Defense Critical Infrastructure: Actions Needed to Improve the Consistency, Reliability, and Usefulness of DOD's Tier 1 Task Critical Asset List

Correspondence issued by the Government Accountability Office with an abstract that begins "The Department of Defense (DOD) relies on a global network of defense critical infrastructure so essential that the incapacitation, exploitation, or destruction of an asset within this network could severely affect DOD's ability to deploy, support, and sustain its forces and operations worldwide and to implement its core missions, including current missions in Iraq and Afghanistan. Because of its importance to DOD operations, this defense critical infrastructure could be vulnerable to attacks by adversaries, and vulnerable to natural disasters and hazards, such as hurricanes and earthquakes. Since September 2003, the Office of the Assistant Secretary of Defense for Homeland Defense and Americas' Security Affairs (ASD[HD&ASA]) has been responsible for developing and ensuring implementation of critical infrastructure protection policy and program guidance. To identify and help assure the availability of this mission-critical infrastructure, in August 2005 DOD established the Defense Critical Infrastructure Program (DCIP), assigning overall responsibility for the program to ASD(HD&ASA). In April 2008, DOD issued an instruction that further assigned responsibilities and prescribed procedures for the implementation of DCIP, among other things. Fair Lending: Race and Gender Data Are Limited for Nonmortgage Lending

Testimony issued by the Government Accountability Office with an abstract that begins "The Federal Reserve Board's (FRB) Regulation B, which implements the Equal Credit Opportunity Act of 1974 (ECOA), generally prohibits lenders from collecting certain data from loan applicants, such as their race or gender, for nonmortgage loans (e.g., small business loans). FRB has stated that this provision of Regulation B minimizes the chances that lenders would use such data in an unlawful and discriminatory manner. However, others argue that the prohibition limits the capacity of researchers and regulators to identify possible discrimination in nonmortgage lending. This testimony is based on the GAO report, Fair Lending: Race and Gender Data Are Limited for Nonmortgage Lending (GAO-08-698, June 27, 2008). Specifically, GAO analyzes (1) studies on possible discrimination in nonmortgage lending and the data used in them, (2) FRB's 2003 decision to retain the prohibition of voluntary data collection, and (3) the benefits and costs of a data collection and reporting requirement. Correspondence issued by the Government Accountability Office with an abstract that begins "At the 1996 World Food Summit (WFS) in Rome, the United States and more than 180 world leaders pledged to halve the total number of undernourished people worldwide from the 1990 level---a commitment that they reaffirmed in 2000 when they established the Millennium Development Goals (MDG), which included a target to halve the proportion or the percentage of the world's population that is undernourished by 2015. More than a decade later, however, the number of undernourished people has not decreased significantly, and about 850 million people, including 170 million children, remain undernourished, according to the United Nations (UN) Food and Agriculture Organization (FAO). Furthermore, the number of undernourished people in sub-Saharan Africa has increased from about 170 million in the period of 1990 to 1992 to over 200 million in the period of 2001 to 2003. Since early 2007, food-related riots have occurred in 15 countries, including 7 in sub-Saharan Africa, leading both the UN Secretary-General and the head of the World Food Program (WFP) to express concern about the impact of chronic undernourishment, or food insecurity, on world peace and security. Testimony issued by the Government Accountability Office with an abstract that begins "About 43,000 traffic fatalities occur annually, and another 290,000 people are seriously injured on the nation's roads. To reduce these numbers, the Safe, Accountable, Flexible, Efficient Transportation Equity Act: A Legacy for Users (SAFETEA-LU) nearly doubled funding for the Federal Highway Administration's (FHWA) Highway Safety Improvement Program (HSIP), authorizing $5.1 billion for 2006 through 2009. SAFETEA-LU also added requirements for states to develop strategic highway safety plans that cover all aspects of highway safety, including infrastructure, behavioral (education and enforcement), and emergency medical services projects; develop crash data analysis systems; and publicly report on the top 5 percent of hazardous locations on all their public roads. SAFETEA-LU also set aside funds for a legacy rail-highway crossing program and a new high-risk rural road program. This testimony provides preliminary information on the implementation of HSIP since SAFETEA-LU. It is based on ongoing work that addresses (1) states' implementation of HSIP following SAFETEA-LU, (2) FHWA's guidance and assistance for states, and (3) results of HSIP to date, including for the two set-aside programs. Testimony issued by the Government Accountability Office with an abstract that begins "The Historically Underutilized Business Zone (HUBZone) program is intended to provide federal contracting opportunities to qualified small business firms in order to stimulate development in economically distressed areas. As manager of the HUBZone program, the Small Business Administration (SBA) is responsible for certifying whether firms meet HUBZone program requirements. To participate in the HUBZone program, small business firms must certify that their principal office (i.e., the location where the greatest number of employees work) is located in a HUBZone and that at least 35 percent of the firm's employees live in HUBZones. Given the Committee's concern over fraud and abuse in the HUBZone program, GAO was asked to (1) proactively test whether SBA's controls over the HUBZone application process were operating effectively to limit program certification to eligible firms and (2) identify examples of selected firms that participate in the HUBZone program even though they do not meet eligibility requirements. To perform its proactive testing, GAO created four bogus businesses with fictitious owners and employees and applied for HUBZone certification. Correspondence issued by the Government Accountability Office with an abstract that begins "New Orleans faces many challenges in the aftermath of Hurricane Katrina including the challenge of reestablishing the health care system and hospitals within the system. Hurricane Katrina, which made landfall on August 29, 2005, and the subsequent flooding caused by the failure of the New Orleans levee systems, resulted in the sudden closure, damage, or disruption in services at many of the New Orleans hospitals. On August 1, 2007, officials representing five New Orleans hospitals that have been the main health care providers in the region since the hurricane, testified before the House Committee on Energy and Commerce's Subcommittee on Oversight and Investigations. The officials stated that since the hurricane they have experienced significant operating losses and that they expect the losses to continue. The official from one of the hospitals that was designated to present an overview of the specific problems facing the five hospitals stated in his testimony that the hospitals expected to experience a combined operating loss of $135 million in calendar year 2007. Nuclear Weapons: Views on NNSA's Proposal to Transform the Nuclear Weapons Complex

