National Preparedness: HHS Is Monitoring the Progress of Its Medical Countermeasure Efforts but Has Not Provided Previously Recommended Spending Estimates

A letter report issued by the Government Accountability Office with an abstract that begins "The Department of Health and Human Services (HHS) has established timelines and milestones for the 72 Public Health Emergency Medical Countermeasures Enterprise (PHEMCE) priorities--33 activities, 25 threat-based approaches, and 14 capabilities--that HHS selected as key to fulfilling PHEMCE strategic goals. However, HHS has not made spending estimates for its medical countermeasure development or procurement priorities (priority countermeasures) publicly available. In the PHEMCE implementation plan, HHS has grouped the 72 PHEMCE priorities into three time frames for completion--near-term (fiscal years 2012 through 2014), midterm (fiscal years 2015 through 2017), and long-term (fiscal year 2018 and beyond). For 21 priority activities, 10 priority threat-based approaches, and 8 priority capabilities, HHS and PHEMCE have identified specific deliverables, each tied to a milestone or set of milestones that delineate the steps necessary to complete deliverables, and established more specific timelines for completion of deliverables and milestones. Simplification of Methods for PET Radiopharmaceutical Syntheses

In an attempt to develop simplified methods for radiochemical synthesis of radiopharmaceuticals useful in Positron Emission Tomography (PET), current commercially available automated synthesis apparati were evaluated for use with solid phase synthesis, thin-film techniques, microwave-accelerated chemistry, and click chemistry approaches. Using combinations of these techniques, it was shown that these automated synthesis systems can be simply and effectively used to support the synthesis of a wide variety of carbon-11 and fluorine-18 labeled compounds, representing all of the major types of compounds synthesized and using all of the common radiochemical precursors available. These techniques are available for use to deliver clinically useful amounts of PET radiopharmaceuticals with chemical and radiochemical purities and high specific activities, suitable for human administration.

TECHNOLOGY DEVELOPMENT AND DEPLOYMENT OF SYSTEMS FOR THE RETRIEVAL AND PROCESSING OF REMOTE-HANDLED SLUDGE FROM HANFORD K-WEST FUEL STORAGE BASIN

In 2011, significant progress was made in developing and deploying technologies to remove, transport, and interim store remote-handled sludge from the 105-K West Fuel Storage Basin on the Hanford Site in south-central Washington State. The sludge in the 105-K West Basin is an accumulation of degraded spent nuclear fuel and other debris that collected during long-term underwater storage of the spent fuel. In 2010, an innovative, remotely operated retrieval system was used to successfully retrieve over 99.7% of the radioactive sludge from 10 submerged temporary storage containers in the K West Basin. In 2011, a full-scale prototype facility was completed for use in technology development, design qualification testing, and operator training on systems used to retrieve, transport, and store highly radioactive K Basin sludge. In this facility, three separate systems for characterizing, retrieving, pretreating, and processing remote-handled sludge were developed. Two of these systems were successfully deployed in 2011. One of these systems was used to pretreat knockout pot sludge as part of the 105-K West Basin cleanup. Annual Hanford Seismic Report for Fiscal Year 2010

The Hanford Seismic Assessment Program (HSAP) provides an uninterrupted collection of high-quality raw and processed seismic data from the Hanford Seismic Network for the U.S. Department of Energy and its contractors. The HSAP is responsible for locating and identifying sources of seismic activity and monitoring changes in the historical pattern of seismic activity at the Hanford Site. The data are compiled, archived, and published for use by the Hanford Site for waste management, natural phenomena hazards assessments, and engineering design and construction. In addition, the HSAP works with the Hanford Site Emergency Services Organization to provide assistance in the event of a significant earthquake on the Hanford Site. The Hanford Seismic Network and the Eastern Washington Regional Network consist of 44 individual sensor sites and 15 radio relay sites maintained by the Hanford Seismic Assessment Team. During FY 2010, the Hanford Seismic Network recorded 873 triggers on the seismometer system, which included 259 seismic events in the southeast Washington area and an additional 324 regional and teleseismic events. There were 210 events determined to be local earthquakes relevant to the Hanford Site. Final Report: A CdZnTe detector for MRI-compatible SPECT Systems

