This report describes the problem of fragmented health care in the United States. The issues the report pinpoints are that it is more costly and of lower quality. The report addresses these issues by summarizing the integrated care delivery model in Medicare. Moreover, the report outlines the two pieces of this program as Accountable Care Organizations (ACOs) and the Medicare Shared Savings Program. Finally, the report discusses the mechanics of these programs to relieve the two issues identified.

Evaluation of facility- and commercial-scale solar energy projects on the Agua Caliente Band of Cahuilla Indians Reservation in Palm Springs, CA. The Agua Caliente Band of Cahuilla Indians (ACBCI) conducted a feasibility and pre-development study of potential solar projects on its lands in southern California. As described below, this study as a logical and necessary next step for ACBCI. Support for solar project development in California, provided through the statewide California Solar Initiative (CSI), its Renewable Portfolio Standard and Feed-in Tariff Program, and recently announced Reverse Auction Mechanism, provide unprecedented support and incentives that can be utilized by customers of California's investor-owned utilities. Department of Energy (DOE) Tribal Energy Program funding allowed ACBCI to complete its next logical step to implement its Strategic Energy Plan, consistent with its energy and sustainability goals.

A well developed infrastructure exists for the reuse and recycling of automotive parts and materials. At the end of a vehicle's useful life many parts are removed and sold for reuse and fluids are recovered for recycling or proper disposal. What remains is shredded, along with other metal bearing scrap such as home appliances, demolition debris and process equipment, and the metals are separated out and recycled. The remainder of the vehicle materials is call shredder residue which ends up in the landfill. As energy and natural resources becomes more treasured, increased effort has been afforded to find ways to reduce energy consumption and minimize the use of our limited resources. Many of the materials found in shredder residue could be recovered and help offset the use of energy and material consumption. For example, the energy content of the plastics and rubbers currently landfilled with the shredder residue is equivalent to 16 million barrels of oil per year. However, in the United States, the recovered materials, primarily polymers, cannot be recycled due to current regulatory barriers which preclude the re-introduction into commerce of certain materials because of residual contamination with substances of concern (SOCs) such as polychlorinated biphenyls (PCBs). The increasing on-chip parallelism has some substantial implications for HPC applications. Currently, hybrid programming models (typically MPI+OpenMP) are employed for mapping software to the hardware in order to leverage the hardware?s architectural features. In this paper, we present an approach that automatically introduces thread level parallelism into Chombo, a parallel adaptive mesh refinement framework for finite difference type PDE solvers. In Chombo, core algorithms are specified in the ChomboFortran, a macro language extension to F77 that is part of the Chombo framework. This domain-specific language forms an already used target language for an automatic migration of the large number of existing algorithms into a hybrid MPI+OpenMP implementation. It also provides access to the auto-tuning methodology that enables tuning certain aspects of an algorithm to hardware characteristics. Performance measurements are presented for a few of the most relevant kernels with respect to a specific application benchmark using this technique as well as benchmark results for the entire application. The kernel benchmarks show that, using auto-tuning, up to a factor of 11 in performance was gained with 4 threads with respect to the serial reference implementation.

A letter report issued by the Government Accountability Office with an abstract that begins "In September 2009, the President announced a revised approach for ballistic missile defense (BMD) in Europe. The European Phased Adaptive Approach (EPAA) is designed to defend against existing and near-term ballistic missile threats and build up defenses over four phases as threats mature and new BMD technologies become available. Although the approach will include capabilities such as radars and landand sea-based BMD assets, the Department of Defense (DOD) has not yet established EPAA life-cycle costs. EPAA is DOD's first implementation of its new, regional approach to BMD. GAO was asked to evaluate DOD's plans for implementing EPAA. GAO reviewed the extent to which: (1) DOD has developed guidance and addressed management of cost and schedule for EPAA, and (2) DOD planning for EPAA is informed by operational performance data. GAO reviewed key legislation, policy and guidance, and initial plans for implementation and asset allocation."

