Overseas Rightsizing: State Has Improved the Consistency of Its Approach, but Does Not Follow Up on Its Recommendations
A letter report issued by the Government Accountability Office with an abstract that begins "The Department of State (State) has improved the consistency of its rightsizing approach across overseas posts. However, differences between future staffing levels it projects are appropriate to meet mission needs and actual staffing levels still exist due to unanticipated events and other factors. GAO reported in 2006 that States Office of Management Policy, Rightsizing and Innovation (M/PRI) had not been conducting its rightsizing reviews consistently. Some reviews discussed various rightsizing elements, such as outsourcing, while others did not. State has since improved the consistency of its reviews by developing a variety of methodological tools and a standard template which it applies to each post. GAO found that over half of the 144 rightsizing projections analyzed were within 10 percent of actual staffing levels as of December 2011. In contrast, over 40 percent of the posts have staffing level differences of over 10 percent. Unanticipated events and other factors, such as changes in policies, contributed to these differences. Medicaid: Federal Oversight of Payments and Program Integrity Needs Improvement
Testimony issued by the Government Accountability Office with an abstract that begins "Oversight of managed care rate-setting has been inconsistent. In August 2010, GAO reported that the Centers for Medicare & Medicaid Services (CMS) had not ensured that all states were complying with the managed care actuarial soundness requirements that rates be developed in accordance with actuarial principles, appropriate for the population and services, and certified by actuaries. For example, GAO found significant gaps in CMSs oversight of 2 of the 26 states reviewedCMS had not reviewed one states rates in multiple years and had not completed a full review of another states rates since the actuarial soundness requirements became effective. Variation in practices across CMS regional offices contributed to these gaps and other inconsistencies in the agencys oversight of states rate setting. GAOs previous work also found that CMSs efforts to ensure the quality of the data used to set rates were generally limited to requiring assurances from states and health plansefforts that did not provide the agency with enough information to ensure the quality of the data used. Higher Education: Improved Tax Information Could Help Pay for College
Testimony issued by the Government Accountability Office with an abstract that begins "We found that multiple Title IV programs and tax expenditures provided substantial aid to populations across income levels. In 2009, 12.8 million students received Title IV aid, and approximately 18 million tax filers claimed a higher education tax benefit for current expenses. The number of students receiving Title IV aid increased from 10.4 million to 12.8 million, or 23 percent, from 2006 to 2009. The number of tax filers benefiting from an education tax expenditure was larger, and increased from 14.4 million to 18 million, or 25 percent, from 2006 to 2009. Recent increases in both Title IV aid and tax expenditures from 2008 to 2009 may be because of enrollment increases and legislative actions, among other factors. Title IV grants tend to benefit students and families with incomes below the national median (about $52,000 from 2006 to 2010), while loans and work-study serve these students as well as students at family incomes above the median. Chemical, Biological, Radiological, and Nuclear Risk Assessments: DHS Should Establish More Specific Guidance for Their Use
A letter report issued by the Government Accountability Office with an abstract that begins "Since 2004, DHSs use of its CBRN risk assessments to inform its CBRN response plans has varied, from directly influencing information in the plans to not being used at all. DHS guidance states that response planning and resource decisions should be informed by risk information. GAOs analysis showed that DHS used its CBRN risk assessments to directly inform 2 of 12 CBRN response plans GAO identified because planners considered the risk assessments to be more accurate than earlier DHS planning assumptions. For another 7 of the 12 plans, DHS officials said that the assessments indirectly informed the plans by providing background information prior to plan development. However, GAO could not independently verify this because DHS officials could not document how the risk assessments influenced the information contained in the plans. GAOs analysis found general consistency between the risk assessments and the plans. For the remaining 3 plans, DHS officials did not use the risk assessments to inform the plans; for 2 of the 3 plans DHS officials told GAO they were not aware of the assessments. Agency Operations: Agencies Must Continue to Comply with Fiscal Laws Despite the Possibility of Sequestration
Testimony issued by the Government Accountability Office with an abstract that begins "The Budget Control Act of 2011, amending the Balanced Budget and Emergency Deficit Control Act of 1985, establishes limits on discretionary spending for fiscal years 2012 through 2021. In addition, the Act specifies additional limits on discretionary spending and automatic reductions in direct spending because legislation was not enacted that would reduce projected deficits by at least $1.2 trillion by the end of fiscal year 2021. Among other things, the Budget Control Act requires the Office of Management and Budget (OMB) to calculate, and the President to order, a sequestration of discretionary and direct spending on January 2, 2013, to achieve reductions for that fiscal year."

