Last year, we reported that the VXX effort was in the earliest stages of development--still developing a business case to launch product development. At that time, an AOA—required for a Milestone A decision initiating the program—was nearing completion and a Milestone A decision was expected to occur sometime in fiscal year 2011. In March 2011 the Navy sent DOD its VXX AOA study, which Navy officials stated was done in compliance with DOD-provided guidance. DOD did not, however, approve the study as it did not find that the study provided a cost effective solution. Rather, OSD and the Navy subsequently decided to update the analysis of alternatives using an acquisition strategy that might result in a more timely and affordable program using additional guidance provided by OSD in December 2011. That guidance reflects insights on requirements gained in the last year and expectations of using a streamlined acquisition approach proposed by the Navy. Correspondence issued by the Government Accountability Office with an abstract that begins "Applying PHMSA’s new distribution integrity management requirements to low-stress transmission pipelines would result in less prescriptive safety requirements for these pipelines. Overall, requirements for distribution pipelines are less prescriptive than requirements for transmission pipelines in part because the former operate at lower pressure and pose lower risks in general than the latter. For example, the integrity management regulations for transmission pipelines allow three types of in-depth physical inspections. In contrast, distribution pipeline operators can customize their integrity management programs to the complexity of their systems, including using a broader range of methods for physical inspection. While PHMSA officials stated that “less prescriptive” does not necessarily mean less safe, they also stated that distribution integrity management requirements for distribution pipelines can be more difficult to enforce than integrity management requirements for transmission pipelines. Currently, PHMSA can grant special permits to modify requirements for individual pipelines, if merited, but applying the new distribution integrity management requirements to low-stress transmission pipelines would affect all such pipelines."

Correspondence issued by the Government Accountability Office with an abstract that begins "Among the tribes that responded to our survey (109), none reported that they were exercising TLOA’s new sentencing authority, and, in open-ended responses, many tribes (86 of 90, or 96 percent) reported challenges to exercising this authority due to funding limitations. Tribes were relatively evenly split among those that reported that they have plans to exercise the new authority (36 of 101, or 36 percent); that they did not know the tribe’s plans to exercise the new authority (34 of 101, or 34 percent) because, for instance, the tribal council has not yet made a decision; and that they did not have plans to do so (31 of 101, or 31 percent). In addition, 64 percent of selected tribes (70 of 109) reported implementing at least half of the requirements necessary for exercising the new sentencing authority, but reported challenges in implementing other requirements. Specifically, these tribes most frequently reported implementing the requirement to maintain a record of the criminal proceeding, and least frequently reported providing the defendant a licensed defense attorney. ONDCP officials stated that they developed the 2010 Strategy’s seven objectives—for example, Strengthen Efforts to Prevent Drug Use in Our Communities—and 106 action items under these objectives through a consultative process with federal, state, and local drug control agencies and other stakeholders. Officials from the four agencies we spoke with as of April 2012 stated that ONDCP sought input from them to develop the Strategy and that as a result, existing agency priorities and activities are reflected in the Strategy. Correspondence issued by the Government Accountability Office with an abstract that begins "Our analysis of DOD’s fiscal year 2011 update relating to the BRAC 2005 budget submission to Congress shows that one-time implementation costs grew from $21 billion originally estimated by the BRAC Commission in 2005 to about $35.1 billion, an increase of about $14.1 billion, or 67 percent, largely due to increased construction costs. We compared the BRAC Commission’s 2005 estimates to DOD’s fiscal year 2011 budget submission and found that 14 of 182 BRAC recommendations accounted for about 72 percent of the cost increase, or about $10.2 billion. Our analysis of those 14 recommendations shows that increased construction costs resulted primarily from additional building projects and additions to planned projects, which DOD deemed necessary after implementation began. For example, one-time costs for realigning the National Geospatial-Intelligence Agency more than doubled from $1.1 billion to $2.6 billion, with military construction accounting for nearly $726 million of that increase due to additional supporting facilities the agency identified as essential to the mission. Overall, military construction costs for the BRAC 2005 round increased 86 percent, from $13.2 billion estimated by the BRAC Commission to $24.5 billion according to DOD’s fiscal year …

