Aviation Safety: Improved Data Collection Needed for Effective Oversight of Air Ambulance Industry

A letter report issued by the Government Accountability Office with an abstract that begins "Air ambulance transport is widely regarded as improving the chances of survival for trauma victims and other critical patients. However, in recent years, the number of air ambulance accidents has led to increased industry scrutiny by government agencies, the public, the media, and the industry itself. The Federal Aviation Administration (FAA), which provides safety oversight, has been called upon by the National Transportation Safety Board (NTSB) and others to issue more stringent safety requirements for the industry. GAO's study addressed (1) recent trends in the air ambulance industry, (2) FAA's challenges in providing safety oversight, and (3) FAA's efforts to address the challenges and what is known about the effects of these efforts. To address these issues, we analyzed FAA, NTSB, and industry data, interviewed federal and industry officials, and conducted five site visits, among other things."

A letter report issued by the Government Accountability Office with an abstract that begins "Nearly three decades after the last order for a new nuclear power reactor in the United States, electric power companies plan to submit 20 applications in the next 18 months to the Nuclear Regulatory Commission (NRC) for licenses to build and operate new reactors. Since 1989, NRC has developed a new license review process that allows a power company to obtain a construction permit and an operating license through a single combined license (COL) based on one of a number of standard reactor designs. NRC expects its new process to enhance the efficiency and predictability of its reviews. GAO reviewed NRC's readiness to evaluate these applications by examining the steps NRC has taken to (1) prepare its workforce and manage its workload and (2) develop its regulatory framework and review process for new reactor activities. GAO reviewed NRC documents for new reactor workforce staffing and training, examined NRC's guidance for the review of license applications, interviewed NRC managers and representatives of nearly all of the COL applicants, and observed NRC's public meetings."

VA Health Care: VA Should Better Monitor Implementation and Impact of Capital Asset Alignment Decisions

A letter report issued by the Government Accountability Office with an abstract that begins "Through its Veterans Health Administration (VHA), the Department of Veterans Affairs (VA) operates one of the largest health care systems in the country. In 1999, GAO reported that better management of VA's large inventory of aged capital assets could result in savings that could be used to enhance health care services for veterans. In response, VA initiated a process known as Capital Asset Realignment for Enhanced Services (CARES). Through CARES, VA sought to enhance veteran care by the appropriate sizing, upgrading, and locating of VA facilities. GAO was asked to examine the CARES process. Specifically, GAO examined (1) how CARES contributes to VHA's capital planning process, (2) the extent to which the CARES process considered capital asset alignment alternatives, and (3) the extent to which VA has implemented CARES decisions and how this implementation has helped VA carry out its mission. To address these issues, we analyzed CARES documents, interviewed VA officials, and conducted six site visits, among other things."

A letter report issued by the Government Accountability Office with an abstract that begins "Pandemic influenza poses a threat to public health at a time when the United Nations' World Health Organization (WHO) has said that infectious diseases are spreading faster than at any time in history. The last major influenza pandemic occurred from 1918 to 1919. Estimates of deaths worldwide if a similar pandemic were to occur have ranged between 30 million and 384 million people. Individual countries and international organizations have developed and begun to implement a strategy for forestalling (that is, containing, delaying, or minimizing the impact of) the onset of a pandemic. Antivirals and vaccines may help forestall a pandemic. GAO was asked to examine (1) constraints upon the use of antivirals and vaccines to forestall a pandemic and (2) efforts under way to overcome these constraints. GAO reviewed documents and consulted with officials of the Departments of State and Health and Human Services (HHS), international organizations, and pharmaceutical manufacturers. WHO commented that the report was comprehensive and useful. HHS stressed that vaccines and antivirals must be viewed in a larger context. Correspondence issued by the Government Accountability Office with an abstract that begins "The Department of Defense (DOD) built the Cheyenne Mountain Operations Center located near Colorado Springs, Colorado, in the early 1960s to withstand a multimegaton-yield-weapon strike and to provide protection against chemical and biological warfare. The mission of the Cheyenne Mountain Directorate is to monitor, process, and interpret air, missile, and space events that could threaten North America or have operational effects on U.S. forces or capabilities. This mission is conducted at five major centers--the Command Center, Air Warning, Missile Correlation, Operations Intelligence Watch, and Space Control--all currently located within Cheyenne Mountain. Elements of United States Strategic Command (USSTRATCOM) and Air Force Space Command are also located in Cheyenne Mountain. The Air Force's modernization of the attack warning systems within Cheyenne Mountain will cost more than $700 million from fiscal years 2000 through 2006. DOD officials have stated that they no longer need to continue operating in this hardened facility considering that the threat of an intercontinental ballistic missile strike in today's environment is low. LLRW ranges from very low-activity to higher-activity waste. To identify potential approaches to overcome these limitations, GAO was asked to examine the extent to which other countries have (1) LLRW inventory databases, (2) timely removal of higher-activity LLRW from waste generator sites, (3) disposition options for all LLRW, and (4) requirements that LLRW generators have financial reserves to cover waste disposition costs, as well as any other approaches that might improve U.S. LLRW management. GAO primarily relied on a survey of 18 countries representing leading LLRW generators to identify their management approaches and to compare them with U.S. survey results and with approaches suggested by LLRW generators, disposal operators, and regulators in the United States."

