VA Drug Formulary: Better Oversight Is Required, but Veterans Are Getting Needed Drugs

A letter report issued by the General Accounting Office with an abstract that begins "During the last three years, the Department of Veterans Affairs (VA) has made significant progress in establishing its national drug formulary, which has generally met with the prescriber acceptance. Most veterans are receiving the drugs the need and rarely register complaints about prescription drugs. However, VA has not been sufficient to ensure that the Veterans Integrated Service Networks (VISN) and medical centers comply with formulary policies and that the flexibility given to them does not compromise VA's goal of formulary standardization. Contrary to VA formulary policy, some facilities omitted national formulary drugs or modified the closest drug classes. Although a limited number of drugs to supplement the national formulary is permitted, formulary differences among facilities are likely to become more pronounced, as more drugs are added by VISNs, decreasing formulary standardization. VA recognizes the trade-off between local flexibility and standardization, but it lacks criteria for determining the appropriateness of adding drugs to supplement the national formulary and therefore may not be able to determine whether the decrease in standardization is acceptable."

Verification of the Accuracy of Sample-Size Equation Calculations for Visual Sample Plan Version 0.9C

Visual Sample Plan (VSP) is a software tool being developed to facilitate the design of environmental sampling plans using a site-map visual interface, standard sample-size equations, a variety of sampling grids and random sampling plans, and graphs to visually depict the results to the user. This document provides comparisons between sample sizes calculated by VSP Version 0.9C, and sample sizes calculated by test code written in the S-Plus language. All sample sizes calculated by VSP matched the independently calculated sample sizes. Also the VSP implementation of the ELIGPRID-PC algorithm for hot spot probabilities is shown to match previous results for 100 standard test cases. The Conclusions and Limitations section of this document lists some aspects of VSP that were not tested by this suite of tests and recommends simulation-based enhancements for future versions of VSP.

SCFA lead lab technical assistance review of the Pit 7 Complex source containment

On January 29-30, 2001 a technical assistance team (TAT) met with the Pit 7 project team at Lawrence Livermore National Laboratory (LLNL) to review technologies being evaluated for remediation in the Site 300 Pit 7 Complex and the process for selecting these technologies. Specifically, the project team presented the TAT with a core need to identify technically and economically practicable technologies and methods to stabilize, contain, or control the tritium and uranium in the source areas at the Pits 3 and 5 landfill area to prevent further releases of these contaminants to groundwater and the migration of tritiated and uranium-contaminated groundwater. The approaches and needs for the systems surrounding the landfills were also presented and discussed. With encouragement from the project team, the TAT expanded its focus to include additional site characterization, a water balance model, and computational models. The TAT was comprised of leading technical and regulatory experts from around the country and was assembled by SCFA's Lead Lab in response to a technical assistance request from John Ziagos, Project Manager for the Pit 7 Area (Technical Assistance Request: LLNL No.1). It is effective not only in preventing the passage of legislation undesirable to the President, but also as a threat, sometimes forcing Congress to modify legislation before it is presented to the President. The objective of this calculation is to determine the structural response of a 21-PWR (pressurized-water reactor) Waste Package (WP) subjected to the 2-m vertical drop on an unyielding surface at three different temperatures. The scope of this calculation is limited to reporting the calculation results in terms of stress intensities in two different WP components. The information provided by the sketches (Attachment I) is that of the potential design of the type of WP considered in this calculation, and all obtained results are valid for that design only.

Metallurgical Laboratory Treatability Study: An Analysis of Passive Soil Vapor Extraction Wells - June 2000 Update

The passive soil vapor extraction (PSVE) system at the MetLab of the Savannah River Site has been operating since May 1998. The results to date on the treatability study indicate the technology is performing well. Well concentrations are decreasing and contour maps of the vadose zone soil gas plume show a decrease in the extent of the plume. In the 2 years of operation approximately 270 pounds of chlorinated organic contaminants have been removed by natural barometric pumping of wells fitted with BaroBall valves (low pressure check valves). The PSVE system is performing well in a cost-effective manner. It is recommended that this system be allowed to continue operating to complete the remediation and to continue monitoring activities to verify and monitor the anticipated contaminant removal rates. The treatability study should be considered successfully completed and the remediation should be considered in full operation.

A Pore-Network Model of In-Situ Combustion in Porous Media

This report the use of dual pore networks (pores and solid sites) for modeling the effect of the microstructure on combustion processes in porous media is considered. The model accounts for flow and transport of the gas phase in the porespace, where convection predominates, and for heat transfer by conduction in the solid phase. Gas phase flow in the pore and throats is governed by Darcy's law.

REGULATIONS ON PHOTOVOLTAIC MODULE DISPOSAL AND RECYCLING.

Environmental regulations can have a significant impact on product use, disposal, and recycling. This report summarizes the basic aspects of current federal, state and international regulations which apply to end-of-life photovoltaic (PV) modules and PV manufacturing scrap destined for disposal or recycling. It also discusses proposed regulations for electronics that may set the ground of what is to be expected in this area in the near future. In the US, several states have started programs to support the recycling of electronic equipment, and materials destined for recycling often are excepted from solid waste regulations during the collection, transfer, storage and processing stages. California regulations are described separately because they are different from those of most other states. International agreements on the movement of waste between different countries may pose barriers to cross-border shipments. Currently waste moves freely among country members of the Organization of Economic Cooperation and Development (OECD), and between the US and the four countries with which the US has bilateral agreements. However, it is expected, that the US will adopt the rules of the Basel Convention (an agreement which currently applies to 128 countries but not the US) and that the Convection's waste classification system will influence …

Surface Sediments in Precooler Ponds 2, 4, and 5: March 2000

Pond 2, Pond 4, and Pond 5 are inactive reactor cooling impoundments built in 1961 on the R-Reactor Effluent System in the east-central portion of the Department of Energy's Savannah River Site in Aiken, South Carolina. These precooler ponds are part of the Par Pond cooling water system and are considered part of the Par Pond operable unit. The intent was not to characterize the ponds, but to identify the maximum levels of contamination that could be exposed if the ponds are drained to remove the danger of dam failure.