The Army and DFAS-IN did not have an effective, repeatable process for identifying the population of active duty payroll accounts. In addition, the Defense Manpower Data Center (DMDC), DOD’s central source for personnel information, did not have an effective process for comparing military pay account files to military personnel files to identify a valid population of military payroll transactions. For example, it took 3 months and repeated attempts before DFAS-IN could provide a population of service members who received active duty Army military pay in fiscal year 2010. Similarly, it took DMDC over 2 months to compare the total number of fiscal year 2010 active duty payroll accounts to its database of personnel files. "Standards for Internal Control in the Federal Government" requires all transactions and other significant events to be clearly documented and the documentation readily available for examination. Testimony issued by the Government Accountability Office with an abstract that begins "Our work on the outcomes of the CBP round 1 rebid found that the number of bidding suppliers and the number of contracts awarded in the CBP round 1 rebid were very similar to the CBP round 1 and about a third of the 1,011 suppliers that bid in the rebid were awarded at least one CBP contract. CMS made improvements to the bidding process for the CBP round 1 rebid—such as providing additional information about disqualification reasons—and significantly fewer bids were disqualified than in round 1. However, many suppliers still had difficulty meeting bid requirements. Of the bids that were disqualified during the initial bid review, 73 percent were disqualified because suppliers failed to provide the required financial documentation or did not meet CMS’s minimum financial standard threshold for suppliers. Testimony issued by the Government Accountability Office with an abstract that begins "Noncompliance does not have a single source but occurs across different types of taxes and taxpayers. For example, individual income tax accounts for the largest portion of the tax gap, but corporate income tax and employment tax are also significant. Further, misreporting by individuals involves business income, non-business income, deductions, and credits. The extent of misreporting depends on the extent to which income tax is withheld or reported to the Internal Revenue Service (IRS) by third parties. For example, nearly 40 percent, or $179 billion, of the 2006 gross tax gap is due to misreporting of non-corporate business income and related self-employment taxes. Much of this misreporting can be attributed to sole proprietors underreporting receipts or over-reporting expenses. Unlike wage and some investment income, sole proprietors’ income is not subject to withholding and only a portion is reported to IRS by third parties."

In testimony, the Director of National Intelligence noted a dramatic increase in cyber activity targeting U.S. computers and systems, including a more than tripling of the volume of malicious software. Varying types of threats from numerous sources can adversely affect computers, software, networks, organizations, entire industries, and the Internet itself. These include both unintentional and intentional threats, and may come in the form of targeted or untargeted attacks from criminal groups, hackers, disgruntled employees, nations, or terrorists. The interconnectivity between information systems, the Internet, and other infrastructures can amplify the impact of these threats, potentially affecting the operations of critical infrastructures, the security of sensitive information, and the flow of commerce. Moreover, the electricity grid’s reliance on IT systems and networks exposes it to potential and known cybersecurity vulnerabilities, which could be exploited by attackers. The potential impact of such attacks has been illustrated by a number of recently reported incidents and can include fraudulent activities, damage to electricity control systems, power outages, and failures in safety equipment."

Home Energy Assistance for Low-Income Occupants of Manufactured Homes

Correspondence issued by the Government Accountability Office with an abstract that begins "In 2005, the most recent year for which complete data were available, occupants of older manufactured homes paid over twice as much on average per square foot for energy---$1.75 per square foot as compared with $0.87---as was paid by occupants of detached homes. Annual energy expenditures for older manufactured homes--about 906 square feet on average--were about $1,369, compared with detached homes--about 2,919 square feet on average--were about $2,060. Energy expenditures--both per square foot and annually--varied significantly by region reflecting regional differences in the types and costs of fuels commonly used to heat and cool homes, income levels, and climate, among other things. In 2005, LIHEAP provided more assistance on a per square foot basis--about $0.33 per square foot--to occupants of older manufactured homes than to those of detached homes--about $0.20 per square foot. However, this assistance covers slightly less of the annual energy expenditures of occupants of older manufactured homes than occupants of detached homes--15 and 17 percent, respectively. Based on our analysis of EIA's RECS data, we estimate that about 3 percent of LIHEAP funds--about $57 million--spent in 2005 were used to assist occupants of older manufactured …

Testimony issued by the Government Accountability Office with an abstract that begins "In March 2012, GAO issued a report on the Centers for Medicare & Medicaid Services’ (CMS) Medicare Advantage (MA) quality bonus payment demonstration—a demonstration CMS initiated rather than implementing the quality bonus program established under the Patient Protection and Affordable Care Act (PPACA). Compared to the PPACA quality bonus program, CMS’s demonstration increases the number of plans eligible for a bonus, enlarges the size of payments for some plans, and accelerates payment phase-in. CMS stated that the demonstration’s research goal is to test whether scaling bonus payments to quality scores MA plans receive increases the speed and degree of annual quality improvements for plans compared with what would have occurred under PPACA. GAO reported that CMS’s Office of the Actuary estimated that the demonstration would cost $8.35 billion over 10 years—an amount greater than the combined budgetary impact of all Medicare demonstrations conducted since 1995. In addition, GAO also found several shortcomings of the demonstration design that preclude a credible evaluation of its effectiveness in achieving CMS’s stated research goal. Correspondence issued by the Government Accountability Office with an abstract that begins "The SNS contains supportive care items for thermal burns, such as bandages, pain medications, intravenous fluids, and topical antimicrobial cream needed for the immediate treatment of burn injuries to reduce the risk of infection and stabilize injured individuals. HHS officials told us that the goal of the SNS is to supplement state and local supplies used for immediate care in the initial response—identified as within 72 hours of sustaining injury. CDC compiled supplies needed for the immediate treatment of burn injuries into kits in 2002 and 2003, based on information provided at that time by burn experts about needed items. Because most medical countermeasures for thermal burns can be found in local hospitals, countermeasures in the SNS would be used to supplement local supplies and inventories, with kits deployed within 24 to 48 hours of notification. The SNS does not contain other countermeasures that may be available for both the immediate care and the longer-term treatment of burn injuries. However, HHS is currently considering whether to acquire some additional countermeasures, including those for longer-term treatment of burn injuries."