The Salt Disposition Integration (SDI) portfolio of projects provides the infrastructure within existing Liquid Waste facilities to support the startup and long term operation of the Salt Waste Processing Facility (SWPF). Within SDI, the Blend and Feed Project will equip existing waste tanks in the Tank Farms to serve as Blend Tanks where 300,000-800,000 gallons of salt solution will be blended in 1.3 million gallon tanks and qualified for use as feedstock for SWPF. Blending requires the miscible salt solutions from potentially multiple source tanks per batch to be well mixed without disturbing settled sludge solids that may be present in a Blend Tank. Disturbing solids may be problematic both from a feed quality perspective as well as from a process safety perspective where hydrogen release from the sludge is a potential flammability concern. To develop the necessary technical basis for the design and operation of blending equipment, Savannah River National Laboratory (SRNL) completed scaled blending and transfer pump tests and computational fluid dynamics (CFD) modeling. A 94 inch diameter pilot-scale blending tank, including tank internals such as the blending pump, transfer pump, removable cooling coils, and center column, were used in this research. A letter report issued by the Government Accountability Office with an abstract that begins "Cyclone Nargis hit Burma's impoverished Irrawaddy Delta on May 2, 2008, leaving nearly 140,000 people dead or missing and severely affecting about 2.4 million others, according to the UN. The Burmese military government initially blocked most access to the affected region; however, amid international pressure, it slowly began allowing international aid workers entry into the region. Since 1997, the United States has imposed sanctions to prohibit, among other things, the exportation of financial services to Burma and transactions with Burmese officials. In response to a congressional mandate, GAO (1) described the assistance UN and U.S. agencies have provided in response to Cyclone Nargis, (2) assessed USAID actions to help ensure funds are used as intended and do not benefit sanctioned entities, and (3) described the challenges responders experienced and the lessons learned. GAO reviewed financial and program documents; interviewed U.S., UN, and nongovernmental organization (NGO) officials; and traveled to Thailand and Burma."

GAO was asked to examine: (1) the number of foster care children with incarcerated parents, (2) strategies used by child welfare and corrections agencies in selected states that may support contact or reunification, and (3) how the Department of Health and Human Services (HHS) and the Department of Justice (DOJ) have helped these agencies support affected children and families. GAO analyzed national data, reviewed federal policies, interviewed state child welfare and corrections officials in 10 selected states that contain almost half of the nation's prison and foster care populations, and visited local child welfare agencies and prisons."

This CRS Report, updated as warranted, discusses the security problem of China's role in weapons proliferation and issues related to the U.S. policy response since the mid- 1990s. China has taken some steps to mollify U.S. and other foreign concerns about its role in weapons proliferation. Nonetheless, supplies from China have aggravated trends that result in ambiguous technical aid, more indigenous capabilities, longer-range missiles, and secondary (retransferred) proliferation. According to unclassified intelligence reports submitted as required to Congress, China has been a "key supplier" of technology, particularly PRC entities providing nuclear and missile-related technology to Pakistan and missile-related technology to Iran.

A letter report issued by the Government Accountability Office with an abstract that begins "The interstate commercial motor carrier industry moves thousands of truckloads of goods every day, and any disruption in one truckload's delivery schedule can have a ripple effect on others. Some waiting time at shipping and receiving facilities--commonly referred to as detention time--is to be expected in this complex environment. However, excessive detention time could impact the ability of drivers to perform within federal hours of service safety regulations, which limit duty hours and are enforced by the Federal Motor Carrier Safety Administration (FMCSA). This report discusses: (1) How regularly do truck drivers experience detention time and what factors contribute to detention time? (2) How does detention time affect the commercial freight vehicle industry? (3) What federal actions, if any, could be taken to address detention time issues? GAO analyzed federal and industry studies and interviewed a nongeneralizable sample of truck drivers, as well as other industry stakeholders and FMCSA officials."

The Administration's FY2012 budget would zero-out certain national activities related to Community Service Block Grant (CSBG), including Rural Community Facilities and Job Opportunities for Low-Income Individuals (JOLI). The budget requests $20 million for Community Economic Development (down from the FY2010 level of $36 million but more than the final FY2011 level of $18 million), and would target these funds toward the multiagency Healthy Food Financing Initiative. The Administration would fund Individual Development Accounts (IDAs, also known as Assets for Independence) at $24 million in FY2012, which is the same level as in FY2010 and FY2011.

The computational fluid dynamics (CFD) and computational structural mechanics (CSM) focus areas at Argonne's Transportation Research and Analysis Computing Center (TRACC) initiated a project to support and compliment the experimental programs at the Turner-Fairbank Highway Research Center (TFHRC) with high performance computing based analysis capabilities in August 2010. The project was established with a new interagency agreement between the Department of Energy and the Department of Transportation to provide collaborative research, development, and benchmarking of advanced three-dimensional computational mechanics analysis methods to the aerodynamics and hydraulics laboratories at TFHRC for a period of five years, beginning in October 2010. The analysis methods employ well-benchmarked and supported commercial computational mechanics software. Computational mechanics encompasses the areas of Computational Fluid Dynamics (CFD), Computational Wind Engineering (CWE), Computational Structural Mechanics (CSM), and Computational Multiphysics Mechanics (CMM) applied in Fluid-Structure Interaction (FSI) problems. Attempts to resolve the Cyprus problem and reunify the island have undergone various levels of negotiation for over 45 years. Talks between Greek and Turkish Cypriot leaders have thus far failed to reach a mutually agreed settlement leaving the country with a solution for unification far from being achieved and raising the specter of a possible permanent separation. This report provides a brief overview of the early history of the negotiations, a more detailed review of the negotiations since 2008, and a description of some of the issues involved in the talks. A side issue involving trade between the European Union and Turkish Cyprus is also addressed.
