A letter report issued by the Government Accountability Office with an abstract that begins "Although the U.S. Census Bureau (Bureau) generally completed the field data collection phase of the 2010 Census consistent with its operational plans, at $13 billion, 2010 was the costliest census in the nation's history. Moving forward, it will be important to both refine existing operations as well as to reexamine the fundamental approach to the census to better address long-standing issues such as securing participation and escalating costs. As requested, this report reviews (1) the conduct of nonresponse follow-up (NRFU), where enumerators collect data from households that did not return their census forms, (2) the implementation of other field operations critical to a complete count, and (3) potential reexamination areas that could help produce a more cost-effective 2020 Census. Having piloted the integrated disability evaluation system (IDES) at 27 military facilities, they are now planning for its expansion military-wide. Part of the National Defense Authorization Act for Fiscal Year 2008 required GAO to report on DOD and VA's implementation of policies on disability evaluations. This report examines: (1) the results of the agencies' evaluation of the IDES pilot, (2) challenges in implementing the IDES pilot to date, and (3) whether DOD and VA's plans to expand the IDES adequately address potential future challenges. First, it is the latest report on the uses of and accountability for Recovery Act funds in selected states and localities. Second, it comments on recipients' reports of the jobs created and retained. The Recovery Act provided $2.1 billion for Head Start and Early Head Start, primarily to expand services. GAO addressed four questions: (1) How have Head Start and Early Head Start grantees used Recovery Act funds, including for expanding enrollment? (2) What challenges have grantees encountered in spending Recovery Act funds? (3) How has the Office of Head Start (OHS) monitored the use of Recovery Act funds? (4) How has the quality of jobs data reported by Recovery Act recipients, particularly Head Start grantees, changed over time? In this report, GAO also updates the status of open recommendations from previous bimonthly and recipient reporting reviews. Correspondence issued by the Government Accountability Office with an abstract that begins "Medicare funds health care services for more than 46 million beneficiaries. The Centers for Medicare & Medicaid Services (CMS)--the agency that administers Medicare--contracts with private organizations known as Quality Improvement Organizations (QIO) to, among other core functions, improve the quality of care for Medicare beneficiaries. CMS contracts with one QIO for each of the 50 states, the District of Columbia, Puerto Rico, and the U.S. Virgin Islands. One of the QIOs' many responsibilities is to review quality of care concerns, raised by Medicare beneficiaries or others, to determine whether Medicare-financed medical services meet professionally recognized standards of health care. Quality of care reviews may address a range of issues, such as inappropriate treatment or hospital staff not administering medications on time; may involve a variety of health care services and settings; and may include a range of Medicare providers or practitioners. A letter report issued by the Government Accountability Office with an abstract that begins "The Federal Emergency Management Agency (FEMA), a component of the Department of Homeland Security (DHS), maps flood hazard areas across the country and makes flood insurance available to more than 20,100 communities through the National Flood Insurance Program. From 2003 through 2008, FEMA spent $1.2 billion in a comprehensive effort to update the nation's flood insurance maps. In 2009, FEMA began an annual review of 20 percent of the nation's flood maps, for which Congress allocated $440 million in 2009 and 2010. As requested, GAO reviewed the actions FEMA has taken to enhance the accuracy of updated flood maps, and FEMA's outreach efforts in conducting flood mapping activities. Established in 1971, HAC is a nonprofit rural housing organization that aims to improve housing conditions for low-income rural residents, especially in high-need areas such as Indian country and Appalachia and among groups such as farmworkers. As part of its mission, HAC also offers technical assistance in developing affordable rural housing and capacity building to a variety of groups involved in rural housing. Federal law requires passenger and major freight railroads to install PTC on most major routes by the end of 2015. Railroads must address other risks by implementing other technologies. The Department of Transportation's (DOT) Federal Railroad Administration (FRA) oversees implementation of these technologies and must report to Congress in 2012 on progress in implementing PTC. As requested, this report discusses railroads' progress in developing PTC and the remaining steps to implement it, the benefits of and challenges in implementing other safety technologies, and the extent of FRA's efforts to fulfill the PTC mandate and encourage the implementation of other technologies. Correspondence issued by the Government Accountability Office with an abstract that begins "From the September 11, 2001, attack on the World Trade Center (WTC) through fiscal year 2010, approximately $475 million in federal funds was made available for screening, monitoring, or treating responders for illnesses and conditions--such as asthma and depression--related to the WTC disaster. Within the Department of Health and Human Services, the Centers for Disease Control and Prevention's (CDC) National Institute for Occupational Safety and Health (NIOSH) awards funds to and oversees the programs that provide screening, monitoring, and treatment services for responders to the WTC attack. The two largest programs, which we refer to here as the WTC health programs, are the New York City Fire Department's (FDNY) WTC Medical Monitoring and Treatment Program, and the New York/New Jersey (NY/NJ) WTC Consortium. These programs began as screening and monitoring programs, tracking the health status of responders related to the WTC disaster. In December 2005, the Congress first appropriated funds that were specifically available for treatment programs for certain responders with health conditions related to the WTC disaster, and in fall 2006, NIOSH began awarding funds for outpatient and inpatient treatment. Correspondence issued by the Government Accountability Office with an abstract that begins "The Department of Justice (DOJ) has reported that crime rates experienced by American Indians are two and a half times higher than those experienced by the general population in the United States. Specifically, from 1992 to 2001 American Indians experienced violent crimes at a rate of 101 violent crimes per 1,000 person annually, compared to the national rate of 41 per 1,000 persons. The federal government plays a major role in prosecuting crimes committed in Indian country. For example, unless a federal statute has granted the state jurisdiction, the federal government has exclusive jurisdiction to prosecute non-Indians who commit crimes against Indians in Indian country, while the federal government and tribal governments both have jurisdiction to prosecute Indian offenders who commit crimes in Indian country. Federal prosecution, however, carries with it the possibility of greater terms of imprisonment, as tribal courts are statutorily limited to a maximum of 3 years imprisonment per offense, regardless of the severity of the offense, for example, a homicide. Correspondence issued by the Government Accountability Office with an abstract that begins "The National Military Strategy of the United States calls upon the Armed Forces to retain the ability to rapidly deploy and sustain capabilities to diverse regions, and the Quadrennial Defense Review 2010 acknowledges the fundamental importance of U.S. capability to project power. The National Security Strategy identifies taking stock of capabilities as one of many ways of reducing military risk. To identify the mobility tools needed for force projection, the Department of Defense (DOD) has conducted several studies, including the fifth and most recent--the Mobility Capabilities and Requirements Study 2016 (MCRS-16). DOD issued the report in February 2010. The intent of the MCRS-16 was to provide senior leaders with a detailed understanding of the range of mobility capabilities needed for possible future strategic environments and help them make investment decisions regarding mobility systems. Specifically, the study was to examine, among other things, how changes in the mobility system affect the outcomes of major operations and to assess the associated risks. A letter report issued by the Government Accountability Office with an abstract that begins "Prior GAO testimonies have described cases of physical abuse of children at youth residential treatment programs and public and private schools. However, children are also vulnerable to sexual abuse. A 2004 Department of Education report estimated that millions of students are subjected to sexual misconduct by a school employee at some time between kindergarten and the twelfth grade (K-12). GAO was asked to (1) examine the circumstances surrounding cases where K-12 schools hired or retained individuals with histories of sexual misconduct and determine the factors contributing to such employment actions and (2) provide an overview of selected federal and state laws related to the employment of convicted sex offenders in K-12 schools. To identify case studies, GAO compared 2007 to 2009 data employment databases from 19 states and the District of Columbia to data in the National Sex Offender Registry. GAO also searched public records from 2000 to 2010 to identify cases in which sexual misconduct by school employees ultimately resulted in a criminal conviction. However, states often define charter schools differently than traditional public schools. Some charter schools operate as a school district, while others are part of a school district and some are for-profit entities. These differences could complicate eligibility determination for federal administrators. GAO was asked: (1) To what extent do charter schools apply for federal discretionary grant programs and what challenges do they face, if any, in doing so? (2) What role has the U.S. Department of Education played in helping charter schools establish their eligibility for federal discretionary grant programs? GAO identified grant programs governmentwide for which charter schools are eligible to apply, surveyed a stratified random sample of charter school officials, and interviewed federal agency officials. Correspondence issued by the Government Accountability Office with an abstract that begins "This report responds to House Report 111-166 to accompany the House bill (H.R. 2647) that later became the National Defense Authorization Act for Fiscal Year 2010. The House Report noted the House Armed Services Committee's concerns that the lessons learned regarding the prevention and management of corrosion in the F-22 Raptor had not been fully applied to the development and acquisition of the F-35 Joint Strike Fighter. The House Report directed that the Director of Corrosion Policy and Oversight evaluate the F-35 program and submit a report to the defense committees within 180 days after the act was enacted. The Department of Defense (DOD) report was also to include implications for existing and future weapon systems based on the findings of the F-35 evaluation. DOD submitted its report to Congress in September 2010. Defense Management: DOD Has a Rigorous Process to Select Corrosion Prevention Projects, but Would Benefit from Clearer Guidance and Validation of Returns on Investment

