A statement of record issued by the Government Accountability Office with an abstract that begins "In 2012, GAO reported that 80 federal programs in eight different agencies fund a variety of transportation services for transportation-disadvantaged populations, which include older Americans. Within the Department of Transportation (DOT), the Federal Transit Administration (FTA) is a key source of federal transportation funding for older Americans. For example, some FTA programs provide formula funding to states to serve transit-dependent populations with special needs. States typically distribute these funds to local nonprofit human service agencies to buy vehicles to transport older adults and people with disabilities, and the funds may support transportation to access a range of activities, such as grocery shopping. While some federal funding programs are transportation focused, transportation was not the primary mission for the vast majority of the 80 programs GAO identified in 2012. For example, the Department of Health and Human Services' Medicaid program reimburses states that provide Medicaid beneficiaries with bus passes, among other transportation options, to access eligible medical services. This growth is due predominantly to increases in prison populations, which are the primary cost driver of BOP's budget. The other two program, project and activity (PPA) elements in the S&E account are associated with the care and custody of federal offenders in contract facilities and maintenance and administration. BOP's Buildings and Facilities (B&F) account, which makes up on average less than 3 percent of its budget, pays for costs associated with site planning; acquisition; and construction of new facilities and costs of remodeling and renovating existing facilities, and related costs. In fiscal year 2014, the budget justification reflected a total of $6.9 billion; of which over 95 percent ($6.8 billion) was for BOP's S&E account."

Medicaid: Use of Claims Data for Analysis of Provider Payment Rates

Correspondence issued by the Government Accountability Office with an abstract that begins "Among the 9 states and 35 services examined, GAO found that all states varied Medicaid payments for at least some services. However, the states differed in the number of services for which they varied payments, in the factors that accounted for variation, and in the magnitude of the variation. Many of the states varied payment rates by at least one of the factors GAO was able to explore in detail: provider type, service setting, and/or patient age. Some states also varied their payment rates for other reasons, such as by geographic region or by physician specialty. GAO also found that most of the median Medicaid payment rates calculated from claims data generally confirmed payment rates published in studies of fee schedules conducted by the American Academy of Pediatrics and the Urban Institute; however, some of the published fee schedule rates were rarely used in practice. Collectively, GAO's findings demonstrate that Medicaid fee-for-service claims data can be a useful source of information for analyzing provider payments. Testimony issued by the Government Accountability Office with an abstract that begins "The schedule and cost for the Department of Homeland Security's (DHS) border enforcement system modernization program known as TECS Mod that is managed by Customs and Border Protection's (CBP) continue to change; while the part managed in parallel by Immigration and Customs Enforcement (ICE) is undergoing major revisions to its scope, schedule, and cost after discovering that its initial solution is not technically viable. CBP's $724 million program intends to modernize the functionality, data, and aging infrastructure of legacy TECS and move it to DHS's data centers by 2016. To date, CBP has deployed functionality to improve its secondary inspection processes to air and sea ports of entry and, more recently, to land ports of entry in 2013. However, CBP is in the process of revising its schedule baseline for the second time in under a year. Further, CBP has not developed its master schedule sufficiently to reliably manage work activities or monitor program progress. These factors raise questions about the certainty of CBP's remaining schedule commitments. A letter report issued by the Government Accountability Office with an abstract that begins "The Economic Development Administration (EDA) implemented a new procedure in fiscal year 2012 that requires its regional offices to complete a standard template to document the results of committee meetings in which proposed projects are discussed and potentially recommended for funding. However, GAO found that for the Public Works and Economic Development (Public Works) program and the Economic Adjustment Assistance (EAA) program—EDA's two largest grant assistance programs—EDA regional offices had not completed the template consistently. GAO estimated that only 46 percent of all projects recommended for funding in fiscal year 2012 under these programs were documented using the complete template. EDA has a history of inconsistent documentation: for example, in 2000 the Department of Commerce's Inspector General reported inconsistencies in how EDA's regional offices documented the project review process. Standards for internal control in the federal government require all transactions and significant events to be clearly documented and available for examination. Testimony issued by the Government Accountability Office with an abstract that begins "Federal programs GAO reviewed that offer financial support to entrepreneurs, such as grants and loans, are fragmented and overlap based on the type of support they are authorized to offer and the type of entrepreneur they are authorized to serve. The Departments of Commerce (Commerce), Housing and Urban Development (HUD), and Agriculture (USDA); the Small Business Administration (SBA); and the Office of Management and Budget (OMB) have taken steps to collaborate more in administering these programs in response to a recommendation in GAO's August 2012 report. For example, OMB has established a Cross-Agency Priority goal for entrepreneurship and small business and an associated interagency working group. However, the four agencies have not implemented a number of good collaborative practices GAO has identified, such as establishing compatible policies and procedures to better support rural businesses. The Government Performance and Results Act Modernization Act of 2010 (GPRAMA) crosscutting framework requires that agencies collaborate in order to address issues such as economic development that transcend more than one agency, and GPRAMA directs agencies to describe how they are working with each other to achieve their program goals. For example, agencies reported that sequestration reduced assistance for education, housing, and nutrition, as well as health and science research and development grants. Agencies also reported delaying investments such as information technology and facilities projects. Some federal services also experienced backlogs and delays as a result of personnel actions including limiting hiring and furloughing employees. However, many of the effects of sequestration could not be quantified or will not be known until future years, if at all, for a number of reasons including the timing of when funds are disbursed (such as grant cycles that start late in the fiscal year), challenges in isolating the effects from other factors, and the lack of currently available performance data for some programs. Moreover, congressional and agency actions mitigated some potential effects by shifting funds to higher priorities while deferring or reducing funding for lower priorities."