Testimony issued by the Government Accountability Office with an abstract that begins "Over the past several years, a serious effort has begun to comprehensively reevaluate how the United States maintains its nuclear deterrent and what the nation's approach should be for transforming its aging nuclear weapons complex. The National Nuclear Security Administration (NNSA), a separately organized agency within the Department of Energy (DOE), is responsible for overseeing this weapons complex, which comprises three nuclear weapons design laboratories, four production plants, and the Nevada Test Site. In December 2007, NNSA issued a draft report on potential environmental impacts of alternative actions to transform the nuclear weapons complex, which NNSA refers to as Complex Transformation. NNSA's preferred action is to establish a number of "distributed centers of excellence" at sites within the existing nuclear weapons complex, including the Los Alamos National Laboratory for plutonium capabilities, the Y-12 Plant for uranium capabilities, and the Pantex Plant for weapons assembly, disassembly, and high explosives manufacturing. NNSA would continue to operate these facilities to maintain and refurbish the existing nuclear weapons stockpile as it makes the transition to a smaller, more responsive infrastructure. Testimony issued by the Government Accountability Office with an abstract that begins "In fiscal year 2006, the federal government spent over $400 billion for a wide variety of goods and services, with the Department of Defense (DOD) being the largest purchaser. Given the large and growing structural deficit, the government must get the best return it can on its investment in goods and services. For decades, GAO has reported on a number of systemic challenges in agencies' acquisition of goods and services. These challenges are so significant and wide-ranging that GAO has designated four areas of contract management across the government to be high-risk. This testimony highlights four key acquisition challenges agencies face: (1) separating wants from needs, (2) establishing and supporting realistic program requirements, (3) using contractors in appropriate circumstances and contracts as a management tool, and (4) creating a capable workforce and holding it accountable."

