Mental Health: Community-Based Care Increases for People With Serious Mental Illness

However, federal mental health spending grew at more than twice the rate of state and local spending. The ability to care for more people in the community has been facilitated by the continued development of new medications that have fewer side effects and are more effective in helping people manage their illness. Furthermore, treatment approaches, such as assertive community treatment, supported employment, and supportive housing, provide the ongoing assistance that adults with serious mental illness (SMI) often need to function in the community. The Health Care Financing Administration (HCFA) has encouraged the use of community-based services for Medicaid beneficiaries with SMI by disseminating information on the use of new medications and treatment models, which can help people function better in the community. A letter report issued by the General Accounting Office with an abstract that begins "The Nuclear Regulatory Commission (NRC) has decided to move its technical training center from Chattanooga, Tennessee, to near its headquarters in Rockville, Maryland. The relocation is intended to improve the effectiveness and efficiency of the agency's operations. The center provides diverse training curricula on such topics as nuclear power plant technology augmented by training on simulators, risk assessment, radiation protection, and regulatory skills. NRC used a reasonable methodology to determine the costs of relocating its facility. It considered costs related to staff salaries, travel costs for center participants, and lease payments. However, NRC used several assumptions that, if changed, would affect the cost of the move. For example, a change in the number of staff who would relocate would have an impact on costs. According to NRC, the objective of the move was not to minimize the cost of operating the center but rather to improve the effectiveness and efficiency of the agency's operations. Interview with Navy veteran Melvin Fenoglio, educator and farmer-rancher. The interview includes Fenoglio's personal experiences about the Pacific Theater during World War II, and the Iwo Jima and Okinawa Campaigns. Fenoglio also talks about early family history, his pre-war education, his acceptance in the V-7 Program, failing out of the Midshipman School and transferring to the U.S. Naval Training Center, yeoman training, gunnery practice off the Hawaiian Islands, his personal observations of the flag-raisings on Mount Suribachi, the USS Little's assignment to radar picket duty at Station Ten, his ship being hit by four kamikazes and sinking on May 3, 1945, rescue in the water by his shipmates, survival in the water for three hours before being picked up by LCS(L)-25, and the lasting effects of his World War II experiences. The interview also includes an appendix with a map.

Single-mode operation of a coiled multimode fiber amplifier

The authors report a new approach to obtain single-transverse-mode operation of a multimode fiber amplifier, in which the gain fiber is coiled to induce significant bend loss for all but the lowest-order mode. They have demonstrated this method by constructing a coiled amplifier using Yb-doped, double-clad fiber with a core diameter of 25 {micro}m and NA of {minus}0.1 (V {approx} 7.4). When operated as an ASE source, the output beam had an M{sup 2} value of 1.09 {+-} 0.09; when seeded at 1,064 nm, the slope efficiency was similar to that of an uncoiled amplifier. This technique does not require exotic fiber designs or increase system complexity and is inexpensive to implement. It will allow scaling of pulsed fiber lasers and amplifiers to significantly higher pulse energies and peak powers and cw fiber sources to higher average powers while maintaining excellent beam quality.