A letter report issued by the Government Accountability Office with an abstract that begins "Based on the number of providers awarded incentive payments, participation in the Department of Health and Human Services' (HHS) Medicare and Medicaid Electronic Health Record (EHR) programs increased substantially from their first year in 2011 to 2012. For hospitals, participation increased from 45 percent of those eligible for 2011 to 64 percent of those eligible for 2012. For professionals, such as physicians, participation increased from 21 percent of those eligible for 2011 to 48 percent of those eligible for 2012. While increases occurred, a substantial percentage of providers that participated in 2011 did not participate in 2012. Officials who oversee the programs at the Centers for Medicare & Medicaid Services (CMS) noted there could be several reasons for this, such as challenges in demonstrating meaningful use, and are monitoring the issue. Various program changes make future participation difficult to estimate. For example, increased stringency of requirements for the programs' second phase beginning in 2014—Stage 2—may slow participation, while the introduction of penalties in 2015 for some providers may motivate participation."

Nuclear Nonproliferation: Stronger Planning and Evaluation Needed for Radiological Security Zone Pilot Project

A letter report issued by the Government Accountability Office with an abstract that begins "Two U.S. agencies—the Nuclear Regulatory Commission (NRC) and the National Nuclear Security Administration (NNSA)—have several ongoing efforts, both in the United States and internationally, to secure radiological sources that could be used to make a terrorist weapon. These efforts include strengthening regulatory requirements, upgrading security, and recovering unwanted or abandoned radiological sources. Domestically, NRC has worked to strengthen regulatory requirements to provide reasonable assurance that U.S. licensees protect high-risk radiological sources. In addition, at the request of licensees, NNSA provides voluntary security upgrades designed to raise security to a level above existing regulatory requirements, consistent with best practices that NNSA has identified. These upgrades include, for example, motion sensors and alarms that are tracked by staff at remote monitoring centers. Internationally, NRC has spent about $12 million since 2002 to help countries establish and strengthen their regulatory frameworks. From fiscal year 2008 through March 2013, NNSA has spent about $304 million to help remove or secure radiological sources in foreign locations. A letter report issued by the Government Accountability Office with an abstract that begins "All the nuclear regulatory bodies in the 16 selected countries in GAO's review—13 of which currently operate nuclear power reactors and 3 of which are developing or considering developing civilian nuclear power programs—have taken steps to strengthen nuclear safety in response to the Fukushima Daiichi accident in Japan. Japan in particular has fundamentally restructured its nuclear regulatory framework, and 3 other countries—China, Sweden, and Vietnam—are providing additional resources to their nuclear regulatory bodies. Countries are taking steps to improve safety with a focus on considering previously unimagined accident scenarios. Specifically, regulatory bodies in several countries (e.g., Belgium, Canada, Russia, and the United States) are now planning for accident scenarios that could involve multiple reactors at a single power plant. In addition, new requirements for emergency equipment, such as backup electric generators, in case of the loss of off-site power, as occurred at the Fukushima Daiichi nuclear power plant, are an area of focus among the regulatory bodies in GAO's review."

A letter report issued by the Government Accountability Office with an abstract that begins "The Small Business Administration (SBA) has established lending standards to protect against default and has measured program performance, but lacks guidance on determining the number of jobs supported by 504 program-funded projects. SBA's guidance for certified development companies (CDC)—nonprofits that provide funding to small businesses to promote local economic development in their communities and are certified by SBA—includes credit underwriting standards for determining ability to repay. SBA also has established performance indicators—such as the number of small businesses assisted and jobs supported—for the 504 loan program. However, SBA does not describe how CDCs should calculate jobs created and retained by 504 projects, a key program requirement. Federal internal control standards require control activities that help participants report information accurately. Without specific guidance, SBA cannot ensure that CDCs are calculating this information consistently or accurately. GAO's analysis of SBA data showed that 504 loans approved in fiscal years 2006 through 2008 had the highest 18-month default rates, which correlated with trends in the private sector for commercial real estate loans."

VA Health Care: Actions Needed to Improve Administration and Oversight of Veterans' Millennium Act Emergency Care Benefit

A letter report issued by the Government Accountability Office with an abstract that begins "The Veterans Millennium Health Care and Benefits Act (Millennium Act) authorizes the Department of Veterans Affairs (VA) to cover emergency care for conditions not related to veterans' service-connected disabilities when veterans who have no other health plan coverage receive care at non-VA providers. However, GAO identified a number of instances where VA staff who processed claims did not comply with applicable requirements of the Millennium Act, its implementing regulations, or VA policies when they denied the claims. Specifically, at the four VA facilities included in this review, GAO found 66 instances of noncompliance among the 128 denied claims reviewed, which led some claims to be inappropriately denied. VA facilities subsequently reconsidered and paid 25 of these claims. GAO also found that VA facilities may not be notifying veterans as required that their Millennium Act claims have been denied. Eighty-three claims out of 128 that GAO reviewed lacked documentation that the veteran was notified of the denial or of his or her appeal rights.
