A letter report issued by the Government Accountability Office with an abstract that begins "Safe and accessible federal courthouses are critical to the U.S. judicial process. The Federal Protective Service (FPS), within the Department of Homeland Security (DHS), the U.S. Marshals Service (Marshals Service), within the Department of Justice (DOJ), the Administrative Office of the U.S. Courts (AOUSC), and the General Services Administration (GSA) are the federal stakeholders with roles related to courthouse security. As requested, this report addresses (1) attributes that influence courthouse security considerations and (2) the extent to which stakeholders have collaborated in implementing their responsibilities and using risk management. GAO analyzed laws and documents, such as security assessments; reviewed GAO's work on key practices for collaboration and facility protection; visited 11 courthouse facilities, selected based on geographic dispersion, age, size, and other criteria; and interviewed agency and judiciary officials. While the results from site visits cannot be generalized, they provided examples of courthouse security activities."

Head Start: Undercover Testing Finds Fraud and Abuse at Selected Head Start Centers

A letter report issued by the Government Accountability Office with an abstract that begins "The Head Start program provides child development services primarily to low-income families and their children. Federal law allows up to 10 percent of families to have incomes above 130 percent of the poverty line--GAO refers to them as over-income families. Families with incomes below 130 percent of the poverty line, or that meet certain other criteria, are referred to as under-income families. Nearly 1 million children a year participate in Head Start, and the American Recovery and Reinvestment Act provided an additional $2.1 billion. GAO received hotline tips alleging fraud by grantees. In response, GAO investigated the allegations, conducted undercover tests to determine if other centers were committing fraud, and documented instances where potentially eligible children were put on Head Start wait lists. On May 18, 2010 GAO testified on the preliminary results of the ongoing investigation. This report reiterates the findings disclosed in GAO's May testimony, and discusses new findings related to specific fraud allegations at two Head Start grantees. Department of Homeland Security: Efforts to Assess Realignment of Its Field Office Structure

Correspondence issued by the Government Accountability Office with an abstract that begins "DHS reported taking some steps to assess the realignment of its regional/field office structure. Since submitting an initial plan to Congress in 2004 that outlined regionalization, consolidation, and colocation opportunities, DHS officials said the agency considered the potential implementation of a unified regional field office structure through two major initiatives--the 2004 I-Staff review and the department's 2010 BUR. However, the I-Staff Regional Concept of Operations was not finalized or adopted, and in April 2012 a senior DHS official involved in the BUR effort stated that DHS no longer intends to implement the BUR recommendation related to regionalization because it is no longer the department's preferred approach. DHS had limited or no documentation related to either of these reviews, including the resulting key decisions from the efforts. As a result, we are unable to determine the extent to which DHS has fully considered the realignment of its regional/field office structure, including costs and benefits, since 2004. Freight Railroad Safety: Hours of Service Changes Have Increased Rest Time, but More Can Be Done to Address Fatigue Risks

A letter report issued by the Government Accountability Office with an abstract that begins "The Rail Safety Improvement Act of 2008 (RSIA) overhauled requirements for how much time certain freight railroad workers can spend on the job (called "hours of service"). Changes included limiting the number of consecutive days on duty before rest is required, increasing minimum rest time from 8 to 10 hours, and requiring rest time to be undisturbed. RSIA also provided for pilot projects and waivers. RSIA's changes became effective for freight railroads in July 2009. GAO was asked to assess (1) the impact of these changes on covered train and engine (T&E) employees, including implications for fatigue, (2) the impact of the changes on the rail industry, and (3) actions the Federal Railroad Administration (FRA) has taken to oversee compliance with hours of service requirements and implement RSIA provisions for pilot projects and waivers. To perform this work, GAO analyzed covered employee work schedules and used models to assess fatigue, surveyed the railroad industry, analyzed FRA inspection and enforcement data, and interviewed federal and railroad officials as well as fatigue and sleep experts."

