Savannah River Site Human Error Data Base Development for Nonreactor Nuclear Facilities

As part of an overall effort to upgrade and streamline methodologies for safety analyses of nonreactor nuclear facilities at the Savannah River Site (SRS), a human error data base has been developed and is presented in this report. The data base fulfills several needs of risk analysts supporting safety analysis report (SAR) development. First, it provides a single source for probabilities or rates for a wide variety of human errors associated with the SRS nonreactor nuclear facilities. Second, it provides a documented basis for human error probabilities or rates. And finally, it provides actual SRS-specific human error data to support many of the error probabilities or rates. Use of a single, documented reference source for human errors, supported by SRS-specific human error data, will improve the consistency and accuracy of human error modeling by SRS risk analysts. It is envisioned that SRS risk analysts will use this report as both a guide to identifying the types of human errors that may need to be included in risk models such as fault and event trees, and as a source for human error probabilities or rates. This phase report summarizes the results of field investigations conducted in 1992 at Technical Area 21 of Los Alamos National Laboratory, as prescribed by the RCRA Facility Investigation work plan for the Technical Area 21 operable unit (also known as OU 1106). This phase report is the last part of a three-part phase report describing the results of field work conducted in 1992 at this operable unit. Phase Report lA, issued on l4 June l993, summarized site geologic characterization activities. Phase report 1B, issued on 28 January 1994, included an assessment of site-wide surface soil background, airborne emissions deposition, and contamination in the locations of two former air filtration buildings. The investigations assessed in Phase Report 1C include field radiation surveys and surface and near-surface sampling to characterize potential contamination at 25 outfalls and septic systems listed as SWMUs in the RFI work plan. Based on the RFI data, it is recommended that no further action is warranted for 8 SWMUs and further action is recommended for 3 SWMUs addressed in this phase report. For 14 SWMUs which represent no immediate threat to human health or environment, deferral of further action/no further action decisions is recommended until outstanding analytical data …

Search Hanford accessible reports electronically system requirements specification

The Emergency, Safety, & Quality (ESQ) Services organization of Westinghouse Hanford Company (WHC) is experiencing changes related to new programs. The programs include Corrective Actions Management Systems, Self-Assessment, Trend Analysis, and Lessons Learned analysis. The programs are pushing up organizational costs while funding and manpower levels are projected to be dropping. A large cost involved in implementing the new programs is the cost for people to locate unstructured information required to make decisions or write documentation. But, most resources for retrieving information are found in structured database systems. This means that unstructured information that must be located has to be found by one of these methods: (1) searching manually through documents, (2) searching individual documents one at a time with word processing programs, (3) searching through text fields with query systems primarily designed for structured database information, or (4) develop a text search and retrieval system designed for unstructured information. One example of this is using Occurrence Reporting Processing System (ORPS) and Quality, Environmental, and Safety Tracking (QUEST) documents. An explanation of this is in the following textual box.
