It is a standard WWI issued form with blank spaces available for personalization. This form was completed by a Lt. Colonel (his signature is unreadable) at Camp Travis, Texas on April 11th 1918. The remaining filled spaces detail the Private Jett Falls, Co. E 359th Infantry had been promoted to Corporal of that same division on March 27th 1918.
