Handwritten affidavit by Bonnie Ray Williams, an employee at the Texas School Book Depository, who worked on the sixth floor on November 22nd. At lunchtime Bonnie went to the fifth floor with a man named Hank where they saw the President pass and heard two shots, which sounded as though they came from just above them. Bonnie saw Lee Harvey Oswald when he arrived at the building at 8 AM and recognized him when he was brought into the Homicide Bureau.

Affidavit In Any Fact by Sam Guinyard who witnessed a person running with a pistol in hand. Guinyard states that he saw the man near Patton Street and 10th Street, and then saw a policeman lying in the street. He later identified the man in a police lineup.

Photocopy of an affidavit given by Mrs. Virginia Davis. Davis states that her sister-in-law, Jeanette Davis, and herself were lying down in their apartments when they heard shots. A woman yelled: "He's dead, he's dead, he's shot!" They saw an officer on the ground outside. Jeanette Davis called the police. A boy was seen running across their lawn, where they later found shells. Davis identified him as the #2 man in a lineup.

Supplementary Offense Report by L. D. Montgomery concerning threats against Wilma Tice, a witness regarding Jack Ruby at Parkland Hospital on November 22, 1963, the day that the President was killed. Tice reported threatening phone calls advising her to keep quiet about seeing Ruby and mysterious activity at her home.

Voluntary statement by Robert E. Edwards as a witness in Dealey Plaza. Edwards describes seeing a man on the fifth floor of the Texas School Book Depository building hiding behind boxes. He states that the gunshots he heard seemed to come from the building.

Voluntary statement made by Marvin Faye Chism at the Dallas Police Department on November 22nd, 1963. Chism states that she heard a shot as President Kennedy fell to his left. Mrs. Kennedy laid him down on the seat and there was another shot.

Affidavit In Any Fact by Helen Louise Markham, a witness to the murder of Officer J. D. Tippit. Markham describes seeing a young white man shoot the officer after he got out of his vehicle.

Voluntary statement by John Stevens Rutter Lawrence, regarding seeing a man in a suit carrying a rifle. Lawrence states that he left the Texaco Building where he worked with Phil Hathaway and other coworkers to see the President in the parade. He describes seeing a large man, estimated to be around 6'5" and 250 pounds or more, wearing a business suit and carrying a rifle.

Handwritten affidavits by William H. Shelly. Shelly states that he was watching the president in the parade when he heard three shots. He encountered a woman who also works in the Texas School Book Depository who was crying and said that the president had been killed. He went back to the building, called his wife to tell her the news, and watched the elevator to make certain that no one exited. Shelly had supervised Lee Harvey Oswald closely since he started working at the Texas School Book Depository. Oswald kept to himself and took walks at noon.

Mercer states that she saw a truck on the side of the entrance road to the overpass. There was a man pulling out a gun case from the back of the truck who proceeded to walk away.

Affidavit In Any Fact by Billy Nolan Lovelady, an employee of the Texas School Book Depository. Lovelady states that he was with Danny Arce, Jack Dougherty, Bill Shelley, and Charles Givens to watch the President in the parade when they heard gunshots. Afterwards he brought police officers up to the 6th floor of the building.
