbon sequestration is maximized. The goal of this project is to achieve DOE's long-term cost goal of sequestering carbon at $10 or less per ton. Because the potential of a forest ecosystem to sequester carbon depends on the species, site quality and management regimes utilized, this project will determine how to optimize carbon sequestration by determining how to optimally manage each species, given a range of site qualities and economic variables. This project also will determine the effects of a carbon credit market on the method and profitability of forest management, the cost of sequestering carbon, and the amount of carbon that can be sequestered. Information from this project will be used to produce user-friendly manuals which will contain economic and biological data for each of the species. These manuals will inform landowners and forest managers how to manage forests for timber and/or carbon credits, how to maximize financial returns, how much money can be earned, and how much carbon can be stored. The Commodity Credit Corporation (CCC), an agency of the U.S. Department of Agriculture (USDA), formerly operated grain storage facilities at two different locations at Everest, Kansas (Figure 1.1). One facility (referred to in this report as the Everest facility) was at the western edge of the city. The second facility (referred to in this report as Everest East) was about 0.5 mi northeast of the town. The CCC/USDA operated these facilities from the early 1950s until the early 1970s, at a time when commercial fumigants containing carbon tetrachloride were in common use by the CCC/USDA and private industry for the preservation of grain in storage. In 1997 the Kansas Department of Health and Environment (KDHE) sampled several domestic drinking water and non-drinking water wells in the Everest area as part of the CCC/USDA Private Well Sampling Program. All of the sampled wells were outside the Everest city limits. Carbon tetrachloride contamination was identified at a single domestic drinking water well (the Nigh well, DW06; Figure 1.1) approximately 3/8 mi northwest of the former Everest CCC/USDA grain storage facility. Crack growth rates of irradiated austenitic stainless steel weld heat affected zone in BWR environments.

Austenitic stainless steels (SSs) are used extensively as structural alloys in the internal components of reactor pressure vessels because of their superior fracture toughness. However, exposure to high levels of neutron irradiation for extended periods can exacerbate the corrosion fatigue and stress corrosion cracking (SCC) behavior of these steels by affecting the material microchemistry, material microstructure, and water chemistry. Experimental data are presented on crack growth rates of the heat affected zone (HAZ) in Types 304L and 304 SS weld specimens before and after they were irradiated to a fluence of 5.0 x 10{sup 20} n/cm{sup 2} (E > 1 MeV) ({approx} 0.75 dpa) at {approx}288 C. Crack growth tests were conducted under cycling loading and long hold time trapezoidal loading in simulated boiling water reactor environments on Type 304L SS HAZ of the H5 weld from the Grand Gulf reactor core shroud and on Type 304 SS HAZ of a laboratory-prepared weld. The effects of material composition, irradiation, and water chemistry on growth rates are discussed.

Effect of Structure on the Storage Characteristics of ManganeseOxide Electrode Materials

Eleven types of manganese-containing electrode materialswere subjected to long-term storage at 55oC in 1M LiPF6 ethylenecarbonate/dimethyl carbonate (EC/DMC) solutions. The amount of manganesedissolution observed depended upon the sample surface area, the averageMn oxidation state, the structure, and substitution levels of themanganese oxide. In some cases, structural changes such as solvateformation were exacerbated by the high temperature storage, andcontributed to capacity fading upon cycling even in the absence ofsignificant Mn dissolution. The most stable materials appear to beTi-substituted tunnel structures and mixed metal layered oxides with Mnin the +4 oxidation state.

Synthesis of the isotopes of elements 118 and 116 in the 249Cf and 245Cm+48Ca fusion reactions

The decay properties of {sup 290}116 and {sup 291}116, and the dependence of their production cross sections on the excitation energies of the compound nucleus, {sup 293}116, have been measured in the {sup 245}Cm({sup 48}Ca,xn){sup 293-x}116 reaction. These isotopes of element 116 are the decay daughters of element 118 isotopes, which are produced via the {sup 249}Cf+{sup 48}Ca reaction. They performed the element 118 experiment at two projectile energies, corresponding to {sup 297}118 compound nucleus excitation energies of E* = 29.2 {+-} 2.5 and 34.4 {+-} 2.3 MeV. During an irradiation with a total beam dose of 4.1 x 10{sup 19} {sup 48}Ca projectiles, three similar decay chains consisting of two or three consecutive {alpha} decays and terminated by a spontaneous fission (SF) with high total kinetic energy of about 230 MeV were observed. The three decay chains originated from the even-even isotope {sup 294}118 (E{sub {alpha}} = 11.65 {+-} 0.06 MeV, T{sub {alpha}} = 0.89{sub -0.31}{sup +1.07} ms) produced in the 3n-evaporation channel of the {sup 249}Cf+{sup 48}Ca reaction with a maximum cross section of 0.5{sub -0.3}{sup +1.6} pb.

The Commodity Credit Corporation (CCC), an agency of the U.S. Department of Agriculture (USDA), formerly operated a grain storage facility in Courtland, Kansas. Prior to 1986, commercial grain fumigants containing carbon tetrachloride were commonly used by the CCC/USDA and the grain industry to preserve stored grain. In 1999, the Kansas Department of Health and Environment (KDHE) identified the former CCC/USDA operation as the likely source of carbon tetrachloride found in groundwater east of the former CCC/USDA facility in Courtland. Sampling by the KDHE in April 1998 had found carbon tetrachloride in the Garman residence lawn and garden well at a concentration of 2.1 {micro}g/L and in the Hoard residence lawn and garden well at a concentration of 0.5 {micro}g/L. Subsequent soil and groundwater sampling by the KDHE at the former CCC/USDA facility found no indication of a continuing source, and subsequent sampling of the affected wells showed generally declining contaminant levels. At the request of the KDHE and the CCC/USDA, Argonne National Laboratory prepared a Work Plan for Groundwater Sampling for Potential Site Reclassification, Courtland, Kansas (Argonne 2004). Mechanical and Thermal Properties of Ultra-High Carbon Steel Containing Aluminum

The properties of ultrahigh carbon steels (UHCS) are strongly influenced by aluminum additions. Hardness studies of quenched UHCS-Al alloys reveal that the temperature for the start of transformation increases with increases in aluminum content. It is shown that this change is a function of the atomic percent of solute and of the valence state when comparisons are made with UHCSs containing silicon and tin as solutes. The thermal expansion of UHCSs with dilute aluminum additions shows no discontinuity in the vicinity of the ferrite-austenite transformation temperature. This is the result of a three phase region of ferrite, carbides and austenite. The slope of the expansion curve is higher in the austenite range than in the ferrite range as a result of the dissolution of carbon in austenite with temperature. Processing to achieve a fine grain size in UHCS-Al alloys was principally by hot and warm working (HWW) followed by isothermal warm working (IWW). The high temperature mechanical properties of a UHCS-10Al-1.5C material show nearly Newtonian-viscous behavior at 900 to 1000 C. Tensile elongations of 1200% without failure were achieved in the 1.5%C material. The high oxidation corrosion resistance of the UHCS-10Al materials is described.