A letter report issued by the Government Accountability Office with an abstract that begins "The 40 poorest countries in the world, known as low-income countries (LICs), have been negatively impacted by successive food, fuel, and financial crises since 2007. In response, international financial institutions (IFI), including the World Bank and International Monetary Fund (IMF), have taken actions to increase financial assistance for affected countries. Between 2008 and 2010, Congress appropriated $3.3 billion to the World Bank's International Development Association, which funds development programs in LICs. Congress also authorized the U.S. representative at the IMF to vote to approve the sale of some of the IMF's gold to increase lending to LICs. LICs' ability to repay debt remains important as financing levels rise and decisions are made about the mix of loans and grants they receive. GAO was asked to examine (1) the economic impact of the crises on LICs, (2) IFIs' responses and reported results, and (3) IFIs' assessment of the impact of the crises on LICs' ability to repay their debt. Department of Homeland Security: Taking Further Action to Better Determine Causes of Morale Problems Would Assist in Targeting Action Plans

A letter report issued by the Government Accountability Office with an abstract that begins "Department of Homeland Security (DHS) employees reported having lower average morale than the average for the rest of the federal government, but morale varied across components and employee groups within the department. Data from the 2011 Office of Personnel Management (OPM) Federal Employee Viewpoint Survey (FEVS)--a tool that measures employees' perceptions of whether and to what extent conditions characterizing successful organizations are present in their agencies--showed that DHS employees had 4.5 percentage points lower job satisfaction and 7.0 percentage points lower engagement in their work overall. Engagement is the extent to which employees are immersed in their work and spending extra effort on job performance. Moreover, within most demographic groups available for comparison, DHS employees scored lower on average satisfaction and engagement than the average for the rest of the federal government. For example, within most pay categories DHS employees reported lower satisfaction and engagement than non-DHS employees in the same pay groups. Levels of satisfaction and engagement varied across components, with some components reporting scores above the non-DHS averages. Correspondence issued by the Government Accountability Office with an abstract that begins "Combating weapons of mass destruction (WMD) and their means of delivery is one of the greatest challenges the United States faces. Traditionally, the use of WMD--which include chemical, biological, radiological, or nuclear weapons--has been constrained by the logic of deterrence and of diplomacy, but these constraints may be of less utility in preventing the use of WMD by rogue states or terrorist groups. The Department of Defense (DOD) assigns top priority to dissuading, deterring, and defeating those who seek to harm the United States directly, especially extremist enemies with WMD. In 1994, Congress established an interagency committee, now known as the Counterproliferation Program Review Committee (CPRC), with a variety of duties related to coordinating the activities and programs of federal agencies that address improvements in the U.S. government's efforts to combat WMD. The Secretary of Defense, as chairman of the CPRC, is required to report its findings biennially. The Departments of Energy, State, and Homeland Security and the Office of the Director of National Intelligence are also members of the CPRC, and must provide it with access to information on all pertinent programs, projects, and activities. A letter report issued by the Government Accountability Office with an abstract that begins "In fiscal year 2010, more than half of the $367 billion dollars the Department of Defense (DOD) spent on contracts was spent on services. Buying services is fundamentally different than buying weapon systems, yet most acquisition regulations, policies, processes, and training remain structured for acquiring weapon systems. Over the last decade, reports from GAO, DOD, and Congress have raised issues about services acquisitions and have also highlighted the importance of acquisition training. GAO previously reported on the training provided to the acquisition workforce as defined by the Defense Acquisition Workforce Improvement Act (DAWIA). This report addresses personnel working on services acquisitions who were outside the DAWIA acquisition workforce--termed non-DAWIA personnel with acquisition-related responsibilities--and the extent to which (1) DOD knows the composition of this population, (2) this population is taking acquisition training, and (3) DOD has implemented past recommendations related to this population. To complete this work, GAO reviewed a nongeneralizable sample of 29 service contracts, relevant policies, and recommendations from previous reports and met with key DOD officials."

A letter report issued by the Government Accountability Office with an abstract that begins "The military services have a degree of discretion regarding whether and how to apply Department of Defense (DOD) acquisition guidance for their uniform development and they varied in their usage of that guidance. As a result, the services had fragmented procedures for managing their uniform development programs, and did not consistently develop effective camouflage uniforms. GAO identified two key elements that are essential for producing successful outcomes in acquisitions: 1) using clear policies and procedures that are implemented consistently, and 2) obtaining effective information to make decisions, such as credible, reliable, and timely data. The Marine Corps followed these two key elements to produce a successful outcome, and developed a uniform that met its requirements. By contrast, two other services, the Army and Air Force, did not follow the two key elements; both services developed uniforms that did not meet mission requirements and had to replace them. Without additional guidance from DOD on the use of clear policies and procedures and a knowledge-based approach, the services may lack assurance that they have a disciplined approach to set requirements and develop new uniforms that meet operational needs."

