Medicaid: Source of Screening Affects Women's Eligibility for Coverage of Breast and Cervical Cancer Treatment in Some States

A letter report issued by the Government Accountability Office with an abstract that begins "Tens of thousands of women die each year from breast or cervical cancer. While screening and early detection through mammograms and Pap tests--followed by treatment--can improve survival, low-income, uninsured women are often not screened. In 1990, Congress authorized the Centers for Disease Control and Prevention (CDC) to fund screening and diagnostic services for such women, which led CDC to establish the National Breast and Cervical Cancer Early Detection Program. The Breast and Cervical Cancer Prevention and Treatment Act of 2000 was also enacted to allow states to extend Medicaid eligibility to women screened under the Early Detection Program and who need breast or cervical cancer treatment. Screened under the program is defined, at a minimum, as screening paid for with CDC funds. GAO examined the Early Detection Program's screening of eligible women, states' implementation of the Treatment Act, Medicaid enrollment and spending under the Treatment Act, and alternatives available to women ineligible for Medicaid under the Treatment Act. End-Stage Renal Disease: CMS Should Monitor Effect of Bundled Payment on Home Dialysis Utilization Rates

A letter report issued by the Government Accountability Office with an abstract that begins "Medicare covers dialysis--a process that removes excess fluids and toxins from the bloodstream--for most individuals with end-stage renal disease (ESRD), a condition of permanent kidney failure. Most patients with ESRD receive dialysis in a facility, while some patients with ESRD are trained to self-perform dialysis in their homes. The Centers for Medicare & Medicaid Services (CMS)--the agency that administers the Medicare program--has taken steps to encourage home dialysis and is in the process of changing the way it pays for dialysis services. Effective 2011, CMS will pay for dialysis services using an expanded bundled payment. The Tax Relief and Health Care Act of 2006 required GAO to report on the costs of home dialysis treatments and training. GAO examined (1) the extent to which the costs of home dialysis differ from the costs of dialysis received in a facility, and (2) CMS's plans to account for home dialysis costs in the expanded bundled payment. GAO obtained information from CMS, the U.S. Renal Data System, ESRD experts, and self-reported cost information from six dialysis providers."

The purpose of the grant was to build a community of scientist and to draw upon their expertise to design and build the world's first dedicated high pressure beamline at a spallation source - the so called Spallation Neutron And Pressure (SNAP) beamline at the Spallation Neutron Source (SNS) at OAk Ridge NAtional LAboratory. . Key to this endeavor was an annual meeting attended by the instrument design team and the executive committee. The discussions at those meeting set an ambitious agenda for beamline design and construction and highlighted key science areas of interest for the community. This report documents in 4 appendices the deliberations at the annual SNAP meetings and the evolution of the beamline optics from concept to construction. The appendices also contain key science opportunities for extreme conditions research.

Bacterial Community Succession During in situ Uranium Bioremediation: Spatial Similarities Along Controlled Flow Paths

Bacterial community succession was investigated in a field-scale subsurface reactor formed by a series of wells that received weekly ethanol additions to re-circulating groundwater. Ethanol additions stimulated denitrification, metal reduction, sulfate reduction, and U(VI) reduction to sparingly soluble U(IV). Clone libraries of SSU rRNA gene sequences from groundwater samples enabled tracking of spatial and temporal changes over a 1.5 y period. Analyses showed that the communities changed in a manner consistent with geochemical variations that occurred along temporal and spatial scales. Canonical correspondence analysis revealed that the levels of nitrate, uranium, sulfide, sulfate, and ethanol strongly correlated with particular bacterial populations. As sulfate and U(VI) levels declined, sequences representative of sulfate-reducers and metal-reducers were detected at high levels. Ultimately, sequences associated with sulfate-reducing populations predominated, and sulfate levels declined as U(VI) remained at low levels. When engineering controls were compared to the population variation via canonical ordination, changes could be related to dissolved oxygen control and ethanol addition. The data also indicated that the indigenous populations responded differently to stimulation for bio-reduction; however, the two bio-stimulated communities became more similar after different transitions in an idiosyncratic manner. This summary of Argonne National Laboratory's Site Environmental Report for calendar year 2007 was written by 20 students at Downers Grove South High School in Downers Grove, Ill. The student authors are classmates in Mr. Howard's Bio II course. Biology II is a research-based class that teaches students the process of research by showing them how the sciences apply to daily life. For the past seven years, Argonne has worked with Biology II students to create a short document summarizing the Site Environmental Report to provide the public with an easy-to-read summary of the annual 300-page technical report on the results of Argonne's on-site environmental monitoring program. The summary is made available online and given to visitors to Argonne, researchers interested in collaborating with Argonne, future employees, and many others. In addition to providing Argonne and the public with an easily understandable short summary of a large technical document, the participating students learn about professional environmental monitoring procedures, achieve a better understanding of the time and effort put forth into summarizing and publishing research, and gain confidence in their own abilities to express themselves in writing. ImSET 3.1: Impact of Sector Energy Technologies Model Description and User's Guide