Oral History Interview with William Starke, July 17, 2007

The National Museum of the Pacific War presents an interview with William Starke. Starke joined the Army Air Forces in March of 1942. He completed flight training and graduated at Williams Field, Arizona, in July of 1943 as a P-38 pilot. He provides details of his flight training and the various planes he flew. In November he traveled to Guadalcanal and joined the 44th Fighter Squadron, 13th Air Force. Starke served as a P-38 Lightning pilot and completed 121 missions throughout the Pacific, including Sansapor, Bougainville and Rabaul in Papua New Guinea, Borneo, Luzon and the Philippines. His squadron flew combat missions and escorted B-24 bombers. Starke provides vivid details of several missions and describes how they acquired the name “Vampire Squadron.” He was discharged in July of 1945. He remained in the Reserves for 20 years and retired as a lieutenant colonel.

Correspondence issued by the Government Accountability Office with an abstract that begins "GAO and Inspector General (IG) reports have noted that the U.S. Citizenship and Immigration Service (USCIS) efforts to modernize over the last 4 years have been unfocused, conducted in an ad hoc and decentralized manner, and, in certain instances, duplicative. In 2006, USCIS decided to reexamine its modernization effort within the context of an agencywide organizational and business transformation initiative. The agency embarked on a transformation of its business processes and technology aimed at increasing national security and integrity, improving customer service, and achieving operational efficiency. We agreed with this approach and recommended that USCIS employ key practices for successful organizational transformations to better ensure the success of its efforts. USCIS plans to complete its transformation by 2013 at an estimated cost of up to $536 million, mostly funded by fee revenues. The Department of Homeland Security (DHS) Appropriations Act for fiscal year 2007 mandated that GAO review USCIS' transformation plans before the agency can obligate $47 million in funding for the transformation. NASA: Long-Term Commitment to and Investment in Space Exploration Program Requires More Knowledge

Correspondence issued by the Government Accountability Office with an abstract that begins "The National Aeronautics and Space Administration (NASA) plans to spend nearly $230 billion over the next two decades implementing the Vision for Space Exploration. In January 2006, NASA publicly released its Exploration Systems Architecture Study (ESAS), which is an effort to identify the best architecture and strategy to implement the President's 2004 Vision for Space Exploration (Vision). The cost estimate for implementing the ESAS through fiscal year 2011 exceeds $31 billion. The estimate through fiscal year 2018 is over $122 billion, and the estimate through fiscal year 2025 is nearly $230 billion. These estimates include the architecture, robotic precursor missions, supporting technologies, and funding needed to service the International Space Station (ISS). NASA plans to implement this architecture through a "go as you can afford to pay" approach, wherein lower-priority efforts would be deferred, descoped, or discontinued to allow NASA to stay within its available budget profile. This approach assumes NASA's budget will increase moderately to keep pace with inflation. Correspondence issued by the General Accounting Office with an abstract that begins "Despite a recent decline in the population that smokes, smoking is considered the leading cause of preventable death in this country. According to the Centers for Disease Control and Prevention (CDC), over 2 million deaths in the 5-year period from 1995 through 1999 were attributable to cigarette smoking. CDC, part of the Department of Health and Human Services (HHS), is a primary source of information on the health consequences of smoking tobacco. CDC reported its most recent estimates of selected health consequences of cigarette smoking in an April 2002 issue of its publication Morbidity and Mortality Weekly Report. CDC reported that, on average, over 440,000 deaths, 5.6 million years of potential life lost, $82 billion in mortality-related productivity losses, and $76 billion in medical expenditures were attributable to cigarette smoking each year from 1995 through 1999. CDC and others tasked with making such estimates face challenges. They build estimates on a set of assumptions and make choices about the data sources and methods used, each of which may have limitations that must be weighed against its advantages. Testimony issued by the General Accounting Office with an abstract that begins "Recent news articles in over 30 states and prominent mental health advocacy organizations have described the difficulty many parents have in accessing mental health services for their children. As these reports documented, some parents choose to place their children in the child welfare or juvenile justice systems in order to obtain the mental health services that their children need. Senators Susan Collins and Joseph Lieberman of the Senate Committee on Governmental Affairs asked GAO to testify on: (1) the number and characteristics of children voluntarily placed in the child welfare and juvenile justice systems to receive mental health services, (2) the factors that influence such placements, and (3) promising state and local practices that may reduce the need for child welfare and juvenile justice placements. Testimony issued by the General Accounting Office with an abstract that begins "Since 1998, the Congress and Administration have focused considerable attention on improving the quality of care in the nation's nursing homes, which provide care for about 1.7 million elderly and disabled residents in about 17,000 homes. GAO has earlier reported on serious weaknesses in processes for conducting routine state inspections (surveys) of nursing homes and complaint investigations, ensuring that homes with identified deficiencies correct the problems without recurrence, and providing consistent federal oversight of state survey activities to ensure that nursing homes comply with federal quality standards. GAO was asked to update its work on these issues and to testify on its findings, as reported in Nursing Home Quality: Prevalence of Serious Problems, While Declining, Reinforces Importance of Enhanced Oversight, GAO-03-561 (July 15, 2003). Compact of Free Association: An Assessment of Current U.S. Proposals to Extend Assistance

