Human events reference for ATHEANA (HERA) database description and preliminary user`s manual

The Technique for Human Error Analysis (ATHEANA) is a newly developed human reliability analysis (HRA) methodology that aims to facilitate better representation and integration of human performance into probabilistic risk assessment (PRA) modeling and quantification by analyzing risk-significant operating experience in the context of existing behavioral science models. The fundamental premise of ATHEANA is that error-forcing contexts (EFCs), which refer to combinations of equipment/material conditions and performance shaping factors (PSFs), set up or create the conditions under which unsafe actions (UAs) can occur. Because ATHEANA relies heavily on the analysis of operational events that have already occurred as a mechanism for generating creative thinking about possible EFCs, a database, called the Human Events Reference for ATHEANA (HERA), has been developed to support the methodology. This report documents the initial development efforts for HERA.

Evaluation of the intrinsic and extrinsic fracture behavior of iron aluminides

Iron aluminides have excellent corrosion resistance in high-temperature oxidizing-sulfidizing environments; however, there are problems at room and medium temperatures with hydrogen embrittlement as related to exposure to moisture. In this research, a coordinated computational modeling/experimental study of mechanisms related to environmental-assisted fracture behavior of selected iron aluminides has been undertaken. The modeling and the experimental work connect at the level of coordinated understanding of the mechanisms for hydrogen penetration and for loss of strength and susceptibility to fracture. The focus of the modeling component has been on the challenging question of accurately predicting the iron vacancy formation energy in Fe{sub 3}Al and the subsequent tendency, if present, for vacancy clustering. The authors have successfully performed, on an ab initio basis, the first calculation of the vacancy formation energy in Fe{sub 3}Al. These calculations include lattice relaxation effects which are quite large for one of the two types of iron sites. This has significant implications for vacancy clustering effects with consequences for hydrogen diffusion. Indeed, the ab-initio-based estimate of the divacancy binding energy indicates a likely tendency toward such clustering for iron vacancies on the sites with large lattice relaxation. The Fiscal Year (FY) 1999 DynCorp Annual Work Plan (AWP) relates DOE-RL work breakdown structure (WBS) to Cost Accounts and to Organizational Structure. Each Cost Account includes a workscope narrative and justification performance and service standards, goals, and deliverables. Basis of estimates are included within each Cost Account to demonstrate the relationship of budget to defined workscope. The FY 1999 AWP reflects the planning assumptions and initiatives that are included in the PHMC Strategic Plan for Infrastructure Optimization which was established in FY 1998. Development of the FY 1999 AWP was in accordance with a sequential series of events and efforts described in the Infrastructure Annual Work Planning and Budget Cycle which was developed and established in conjunction with the Strategic Plan. The Strategic Plan covers a rolling five year span of time and is updated at the start of each fiscal year as the beginning of the annual work planning and budget cycle for the following fiscal year. Accordingly the planning for the FY 1999 AWP began in January 1998. Also included in the annual work planning and budget cycle, and the basis for the budget in this AWP, is the development of a requirements-based budget.

This document represents Fluor Daniel Hanford`s and DynCorp`s Tri-Cities Strategic Plan for Fiscal Years 1998--2002, the road map that will guide them into the next century and their sixth year of providing safe and cost effective infrastructure services and support to the Department of Energy (DOE) and the Hanford Site. The Plan responds directly to the issues raised in the FDH/DOE Critical Self Assessment specifically: (1) a strategy in place to give DOE the management (systems) and physical infrastructure for the future; (2) dealing with the barriers that exist to making change; and (3) a plan to right-size the infrastructure and services, and reduce the cost of providing services. The Plan incorporates initiatives from several studies conducted in Fiscal Year 1997 to include: the Systems Functional Analysis, 200 Area Water Commercial Practices Plan, $ million Originated Cost Budget Achievement Plan, the 1OO Area Vacate Plan, the Railroad Shutdown Plan, as well as recommendations from the recently completed Review of Hanford Electrical Utility. These and other initiatives identified over the next five years will result in significant improvements in efficiency, allowing a greater portion of the infrastructure budget to be applied to Site cleanup. This document provides an evaluation and recommendations for radiological workplace air monitoring and response to ventilation failure for the Critical Mass Laboratory, 209-E Building. The Critical Mass Laboratory, part of the 209-E Building, was designed to provide a heavily shielded room where plutonium and uranium liquid solutions could be brought into various critical configurations under readily controlled and monitored conditions. The facility is contained within a one-story L-shaped concrete block and reinforced concrete building. One wing houses offices, a control room, shops, and a common area while the other wing includes an equipment room, the change room, work areas, and the two-story Critical Assembly Room (CAR). Three of the rooms contain radiologically contaminated equipment and materials.