This 3.1 version of the Impact of Sector Energy Technologies (ImSET) model represents the next generation of the previously-built ImSET model (ImSET 2.0) that was developed in 2005 to estimate the macroeconomic impacts of energy-efficient technology in buildings. In particular, a special-purpose version of the Benchmark National Input-Output (I-O) model was designed specifically to estimate the national employment and income effects of the deployment of Office of Energy Efficiency and Renewable Energy (EERE)–developed energy-saving technologies. In comparison with the previous versions of the model, this version features the use of the U.S. Bureau of Economic Analysis 2002 national input-output table and the central processing code has been moved from the FORTRAN legacy operating environment to a modern C++ code. ImSET is also easier to use than extant macroeconomic simulation models and incorporates information developed by each of the EERE offices as part of the requirements of the Government Performance and Results Act. While it does not include the ability to model certain dynamic features of markets for labor and other factors of production featured in the more complex models, for most purposes these excluded features are not critical. First search for extraterrestrial neutrino-induced cascades with IceCube

We report on the first search for extraterrestrial neutrino-induced cascades in IceCube.The analyzed data were collected in the year 2007 when 22 detector strings were installed and operated. We will discuss the analysis methods used to reconstruct cascades and to suppress backgrounds. Simulated neutrino signal events with a E-2 energy spectrum, which pass the background rejection criteria, are reconstructed with a resolution Delta(log E) ~;; 0.27 in the energy range from ~;; 20 TeV to a few PeV. We present the range of the diffuse flux of extra-terrestrial neutrinos in the cascade channel in IceCube within which we expect to be able to put a limit.

An Approach to Understanding Cohesive Slurry Settling, Mobilization, and Hydrogen Gas Retention in Pulsed Jet Mixed Vessels

The Hanford Waste Treatment and Immobilization Plant (WTP) is being designed and built to pretreat and vitrify a large portion of the waste in Hanford’s 177 underground waste storage tanks. Numerous process vessels will hold waste at various stages in the WTP. Some of these vessels have mixing-system requirements to maintain conditions where the accumulation of hydrogen gas stays below acceptable limits, and the mixing within the vessels is sufficient to release hydrogen gas under normal conditions and during off-normal events. Some of the WTP process streams are slurries of solid particles suspended in Newtonian fluids that behave as non-Newtonian slurries, such as Bingham yield-stress fluids. When these slurries are contained in the process vessels, the particles can settle and become progressively more concentrated toward the bottom of the vessels, depending on the effectiveness of the mixing system. One limiting behavior is a settled layer beneath a particle-free liquid layer. The settled layer, or any region with sufficiently high solids concentration, will exhibit non-Newtonian rheology where it is possible for the settled slurry to behave as a soft solid with a yield stress. In this report, these slurries are described as settling cohesive slurries.

This Technical Review Report (TRR) documents the review, performed by the Lawrence Livermore National Laboratory (LLNL) Staff, at the request of the U.S. Department of Energy (DOE), on the Safety Analysis Report for Packaging, Model 9975, Revision 0, dated January 2008 (S-SARP-G-00003, the SARP). The review includes an evaluation of the SARP, with respect to the requirements specified in 10 CFR 71, and in International Atomic Energy Agency (IAEA) Safety Standards Series No. TS-R-1. The Model 9975-96 Package is a 35-gallon drum package design that has evolved from a family of packages designed by DOE contractors at the Savannah River Site. Earlier package designs, i.e., the Model 9965, the Model 9966, the Model 9967, and the Model 9968 Packagings, were originally designed and certified in the early 1980s. In the 1990s, updated package designs that incorporated design features consistent with the then newer safety requirements were proposed. The updated package designs at the time were the Model 9972, the Model 9973, the Model 9974, and the Model 9975 Packagings, respectively. Novel Large Area High Resolution Neutron Detector for the Spallation Neutron Source