Grants Management: Improving the Timeliness of Grant Closeouts by Federal Agencies and Other Grants Management Challenges
An agency summary issued by the Government Accountability Office with an abstract that begins "Closeout is an important final point of grants accountability. It helps to ensure that grantees have met all financial and reporting requirements. It also allows federal agencies to identify and redirect unused funds to other projects and priorities as authorized or to return unspent balances to the Department of the Treasury (Treasury). At the end of fiscal year 2011, GAO identified more than $794 million in funding remaining in expired grant accounts (accounts that were more than 3 months past the grant end date and had no activity for 9 months or more) in the Payment Management System (PMS). GAO found that undisbursed balances remained in some grant accounts several years past their expiration date: $110.9 million in undisbursed funding remained unspent more than 5 years past the grant end date, including $9.5 million that remained unspent for 10 years or more. Nevertheless, the more than $794 million in undisbursed balances remaining in PMS represents an improvement in closing out expired grant accounts with undisbursed balances in PMS compared to the approximately $1 billion GAO found in 2008. Defense Health: Coordinating Authority Needed for Psychological Health and Traumatic Brain Injury Activities [Reissued on January 27, 2012]
A letter report issued by the Government Accountability Office with an abstract that begins "From fiscal year 2007 through fiscal year 2010, DOD activities for the treatment and research of PH and TBI received more than $2.7 billion. In fiscal year 2007, funding for these activities totaled $900 million; in fiscal year 2008, it was $573.8 million; in fiscal year 2009, $395 million; and in fiscal year 2010, $838.6 million. GAO found, however, that the reports DOD provided to Congress on these activities did not include expenditures, as required by law, and that the obligations data they contained were unreliable. Governmentwide policies call for agencies to have effective internal controls to assure accurate reporting of obligations and expenditures. However, the Office of the Assistant Secretary of Defense for Health Affairs has not developed quality control mechanisms to help ensure that data on PH and TBI activities are complete and accurate. Further, although DOD listed patient care among reported costs, it did not specify what those costs included, making it difficult for decisionmakers and Congress to fully understand the costs."

Supply Chain Security: CBP Needs to Conduct Regular Assessments of Its Cargo Targeting System
A letter report issued by the Government Accountability Office with an abstract that begins "U.S. Customs and Border Protection (CBP), within the Department of Homeland Security (DHS), employs a risk-based approach that uses the Automated Targeting System (ATS) and other tools to identify (target) maritime cargo shipments for further examination. ATS is a web-based enforcement and decision support system that includes a set of rules to assess the risk level for each arriving cargo shipment. This set of rules is referred to as the maritime national security weight set (weight set) because each rule in the set has a specific weighted value assigned to it. CBP classifies the risk scores from the weight set as low, medium, or high risk. CBP policy states that a shipment's risk score is to determine, in part, actions taken by CBP officers (targeters) at the ports. Specifically, targeters are generally required to review shipment data for all medium-risk and high-risk shipments and hold high-risk shipments for examination. The risk score, however, is not the sole factor that determines whether a targeter reviews the data for a shipment or whether CBP examines a shipment. Critical Infrastructure Protection: An Implementation Strategy Could Advance DHS's Coordination of Resilience Efforts across Ports and Other Infrastructure
A letter report issued by the Government Accountability Office with an abstract that begins "The Department of Homeland Security (DHS) is developing a resilience policy, but an implementation strategy is a key next step that could help strengthen DHS resilience efforts. DHS defines resilience as the ability to resist, absorb, recover from, or adapt to adversity, and some high-level documents currently promote resilience as a key national goal. Specifically, two key White House documents emphasize resilience on a national level--the 2011 Presidential Policy Directive 8 and the 2012 National Strategy for Global Supply Chain Security. Since 2009, DHS has emphasized the concept of resilience and is currently in the process of developing a resilience policy, the initial steps of which have included creating two internal entities--the Resilience Integration Team and the Office of Resilience Policy (ORP). According to ORP officials, they saw a need to establish a policy that provides component agencies with a single, consistent, departmentwide understanding of resilience that clarifies and consolidates resilience concepts from high-level guiding documents, and helps components understand how their activities address DHS's proposed resilience objectives. Information Technology: DHS Needs to Further Define and Implement Its New Governance Process
A letter report issued by the Government Accountability Office with an abstract that begins "The Department of Homeland Security (DHS) has defined a vision for its new information technology (IT) governance process, which includes a tiered oversight structure that defines distinct roles and responsibilities throughout the department. The new governance framework and the associated policies and procedures are generally consistent with recent Office of Management and Budget (OMB) guidance and with best practices for managing projects and portfolios identified in GAOs IT Investment Management framework, with two practices partially addressed and seven others fully addressed. For example, consistent with OMB guidance calling for the Chief Information Officer (CIO) to play a significant role in overseeing programs, DHSs draft procedures require that lower-level boards overseeing IT programs include the DHS CIO, a component CIO, or a designated executive representative from a CIO office.
