Ambulance Services: Changes Needed to Improve Medicare Payment Policies and Coverage Decisions

Testimony issued by the General Accounting Office with an abstract that begins "The Balanced Budget Act of 1997 required Medicare to change its payment system for ambulance services. In response, the Health Care Financing Administration (HCFA), now called the Centers for Medicare and Medicaid Services (CMS), proposed a fee schedule to standardize payments across provider types on the basis of national rates for particular services. Under the act, the fee schedule was to have applied to ambulance services furnished on or after January 1, 2000. HCFA published a proposed rule in September 2000 and has received public comment, but it has not yet issued a final rule. This testimony discusses the unique concerns of rural ambulance providers and the likely effects of the proposed fee schedule on these providers. Many rural ambulance providers face a set of unique challenges in implementing an appropriate payment policy. Rural providers--particularly those serving large geographic areas with low population density--tend to have high per-trip costs compared with urban and suburban providers. The proposed Medicare fee schedule does not sufficiently distinguish the providers serving beneficiaries in the most isolated rural areas and may not appropriately account for the higher costs of low-volume providers."

Preparedness activities include (1) increasing federal, state, and local response capabilities; (2) developing response teams; (3) increasing the availability of medical treatments; (4) participating in and sponsoring exercises; (5) aiding victims; and (6) providing support at special events, such as presidential inaugurations and Olympic games. To coordinate their efforts to combat terrorism, federal agencies are developing interagency response plans, participating in various interagency work groups, and entering into formal agreements with other agencies to share resources and capabilities. However, coordination of federal terrorism research, preparedness, and response programs is fragmented, raising concerns about the ability of states and localities to respond to a bioterrorist attack. These concerns include insufficient state and local planning and a lack of hospital participation in training on terrorism and emergency response planning. GAO reviewed six draft plans developed by the four military services, the Defense Logistics Agency, and the U.S. Transportation Command to overcome this problem. The six draft plans have significantly improved on DOD's original plan for tracking inventory. They are, however, inconsistent in content and do not reflect a coordinated, DOD-wide approach to correcting the lack of visibility over inventory being shipped. These draft plans also lack detailed performance measures for monitoring progress and determining the effectiveness of the actions after they are implemented. They do not address how DOD will ensure appropriate coordination among the military services and DOD commands in the implementation of the corrective actions or how DOD will monitor future progress. Lastly, the Office of the Secretary of Defense has not established a deadline for when the plans should be finalized."

This includes developing appropriate information on the number and location of employees and their key competences and skills, as well as data on the profile of the workforce, and performance goals and measures for human capital approaches. NASA's legislative proposal for streamlined demonstration and alternative personnel system authority would provide a mechanism for converting a successful demonstration project, upon approval by the Office of Personnel Management (OPM), to a permanent alternative personnel system, without the need for special legislation and without any limitation on the number of employees that would be covered by the project as is under current authority. Human capital systems should have appropriate transparency and adequate safeguards in order to help ensure that it is efficient, effective, credible, and equitable."

GAO considered a number of leading and emerging biometric technologies that could potentially be used for securing the nations borders. The seven leading biometric technologies include facial recognition, fingerprint recognition, hand geometry, iris recognition, retina recognition, signature recognition, and speaker recognition. To evaluate the effectiveness of biometrics in border control, it is important to recognize that the use of biometric technology would be but one component of the decision to support systems that determine who is allowed to enter the United States and who is not. To combat the drug trade, the U.S. government developed a counternarcotics strategy consisting of five pillars--alternative livelihoods, elimination and eradication, interdiction, law enforcement and justice, and public information. The Emergency Supplemental Appropriations Act of 2005 directed GAO to examine the use of all fiscal year 2005 funds administered by the U.S. Agency for International Development (USAID) and Department of State (State) for Afghan counternarcotics programs. To comply with this mandate, we examined progress under each counternarcotics pillar, challenges faced, and efforts to ensure that funds were used for intended purposes. To address these objectives, GAO reviewed pertinent USAID and State documents and met with cognizant U.S. and international officials in Washington, D.C., and Afghanistan. Testimony issued by the Government Accountability Office with an abstract that begins "In August 2006, the Transportation Security Administration (TSA) substantially modified its passenger screening policies based on the alleged transatlantic bomb plot uncovered by British authorities. With the aim of closing security gaps revealed by the alleged plot, the revised policies severely restricted the amount of liquids, gels, and aerosols TSA allowed passengers to bring through the checkpoint. At the Committee's request, GAO tested whether security gaps exist in the passenger screening process. To perform this work, GAO attempted to (1) obtain the instructions and components needed to create devices that a terrorist might use to cause severe damage to an airplane and threaten the safety of passengers and (2) test whether GAO investigators could pass through airport security checkpoints undetected with all the components needed to create the devices. GAO conducted covert testing at a nonrepresentative selection of 19 airports across the country. After concluding its tests, GAO provided TSA with two timely briefings to help it take corrective action. CMS relies extensively on contractors to help it carry out its basic mission. Congress appropriated to CMS $1 billion for start-up administrative costs to implement provisions of MMA. Because CMS had discretion on how to use the appropriation, Congress asked GAO to determine (1) how CMS used the $1 billion MMA appropriation, (2) whether CMS's contracting practices and related internal controls were adequate to avoid waste and to prevent or detect improper payments, and (3) whether payments to contractors were properly supported as a valid use of government funds. These capabilities are primarily found at DOE's two key emergency response facilities--the Remote Sensing Laboratories at Nellis Air Force Base, Nevada, and Andrews Air Force Base, Maryland. These capabilities took on increased significance after the attacks of September 11, 2001, because of heightened concern that terrorists may try to detonate a nuclear or radiological device in a major U.S. city. VA oversees the Ocwen contract, which terminates in 2008, through onsite property inspections and other means. How DOD is managing this investment has been a matter of concern for some time. Since 1990, GAO has designated DOD's weapon system acquisitions as a high-risk area for fraud, waste, abuse, and mismanagement. DOD has experienced cost overruns, missed deadlines, performance shortfalls, and persistent management problems. In light of the serious budget pressures facing the nation, such problems are especially troubling. GAO has issued hundreds of reports addressing broad-based issues, such as best practices, as well as reports focusing on individual acquisitions. These reports have included many recommendations. To this end, the Departments of Defense and Homeland Security provide ships and aircraft to disrupt the flow of illicit drugs, primarily cocaine, shipped from South America through the Caribbean Sea and eastern Pacific Ocean--an area known as the transit zone. The Office of National Drug Control Policy (ONDCP) oversees the U.S. anti-drug strategy. The Joint Interagency Task Force-South (JIATF-South) directs most transit zone operations. A letter report issued by the Government Accountability Office with an abstract that begins "The Department of State is in the early stages of a proposed multibillion dollar program to build secure new embassies and consulates around the world. Under the proposed Capital Security Cost-Sharing Program, all agencies with staff assigned to overseas diplomatic missions would share in construction costs. This report describes (1) the rationale for and development of the program, (2) agency concerns about the program, and (3) the influence of the program on agencies' overseas staff levels."