A letter report issued by the Government Accountability Office with an abstract that begins "The Department of Veterans Affairs (VA) and Department of Defense (DOD) do not require that all collaboration sites--locations where the two departments share health care resources through hundreds of agreements and projects--develop and use performance measures to assess their effectiveness and efficiency. Officials cited several reasons for this, including not wanting to overburden sites with measures and monitoring requirements. Although VA and DOD require some limited performance information--such as the return on investment for pilot projects--without comprehensive performance measures, they lack information that could help decision makers assess collaboration sites' overall progress in meeting the departments' shared goals of improved health care access, quality, and costs; identify areas for improvement; and make informed decisions. Also, the departments cannot document the overall cost effectiveness of their collaboration efforts. In the absence of required measures for all sites, some have developed their own, but these fragmented efforts do not provide sufficient information about the overall results of collaborations."

A letter report issued by the Government Accountability Office with an abstract that begins "The Department of Labor (Labor) was challenged to process the substantial increase in petitions filed for the Trade Adjustment Assistance (TAA) for Workers program after related legislation was enacted in 2009. Labor initially had insufficient capacity to handle this increased workload, leading to processing delays and data recording errors. For example, in the quarter after the 2009 legislation took effect, Labor took an average of 153 days to process a petition—nearly four times the statutory limit. Labor responded with corrective action, including hiring new staff and adding additional quality control steps for processing petitions. Partly as a result of these efforts, processing times fell substantially. Moreover, GAO found that Labor's petition investigation process, as of June 2012, generally conformed to best practices for internal controls."

A letter report issued by the Government Accountability Office with an abstract that begins "While their views varied to some degree, federal agency officials and advocacy groups GAO contacted identified a number of challenges that small, minority-owned businesses may face in pursuing federal government contracts. For example, officials and advocacy groups pointed to a lack of performance history and knowledge of the federal contracting process as significant barriers. Officials from advocacy groups cited additional challenges, such as difficulty gaining access to contracting officials and decreased contracting opportunities resulting from contract bundling—the consolidation of two or more contracts previously performed under smaller contracts, into a single contract. Officials from agencies that accounted for 70 percent of federal contracting with small, minority-owned businesses—(the Departments of Defense, Health and Human Services, and Homeland Security, and the General Services Administration) told GAO that they conducted outreach to help small, minority-owned businesses with these challenges. Their outreach efforts include one-on-one interviews between contracting office staff and businesses seeking federal contracts. Linguistic and cultural barriers were identified as a challenge on a limited basis."

A letter report issued by the Government Accountability Office with an abstract that begins "We identified five major actions the Food and Drug Administration (FDA) is to complete under the FDA Food Safety Modernization Act (FSMA) to establish a reliable system that uses third-party audits conducted by foreign governments or other third parties to help ensure food safety. FDA officials and others report that each of these actions presents challenges that must be addressed. First, FDA is to develop new preventive controls and related guidance for all of the foods under its jurisdiction--such as produce, milk, cheese, spices, soft drinks, and processed foods--and will need to develop appropriate training, particularly for foreign producers and processors, which poses a challenge because FDA is responsible for a variety of food industries. Second, FDA is to establish a voluntary user fee program for importers that encourages the use of third-party certifications, and it faces a challenge in developing a program that encourages importers to participate. Testimony issued by the Government Accountability Office with an abstract that begins "This testimony discusses issues related to women in management. Although women's representation across the general workforce is growing, there remains a need for information about the challenges women face in advancing their careers. In 2001, using 1995 and 2000 data from the Current Population Survey, we found women were less represented in management than in the overall workforce in 4 of the 10 industries reviewed. We also found differences in the characteristics and pay of male and female managers, which we explored using statistical modeling techniques. To respond to Congress' request that we update this information to 2007, we addressed the following three questions: (1) What is the representation of women in management positions compared to their representation in nonmanagement positions by industry? (2) What are the key characteristics of women and men in management positions by industry? and (3) What is the difference in pay between women and men in full-time management positions by industry?"