Testimony issued by the General Accounting Office with an abstract that begins "The United States entered into the Compact of Free Association with the Federated States of Micronesia (FSM) and the Republic of the Marshall Islands (RMI) In 1986. The Compact has provided U.S. assistance to the FSM and the RMI in the form of direct funding as well as federal services and programs. The Compact allows for migration from both countries to the United States and established U.S. defense rights and obligations in the region. Provisions of the Compact that deal with economic assistance were scheduled to expire in 2001; however, they will remain in effect for up to 2 additional years while the affected provisions are renegotiated. Current U.S. proposals to the FSM and the RMI to renew expiring assistance would require Congress to approve $3.4 billion in new authorizations. The proposals would provide decreasing levels of annual grant assistance over a 20-year term. Simultaneously, the proposals would require building up a trust fund for each country with earnings that would replace grants once those grants expire. Testimony issued by the General Accounting Office with an abstract that begins "The government faces a unique opportunity to create an organization that is extremely effective in protecting the nation's borders and citizens against terrorism. There is likely to be considerable benefit over time from restructuring some of the homeland security functions, including reducing risk and improving the economy, efficiency, and effectiveness of consolidated agencies and programs. Sorting out those programs and agencies that would most benefit from consolidation versus those in which dual missions must be balanced in order to achieve a more effective fit in the proposed Department of Homeland Security is a difficult but critical task. Moreover, the magnitude of the challenges that the new department faces will clearly require substantial time and effort, and it will take institutional continuity and additional resources to be fully effective. In the short term, issues to be resolved include the harmonization of communication systems, information technology systems, human capital systems, the physical location of people and other assets, and other factors. Testimony issued by the General Accounting Office with an abstract that begins "A weak overall control environment and breakdowns in key internal controls have left the Army's purchase card program vulnerable to fraudulent, improper, and abusive purchases. At five Army major commands and one installation in each of the commands, GAO found that the Army had not established an effective internal control environment. As the use of purchase cards has expanded, Army management has not emphasized internal controls to ensure that individual transactions were for authorized purposes or that they adhered to legal and regulatory requirements. GAO identified a substantial number of purchases for which cardholder and approving officials had not adhered to important internal controls and were not in accordance with valid requirements, policies, and procedures. The weaknesses identified in the control environment and the breakdown in specific internal control activities resulted in potentially fraudulent and other improper transactions not being prevented or properly identified."

Testimony issued by the General Accounting Office with an abstract that begins "In fiscal year 2001, the Army had 430,000 individually billed travel card accounts, and about $619 million in related charges. Most Army cardholders properly used their travel cards and promptly paid amounts owed. However, the Army's delinquency rate is higher than any other Department of Defense (DOD) component or executive branch agency. GAO also identified numerous instances of potentially fraudulent and abusive activity related to the travel cards. During fiscal year 2001, at least 200 Army employees wrote three or more nonsufficient funds or "bounced" checks to Bank of America as payment for their travel bills--potentially fraudulent acts. GAO found little evidence of documented disciplinary action against Army personnel who misused the card, or that Army travel program managers or supervisors were even aware that travel cards were being used for personal use. For fiscal year 2001, the Army had significant breakdowns in key internal controls over individually billed travel cards that stemmed from a weak overall environment, flawed policies and procedures, and a lack of adherence to valid policies and procedures.
