Drug Shortages: Threat to Public Health Persists, Despite Actions to Help Maintain Product Availability

Although reports of new drug shortages declined in 2012, the total number of shortages active during a given year—including both new shortages reported and ongoing shortages that began in a prior year—has increased since 2007. Provider association representatives reported that drug shortages may force providers to ration care or face difficulties finding alternative drugs."

Presidential Helicopter Acquisition: Update on Program's Progress toward Development Start

Correspondence issued by the Government Accountability Office with an abstract that begins "GAO found that the Navy continued to make progress in the past year toward (1) establishing a knowledge-based program that generally aligned with acquisition best practices and (2) meeting corresponding statutory certification requirements for entering the Department of Defense (DOD) acquisition process in the engineering and manufacturing development phase. The Navy has met or expects to meet requirements relating to assessment of technology readiness, making appropriate trade-offs to achieve affordability and getting approval of requirements, and development of an independent cost estimate. DOD has, however, waived a requirement for competitive prototyping and the Navy plans to defer a system level preliminary design review (PDR) until after the start of development. GAO reviewed DOD’s waiver of competitive prototyping for the VXX program last year and found that it addresses one of the two bases provided in Weapon Systems Acquisition Reform Act of 2009 for such waivers; that is, that the cost to provide competitive prototypes exceeded the expected benefits. Inspectors General: Oversight of Small Federal Agencies and the Role of the Inspectors General

Testimony issued by the Government Accountability Office with an abstract that begins "The Inspector General Act of 1978, as amended (IG Act), originally established inspectors general (IG) appointed by the President and confirmed by the Senate in 12 major departments and agencies of the government to conduct and supervise independent audits and investigations; recommend policies to promote economy, efficiency, and effectiveness; and prevent and detect fraud and abuse in their departments' and agencies' programs and operations. Based in part on GAO's findings that the internal audit offices of small federal agencies lacked independence and provided inadequate coverage of important programs, the Congress passed the IG Act Amendments of 1988 to establish IGs in designated federal entities (DFE), which are generally smaller agencies established in various statutes as commissions, boards, authorities, corporations, endowments, foundations, institutions, agencies, and administrations identified by the act. The DFE IGs are appointed by their respective entity heads with duties and responsibilities similar to those of IGs appointed by the President. The Congress used a budget threshold of $100 million to help determine which DFEs should have IGs. However, additional DFEs below this threshold were also included for specific reasons."

Status of Efforts to Initiate an Amphibious Combat Vehicle Program

Correspondence issued by the Government Accountability Office with an abstract that begins "The Amphibious Combat Vehicle (ACV) effort has not yet reached Milestone A—the decision point in DOD’s acquisition process that generally authorizes assessment of potential technologies for an eventual acquisition program. At this point, Marine Corps officials are weighing the cost and technological feasibility of their required capabilities. In November 2011, the Marines Corps began an Analysis of Alternatives (AOA)—a key first step in the acquisition process intended to assess alternative weapon system solutions for addressing a validated need—to identify an affordable alternative to the canceled Expeditionary Fighting Vehicle. Following completion of that study in mid-2012, the Marine Corps Commandant directed the program to perform a second study to assess the feasibility and affordability of a variant capable of higher water speed. This second analysis has been completed, but no formal decisions have been made regarding whether to commence an acquisition program or what path it will take, if initiated. Program officials suggest that development of a high water speed technology may prove unaffordable at this time."

Oral History Interview with Gus Cargile, March 10, 2014

The National Museum of the Pacific War presents an oral interview with Gus Cargile. Cargile entered the Navy as an aviation cadet in May 1942. After graduation in August 1943 he took a Marine commission. Cargile trained in Georgia, Kansas, Texas and Florida before he became an instructor. He taught bombing, shooting and night flying in Florida and Texas. He flew the SNV in basic and a North American SNJ in advanced, and went on to fly the F4U Corsair, F6F, F7F, SBD. He made landings on the USS Guadalcanal (CVE-60). He was discharged as captain in September 1946 and went into the Reserves.

Oral History Interview with Ralph Sparks, April 10, 2014

The National Museum of the Pacific War presents an interview with Ralph Sparks. Sparks was drafted into the Army in 1943 and for part of basic training, they went to North Dakota to harvest a bumper wheat crop. He then trained in field artillery before heading for France in July, 1944. Sparks served as a forward observer around Metz. He also recalls moving into Belgium during the Battle of the Bulge. Sparks recalls a story where his unit raided a wine cellar. When the war ended, Sparks made his way back to the US and was discharged in December, 1945.

