Accident investigation board report on the May 14, 1997, chemical explosion at the Plutonium Reclamation Facility, Hanford Site,Richland, Washington - final report

On May 14, 1997, at 7:53 p.m. (PDT), a chemical explosion occur-red in Tank A- 109 in Room 40 of the Plutonium Reclamation Facility (Facility) located in the 200 West Area of the Hanford Site, approximately 30 miles north of Richland, Washington. The inactive processing Facility is part of the Plutonium Finishing Plant (PFP). On May 16, 1997, Lloyd L. Piper, Deputy Manager, acting for John D. Wagoner, Manager, U.S. Department of Energy (DOE), Richland Operations Office (RL), formally established an Accident Investigation Board (Board) to investigate the explosion in accordance with DOE Order 225. 1, Accident Investigations. The Board commenced its investigation on May 15, 1997, completed the investigation on July 2, 1997, and submitted its findings to the RL Manager on July 26, 1997. The scope of the Board`s investigation was to review and analyze the circumstances of the events that led to the explosion; to analyze facts and to determine the causes of the accident; and to develop conclusions and judgments of need that may help prevent a recurrence of the accident. Advanced Reservoir Characterization in the Antelope Shale to Establish the Viability of CO(2) Enhanced Oil Recovery in California`s Monterey formation Siliceous Shales. Progress report, April 1-June 30, 1997

The primary objective of this research is to conduct advanced reservoir characterization and modeling studies in the Antelope Shale reservoir. Characterization studies will be used to determine the technical feasibility of implementing a C0{sub 2} enhanced oil recovery project in the Antelope Shale in Buena Vista Hills Field. The Buena Vista Hills Pilot C0{sub 2} project will demonstrate the economic viability and widespread applicability of C0{sub 2} flooding in fractured siliceous shale reservoirs of the San Joaquin Valley. The research consists of four primary work processes: Reservoir Matrix and Fluid Characterization; Fracture Characterization; Reservoir Modeling and Simulation; and C0{sub 2} Pilot Flood and Evaluation. Work done in these areas is subdivided into two phases or budget periods. The first phase of the project will focus on the application of a variety of advanced reservoir characterization techniques to determine the production characteristics of the Antelope Shale reservoir. Reservoir models based on the results of the characterization work will be used to evaluate how the reservoir will respond to secondary recovery and EOR processes.