DOD intends to set premiums at a level equal to the expected costs of providing TRS benefits. The National Defense Authorization Act for 2007 required GAO to review TRS costs. As discussed with the committees of jurisdiction, GAO compared (1) the TRS premiums established by DOD to the reported costs of providing benefits under TRS in 2006 and (2) DOD's projected costs for TRS before implementation to DOD's reported costs for the program in 2005 and 2006. Correspondence issued by the Government Accountability Office with an abstract that begins "The federal government widely uses magnetic tapes for data storage and data recovery. According to allegations made by a magnetic-tape company official, federal agencies are selling used magnetic tapes containing sensitive government data to companies which then resell them to the general public. While this is not an illegal practice, Congress is concerned that magnetic tapes containing sensitive government data have become available to the public in this manner. There is no general legal requirement that the government erase all data on all magnetic tapes before disposing of them. However, the National Institute of Standards and Technology (NIST) has issued guidelines that instruct agencies to properly sanitize magnetic tapes with certain kinds of sensitive data before they leave agency control. In its guidelines, NIST defines sanitization as the general process of removing data from storage media, such that there is reasonable assurance that the data may not be easily retrieved and reconstructed."

FAA's current authorization expires in 6 months. The proposal calls for major changes to FAA's funding and budget structure that are intended to address concerns about the long-term revenue adequacy, equity, and efficiency of FAA's current funding structure and to provide a more stable, reliable basis for funding a new air traffic control system that FAA is developing (at an estimated cost of $15 billion to 22 billion through 2025) to meet forecasted increases in air travel demand. The proposal would introduce cost-based charges for commercial users of air traffic control services, eliminate many current taxes, substantially raise fuel taxes for general aviation users, charge commercial and general aviation users a fuel tax to pay primarily for airport capital improvements, modify FAA's budget accounts to align with specific FAA activities, and link the portion of FAA's budget that comes from the Treasury's General Fund with public benefits FAA provides. Testimony issued by the Government Accountability Office with an abstract that begins "The United States is the largest provider of food aid in the world, accounting for over half of all global food aid supplies intended to alleviate hunger. Since the 2002 reauthorization of the Farm Bill, Congress has appropriated an average of $2 billion per year for U.S. food aid programs, which delivered an average of 4 million metric tons of agricultural commodities per year. Despite growing demand for food aid, rising business and transportation costs have contributed to a 43-percent decline in average tonnages delivered over the last 5 years. For the largest U.S. food aid program, these costs represent approximately 65 percent of total food aid expenditures, highlighting the need to maximize the efficiency and effectiveness of food aid. To inform Congress as it reauthorizes the 2007 Farm Bill, GAO examined some key challenges to the (1) efficiency of delivery and (2) effective monitoring of U.S. food aid."

Testimony issued by the Government Accountability Office with an abstract that begins "Since its creation, the Social Security number (SSN) has evolved beyond its intended purpose to become the identifier of choice for public and private sector entities, and it is now used for myriad non-Social Security purposes. This is significant because a person's SSN, along with name and date of birth, are the key pieces of personal information used to perpetrate identity theft. Consequently, the potential for misuse of the SSN has raised questions about how private and public sector entities obtain, use, and protect SSNs. Accordingly, this testimony focuses on describing the (1) use of SSNs by government agencies, (2) use of SSNs by the private sector, and (3) vulnerabilities that remain to protecting SSNs. For this testimony, we primarily relied on information from our prior reports and testimonies that address public and private sector use and protection of SSNs. These products were issued between 2002 and 2006 and are listed in the Related GAO Products section at the end of this statement. We conducted our reviews in accordance with generally accepted government auditing standards."

Testimony issued by the Government Accountability Office with an abstract that begins "Under the Medicare Prescription Drug, Improvement, and Modernization Act of 2003 (MMA), dual-eligible beneficiaries--individuals with both Medicare and Medicaid coverage--have their drug costs covered under Medicare Part D rather than under state Medicaid programs. The MMA requires the Centers for Medicare & Medicaid Services (CMS) to enroll these beneficiaries in a Medicare prescription drug plan (PDP) if they do not select a plan on their own. CMS enrolled about 5.5 million dual-eligible beneficiaries in late 2005 and about 634,000 who became dually eligible during 2006. GAO was asked to testify on (1) CMS's process for enrolling new dual-eligible beneficiaries into PDPs and its effect on access to drugs and (2) how CMS set the effective coverage date for certain dual-eligible beneficiaries and its implementation of this policy. Correspondence issued by the Government Accountability Office with an abstract that begins "The United States Marine Corps and the Special Operations Command determined in 1999 that there was a need for a weapon system that could be carried inside the V-22 Osprey and deployed to support assault operations. The Expeditionary Fire Support System (EFSS), which consists of a vehicle that tows a 120 mm mortar and another vehicle that tows an ammunition trailer, is one of the systems now being developed to meet this need. In June 2005, the Marine Corps approved low-rate initial production of 6 EFSS units--each unit made up of two vehicles, a mortar and ammunition trailer. From May to July 2007, the Marine Corps Operational Test and Evaluation Activity, the independent test agency for the Marines, conducted initial operational testing and evaluation of the EFSS using refurbished developmental prototypes. The program office began receiving the low-rate production mortars in late October 2007 and is now going through the acceptance process. The delivery of the low-rate production vehicles to the government was delayed until mid-November 2007 so that problems identified during the summer 2007 operational test could be fixed. Prioritization of WAS Enhancement Ideas

This report is part of the Web-at-Risk project. Several enhancement ideas for future releases of the Web Archiving Service (WAS) have been identified. In order to refine and prioritize these ideas as well as elicit new enhancement ideas, 16 project curators participated in an online exercise between November 27 and December 16, 2007. The exercise employed a Delphi technique that consisted of a series of three online questionnaires, each available for a five or six day period.