Tank waste remediation system vadose zone program plan

The objective of the vadose zone characterization under this program is to develop a better conceptual geohydrologic model of identified tank farms which will be characterized so that threats to human health and the environment from past leaks and spills, intentional liquid discharges, potential future leaks during retrieval, and from residual contaminants that may remain in tank farms at closure can be explicitly addressed in decision processes. This model will include geologic, hydrologic, and hydrochemical parameters as defined by the requirements of each of the TWRS programs identified here. The intent of this TWRS Vadose Zone Program Plan is to provide justification and an implementation plan for the following activities: Develop a sufficient understanding of subsurface conditions and transport processes to support decisions on management, cleanup, and containment of past leaks, spills, and intentional liquid discharges; Develop a sufficient understanding of transport processes to support decisions on controlling potential retrieval leaks; Develop a sufficient understanding of transport processes to support decisions on tank farm closure, including allowable residual waste that may remain at closure; and Provide new information on geotechnical properties in the 200 Area to supplement data used for design and performance assessment for immobilized low-activity waste disposal facilities.

DNA sequencing with capillary electrophoresis and single cell analysis with mass spectrometry

Since the first demonstration of the laser in the 1960`s, lasers have found numerous applications in analytical chemistry. In this work, two different applications are described, namely, DNA sequencing with capillary gel electrophoresis and single cell analysis with mass spectrometry. Two projects are described in which high-speed DNA separations with capillary gel electrophoresis were demonstrated. In the third project, flow cytometry and mass spectrometry were coupled via a laser vaporization/ionization interface and individual mammalian cells were analyzed. First, DNA Sanger fragments were separated by capillary gel electrophoresis. A separation speed of 20 basepairs per minute was demonstrated with a mixed poly(ethylene oxide) (PEO) sieving solution. In addition, a new capillary wall treatment protocol was developed in which bare (or uncoated) capillaries can be used in DNA sequencing. Second, a temperature programming scheme was used to separate DNA Sanger fragments. Third, flow cytometry and mass spectrometry were coupled with a laser vaporization/ionization interface.

Calculational note for the radiological effects of a UO{sub 3} release from the T-hopper storage pad

UO{sub 3} powder is stored at the T-hopper storage area associated with the 2714-U building in the 200 west area. The T-hopper containers and 13 drums containing this material are used to store the powder on pads immediately north of the building. An interim safety basis document (WHC,1996) was issued in 1996. In this document the isotope{sup 99}Tc was not included in the source term used to calculate the radiological consequences of a postulated release of the powder. This calculational note is intended to remedy that deficiency. Input Data and Assumptions indicate that there is a total of 669,045 kg of uranium at the facility that is at risk for release to the atmosphere. Since uranium comprises 83.217 wt% of UO{sub 3}, this represents a little over 804 tonnes of powder. The postulated accident scenario is that 0.001 of the material at risk is released to the atmosphere.

304L stainless steel resistance to cesium chloride

B and W Hanford Company have two Oak Ridge National Laboratory (ORNL) Type 4 canisters filled with cesium chloride (CsCl) originally produced at WESF (Waste Encapsulation and Storage Facility). These canisters are constructed of 304L stainless steel per drawing ORNL 970-294. Instead of removing the CsCl from the Type 4 canisters and repacking into an Inner Capsule, it is intended (for ALARA, schedule and cost purposes) that the Type 4 canisters be decontaminated (scrubbed) and placed [whole] inside a Type ``W`` overpack. The overpack is constructed from 316L stainless steel. Several tests have been run by Pacific Northwest National Laboratory (PNNL) over the. years documenting the corrosion compatibility of 316L SS with CsCl (Bryan 1989 and Fullam 1972). However, no information for 304L SS compatibility is readily available. This document estimates the corrosion resistance of 304L stainless steel in a WESF CsCl environment as it compares with that of 316L stainless steel.

Longitudinal instability studies at the SURF II storage ring at NIST.

Measurements of the longitudinal instability observed in the storage ring at the Synchrotrons Ultraviolet Radiation Facility (SURF II) at the National Institute of Standards and Technology (NET) were performed to understand the mechanism driving the instability. The instability, studied in depth by Ralcowsky and others, manifests itself in broad resonance features in the horizontal and vertical motion spectrum of the synchrotrons light from DC to a few kHz. Also observed are multiple synchrotrons harmonics that modulate the revolution harmonics; these are characteristic of longitudinal phase oscillations. These spectral features of the motion are found to be correlated with the periodic lengthening and shortening of the bunch length on time scales from {approximately}0.1 ms to 20 ms, depending on machine and radio-frequency (rf) system parameters. In this report, the growth rate of the instability is determined from measurements using an rf pickup electrode. The measured growth rates are compared to computed growth rates from an analytical model. Recommendations are made regarding options to control or mitigate the instability. In light of upgrade plans for SURF III, a few comments are presented about the beam lifetime.