The key objective of this project is to develop the enabling technology for future MRI-compatible nuclear (e.g. SPECT) imaging system, and to demonstrate the feasibility of performing simultaneous MR and SPECT imaging studies of the same object. During the past three years, we have developed (a) a MRI-compatible ultrahigh resolution gamma ray detector and associated readout electronics, (b) a theoretical approach for modeling the effect of strong magnetic field on SPECT image quality, and (c) a maximum-likelihood (ML) based reconstruction routine with correction for the MR-induced distortion. With this support, we have also constructed a four-head MR-compatible SPECT system and tested the system inside a 3-T clinical MR-scanner located on UI campus. The experimental results obtained with this system have clearly demonstrated that sub-500um spatial resolution can be achieved with a SPECT system operated inside a 3-T MRI scanner. During the past three years, we have accomplished most of the major objectives outlined in the original proposal. These research efforts have laid out a solid foundation the development of future MR-compatible SPECT systems for both pre-clinical and clinical imaging applications.

Ammonia Process by Pressure Swing Adsorption

The overall objective of the project is to design, develop and demonstrate a technically feasible and commercially viable system to produce ammonia along with recovery of the products by adsorption separation methods and significantly decrease the energy requirement in ammonia production. This is achieved through a significantly more efficient ammonia psa recovery system. The new ammonia recovery system receives the reactor effluents and achieves complete ammonia recovery, (which completely eliminates the energy intensive refrigeration and condensation system currently used in ammonia production). It also recovers the unused reactants and recycles them back to the reactor, free of potential reactor contaminants, and without the need for re-compression and re-heat of recycle stream thereby further saving more energy. The result is a significantly lower energy consumption, along with capital cost savings.

Biospecimen Reporting for Improved Study Quality

Human biospecimens are subjected to collection, processing, and storage that can significantly alter their molecular composition and consistency. These biospecimen preanalytical factors, in turn, influence experimental outcomes and the ability to reproduce scientific results. Currently, the extent and type of information specific to the biospecimen preanalytical conditions reported in scientific publications and regulatory submissions varies widely. To improve the quality of research that uses human tissues, it is crucial that information on the handling of biospecimens be reported in a thorough, accurate, and standardized manner. The Biospecimen Reporting for Improved Study Quality (BRISQ) recommendations outlined herein are intended to apply to any study in which human biospecimens are used. The purpose of reporting these details is to supply others, from researchers to regulators, with more consistent and standardized information to better evaluate, interpret, compare, and reproduce the experimental results. The BRISQ guidelines are proposed as an important and timely resource tool to strengthen communication and publications on biospecimen-related research and to help reassure patient contributors and the advocacy community that their contributions are valued and respected.

High-Intensity Proton Accelerator (open access)

High-Intensity Proton Accelerator

Analysis is presented for an eight-cavity proton cyclotron accelerator that could have advantages as compared with other accelerators because of its potentially high acceleration gradient. The high gradient is possible since protons orbit in a sequence of TE111 rotating mode cavities of equally diminishing frequencies with path lengths during acceleration that greatly exceed the cavity lengths. As the cavities operate at sequential harmonics of a basic repetition frequency, phase synchronism can be maintained over a relatively wide injection phase window without undue beam emittance growth. It is shown that use of radial vanes can allow cavity designs with significantly smaller radii, as compared with simple cylindrical cavities. Preliminary beam transport studies show that acceptable extraction and focusing of a proton beam after cyclic motion in this accelerator should be possible. Progress is also reported on design and tests of a four-cavity electron counterpart accelerator for experiments to study effects on beam quality arising from variations injection phase window width. This device is powered by four 500-MW pulsed amplifiers at 1500, 1800, 2100, and 2400 MHz that provide phase synchronous outputs, since they are driven from a with harmonics derived from a phase-locked 300 MHz source.