A letter report issued by the Government Accountability Office with an abstract that begins "Corrosion costs DOD over $23 billion annually, affects both equipment and facilities, and threatens personnel safety. DOD has taken steps to improve its corrosion prevention and control (CPC) efforts. These efforts include reorganizing the DOD-wide Corrosion Office and instituting Corrosion Executive positions in each of the military departments. In response to the Senate Appropriations Committee Report accompanying the fiscal year 2010 DOD appropriations bill, GAO evaluated to what extent (1) the Corrosion Executives are involved in preparing CPC project proposals for submission, (2) the Corrosion Office has created a process to review and select projects for funding, and (3) the military departments have validated the return on investment (ROI) for funded projects. GAO also reviewed the process the Corrosion Office uses to determine the CPC activities that it will fund. To carry out this study, GAO observed project selection panel meetings, interviewed corrosion officials, and reviewed documents and project proposals."

2010 Census: Key Efforts to Include Hard-to-Count Populations Went Generally as Planned; Improvements Could Make the Efforts More Effective for Next Census

A letter report issued by the Government Accountability Office with an abstract that begins "To overcome the long-standing challenge of enumerating hard-to-count (HTC) groups such as minorities and renters, the U.S. Census Bureau (Bureau), used outreach programs, such as paid advertising, and partnered with thousands of organizations to enlist their support for the census. The Bureau also conducted Service-Based Enumeration (SBE), which was designed to count people who frequent soup kitchens or other service providers, and the Be Counted/Questionnaire Assistance Center (QAC) program, designed to count individuals who believed the census had missed them. As requested, GAO assessed how the design of these efforts compared to 2000 and the extent to which they were implemented as planned. GAO reviewed Bureau budget, planning, operational, and evaluation documents; observed enumeration efforts in 12 HTC areas; surveyed local census office managers; and interviewed Bureau officials."
