Statement given by Samuel David Rubenstein on December 12, 1963 in Dallas, Texas. The statement is presented in a question and answer format. Rubenstein stated that he was born in Chicago in 1912. He is of Jewish ethnicity and a brother of Jack Ruby.

Report to Chief J. E. Curry by J. H. Sawyer concerning a statement taken from Deputy Chief George L. Lumpkin. Sawyer states that Lumpkin's statement described his activities on the morning of the shooting of Lee Harvey Oswald, and stated that he did not know Jack Ruby nor did he see him in the City Hall basement.

Report to Chief J. E. Curry by P. L. Bentley regarding the polygraph examination given to N. J. Daniels. Bentley states that the examination was given to determine if Daniels was telling the truth in his statement. He states that during the pre-test Daniels claimed that he did not think the person that entered the basement was Jack Ruby, and that he was also very confused about what he said in his statement. Six of the nine questions asked indicated that his answers were false, one answer was indicated as true, and two of the questions were not answered.

Statement by J. H. Sawyer concerning Officer's assignments, the murder of Lee Harvey Oswald, and familiarity with Jack Ruby. Sawyer describes assisting in the transfer of Lee Harvey Oswald and witnessing Jack Ruby being handcuffed.

Criminal intelligence report regarding Declan Ford. Ford stated that Lee Harvey Oswald and his wife came to a small party in his home. Oswald was withdrawn and difficult to talk to. Oswald's wife, Marina, stayed at his home while they were having domestic problems.

Report to Chief J. E. Curry by E. R. Gaddy, concerning the prior arrest of Jack Ruby on June 1, 1954. Gaddy states that Ruby was arrested for carrying a concealed weapon, and that he does not remember anything about the case.

Report to Chief J. E. Curry by W. R. Westbrook, concerning Officer Ray Hawkins and a membership card at the Carousel Club. Westbrook states that Hawkins stated that he had been to the Carousel Club two or three times and that Ruby had said he would give him a permanent pass.