Neutron scattering is a powerful technique that is critically important for materials science and structural biology applications. The knowledge gained from past developments has resulted in far-reaching advances in engineering, pharmaceutical and biotechnology industries, to name a few. New facilities for neutron generation at much higher flux, such as the SNS at Oak Ridge, TN, will greatly enhance the capabilities of neutron scattering, with benefits that extend to many fields and include, for example, development of improved drug therapies and materials that are stronger, longer-lasting, and more impact-resistant. In order to fully realize this enhanced potential, however, higher neutron rates must be met with improved detection capabilities, particularly higher count rate capability in large size detectors, while maintaining practicality. We have developed a neutron detector with the technical and economic advantages to accomplish this goal. This new detector has a large sensitive area, offers 3D spatial resolution, high sensitivity and high count rate capability, and it is economical and practical to produce. The proposed detector technology is based on B-10 thin film conversion of neutrons in long straw-like gas detectors. RIA Superconducting Drift Tube Linac R & D

Cavity and cryomodule development work for a superconducting ion linac has been underway for several years at the National Superconducting Cyclotron Laboratory. The original application of the work was the proposed Rare Isotope Accelerator. At present, the work is being continued for use with the Facility for Rare Isotope Beams (FRIB). The baseline linac for FRIB requires 4 types of superconducting cavities to cover the velocity range needed to accelerate an ion beam to #21; 200 MeV/u: 2 types of quarter-wave resonator (QWR) and 2 types of half-wave resonator (HWR). Superconducting solenoids are used for focussing. Active and passive shielding is required to ensure that the solenoids’ field does not degrade the cavity performance. First prototypes of both QWR types and one HWR type have been fabricated and tested. A prototype solenoid has been procured and tested. A test cryomodule has been fabricated and tested. The test cryomodule contains one QWR, one HWR, one solenoid, and one super-ferric quadrupole. This report covers the design, fabrication, and testing of this cryomodule

Fast spot-based multiscale simulations of granular drainage

We develop a multiscale simulation method for dense granular drainage, based on the recently proposed spot model, where the particle packing flows by local collective displacements in response to diffusing"spots'" of interstitial free volume. By comparing with discrete-element method (DEM) simulations of 55,000 spheres in a rectangular silo, we show that the spot simulation is able to approximately capture many features of drainage, such as packing statistics, particle mixing, and flow profiles. The spot simulation runs two to three orders of magnitude faster than DEM, making it an appropriate method for real-time control or optimization. We demonstrateextensions for modeling particle heaping and avalanching at the free surface, and for simulating the boundary layers of slower flow near walls. We show that the spot simulations are robust and flexible, by demonstrating that they can be used in both event-driven and fixed timestep approaches, and showing that the elastic relaxation step used in the model can be applied much less frequently and still create good results.

Final Report - Dynamic Path Scheduling through Extensions to Generalized Multiprotocol Label Switching (GMPLS)

The major accomplishments of the project are the successful software implementation of the Phase I scheduling algorithms for GMPLS Label Switched Paths (LSPs) and the extension of the IETF Path Computation Element (PCE) Protocol to support scheduling extensions. In performing this work, we have demonstrated the theoretical work of Phase I, analyzed key issues, and made relevant extensions. Regarding the software implementation, we developed a proof of concept prototype as part of our Algorithm Evaluation System (AES). This implementation uses the Linux operating system to provide software portability and will be the foundation for our commercial software. To demonstrate proof of concept, we have implemented LSP scheduling algorithms to support two of the key GMPLS switching technologies (Lambda and Packet) and support both Fixed Path (FP) and Switched Path (SP) routing. We chose Lambda and Packet because we felt it was essential to include both circuit and packet switching technologies as well as to address all-optical switching in the study. As conceptualized in Phase I, the FP algorithms use a traditional approach where the LSP uses the same physical path for the entire service duration while the innovative SP algorithms allow the physical path to vary during the service duration. …