The National Museum of the Pacific War presents an oral interview with Walter Neumann. Neumann joined the Navy in April, 1944 and trained at Great Lakes. He attended radio school before reporting aboard YMS-231. They were just getting underway in the Pacific when the war ended. They shaped course for Japan to work minesweeping duties during the occupation. He was discharged I n1947 but stayed in the active reserves.

Oral History Interview with Milford Lee, October 10, 2014

The National Museum of the Pacific War presents an oral interview with Milford Lee. After high school, Lee performed in a traveling big band. Once the war started, he became a draftsman at Boeing and took the aviation cadet exam. In January 1943 he began flight training, earning his wings that fall. He served as a P-38 fighter escort and group leader over Ploiesti. He details the protocol for conducting large raids, from briefing to debriefing, describing intense antiaircraft fire over the target. In Southern France, his plane was experimentally modified to carry 3,000 pounds' worth of bombs. In the fall of 1944, Lee was recruited for an Army big band that traveled to remote bases and emergency landing strips. While performing in the Balkans, he was captivated by a Partisan commander, later revealed to be Tito. After 56 missions, Lee was rotated to the States and in November 1945 was discharged. He went on to earn a doctorate in nuclear physics and joined a small research team that revolutionized fiber-optic communication.

Oral History Interview with Seserino Calderon, November 10, 2014

The National Museum of the Pacific War presents an oral interview with Seserino Calderon. Calderon was drafted into the Army in September, 1942 and trained in California. He was trained and assigned to the 161st Infantry Regiment, 25th Infantry Division and sent to Guadalcanal in late 1942. He served as a 60mm mortar man. After a brief invasion of Munda to take an airfield, Calderon enjoyed some rest at New Zealand before the invasion of the Philippines. His unit landed at Lingayen Gulf on Luzon. Calderon returned to the US and was discharged in November, 1945.

Oral History Interview with Ken Coonrod, June 10, 2014

The National Museum of the Pacific War presents an interview with Ken Coonrod. Coonrod joined the Navy in the fall of 1944. He served as an Electrician’s Mate Third Class in the Asiatic-Pacific Theater. In late 1944, Coonrod was assigned to the Philippines. In mid-1945, he traveled to Okinawa, in preparation for the invasion of Japan. He was assigned to an LCVP just before the war ended. Coorod was transferred to Nagoya, Japan, and served with occupation forces aboard USS Littlehales (AGS-7). He returned to the US and received his discharge in 1946.

The National Museum of the Pacific War presents an oral interview with Philip Schneiderman. Schneiderman joined the Coast Guard in 1944 when he was 15 years old because he was able to get the doctor to waive him as a result of procuring a set of tires for the doctor. He was shipped to New Guinea to catch his ship and was in the hospital with a tropical disease when his ship came in. He snuck out of the Navy hospital to join his ship. He shares several anecdotes about life aboard an LST in the Pacific during WWII. He recalls going to Luzon and Okinawa as well as other points around New Guinea landing supplies and equipment. He was discharged in May 1946.

Oral History Interview with Everett Dyer, December 10, 2014

The National Museum of the Pacific War presents an oral interview with Everett Dyer. Dyer was born in Bristol, Vermont on 23 March 1918 into a family of six children. After receiving his diploma from high school he attended the University of Houston and graduated in 1941. Upon being drafted in 1941, he was sent to Camp Roberts, California for basic training. After completing infantry training he went to Fort Lewis, Washington to join the 41st Infantry Division Headquarters as an aide to General Horace Fuller. In April the division went aboard HMS Queen Elizabeth for a nineteen day voyage to Sydney, Australia. Upon his arrival he was assigned to the headquarters operations staff (G-3) responsible for preparing casualty reports. During January 1943 the division participated in the battle of Buna, Gona on New Guinea and Dyer comments on the high casualties suffered. He recalls making an amphibious landing at Hollandia by lifeboat in April 1944 and being strafed by Japanese planes while on the beach. He also witnessed attacks by Japanese kamikazes on ships in the bay. Dyer was a member of the retinue that greeted General McArthur as he came ashore. While on Biak, he developed typhus, resulting …

Oral History Interview with James Womack, December 10, 2014

The National Museum of the Pacific War presents an oral interview with James Womack. Womack was born in Decatur, Texas in October 1924 and graduated from high school in 1941. Upon enlisting in the Army Air Corps in 1942, he went to Sheppard Field, Texas for basic training. This was followed by a two month flight engineer course. In 1943 he was assigned to the 70th Ferrying Command, 20th Ferrying Squadron. Womack ferried B-26 bombers, B-17 bombers, and C-46 transport planes to various destinations. After the Surrender of Japan he entered the pilot training program but with the temporary termination of the program he transferred to the 8th Air Force Intelligence section. In 1949 he was commissioned and remained in the Air Force until retiring in1963.
