On July 19, 2006, GAO testified on the results of its purchase card work. This report summarizes the testimony and provides recommendations. Department of Homeland Security (DHS) cardholders made thousands of transactions related to hurricane relief operations. GAO analyzed transactions between June and November of 2005 to determine if (1) DHS's control environment and management of purchase card usage were effective; (2) DHS's key internal control activities operated effectively and provided reasonable assurance that purchase cards were used appropriately; and (3) potentially fraudulent, improper, and abusive purchase card activity existed at DHS."

To accomplish these goals, the Army focused on upgrading and modernizing existing equipment, rapidly fielding new equipment, incorporating new technologies as they became available, and restructuring aviation warfighting units. Initially, fielding the developmental Comanche helicopter was a key focus of modernization, but when the Comanche program was terminated in 2004, an investment strategy was presented to Congress that would redistribute $14.6 billion of planned Comanche funding through fiscal year 2011 to enhance a broad range of Army aviation modernization efforts. Furthermore, the Army is currently re-evaluating the plans that were established in 2004 by conducting several assessments, tracking progress, and assessing future capability requirements, and intends to develop an updated Aviation Modernization Plan in 2010. A letter report issued by the General Accounting Office with an abstract that begins "Since the 1920s, the Army's Ceremonial Rifle Program has lent obsolete or condemned rifles to veterans' organizations for funerals and other ceremonies. The M-1 is the only rifle currently authorized for this purpose. The National Defense Authorization Act for Fiscal Year 2000 authorized the Secretary of the Army to conditionally loan or donate M-1 rifles and required that GAO review and report on the Secretary's exercise of authority under the amended law. The Army issued an interim change to its regulation governing the Ceremonial Rifle Program that partially addresses the Secretary's authority under the law. This interim change addresses (1) the number of rifles that may be provided and (2) the security, safety, and accountability over provided rifles. From the time the law was amended in October 1999 through March 2001, the Army reported providing 232 organizations with 2,054 rifles and had 937 open requests for 8,395 rifles. The Army estimates that it has provided more than 300,000 rifles under the Ceremonial Rifle Program since the program's inception."

Kidney transplant recipients must take costly medications to avoid transplant failure. Unless a transplant recipient is eligible for Medicare other than on the basis of ESRD, Medicare coverage, including that for medications, ends 36 months posttransplant. Pediatric transplant recipients, including those who were under 18 when transplanted but are now adults (transitional recipients), may be more likely than their adult counterparts to lose access to medications once Medicare coverage ends because they may lack access to other health insurance coverage. GAO was asked to examine (1) the percentage of transplant failures and subsequent outcomes--retransplant, dialysis, or death--among pediatric, transitional, and adult kidney transplant recipients and (2) how the cost to Medicare for a beneficiary with a functioning transplant compares with the cost for a beneficiary with a transplant failure. If travel to IDP- or CZTR-designated areas begins during one month and concludes during another (known as cross-month travel), members could receive 2 full months of benefits. In this report, we refer to airlines that are not in ATOS as non-legacy airlines. Two other processes are used to oversee 99 non-legacy passenger airlines, which represent a fast-growing segment of the commercial aviation passenger industry and carried about 200 million passengers in 2004. The National Work Program Guidelines (NPG) establishes a set of inspection activities for non-legacy airlines. The Surveillance and Evaluation Program (SEP) uses principles of system safety to identify additional risk-based inspections for those airlines. GAO's objective was to assess the processes used by FAA to ensure the safety of non-legacy passenger airlines. GAO reviewed the strengths of FAA's inspection oversight for non-legacy passenger airlines and the issues that hinder its effectiveness."

Prescription Drugs: FDA Guidance and Regulations Related to Data on Elderly Persons in Clinical Drug Trials

Correspondence issued by the Government Accountability Office with an abstract that begins "Elderly persons use drugs at a higher rate than younger persons, in part because elderly persons experience higher rates of certain diseases--such as cancer, Parkinson's disease, and heart disorders. Elderly persons--those age 65 and older--are also more likely than younger adults to experience complications when taking some prescription drugs. For example, because of their decreased liver and kidney functions, elderly persons often lack the ability to eliminate drugs from their bodies as efficiently as younger adults, making elderly persons more likely to experience side effects associated with drugs. As a result, the Food and Drug Administration (FDA) has noted that it is important that drugs be studied for use by elderly persons during the clinical drug trials--that is, those drug studies conducted by drug sponsors before they submit an application to have a drug approved for marketing. FDA is responsible for oversight of clinical drug trials and deciding whether to approve new drugs for marketing in the United States. This responsibility includes determining if drugs are safe and effective for the people expected to use them, including elderly persons. Global Health: U.S. Agencies Support Programs to Build Overseas Capacity for Infectious Disease Surveillance

A letter report issued by the Government Accountability Office with an abstract that begins "The rapid spread of severe acute respiratory syndrome (SARS) in 2003 shows that disease outbreaks pose a threat beyond the borders of the country where they originate. Over the past decade, the United States has initiated a broad effort to ensure that countries can detect any disease outbreaks that may constitute a public health emergency of international concern. Three U.S. agencies--the Centers for Disease Control and Prevention (CDC), the U.S. Agency for International Development (USAID), and the Department of Defense (DOD)--support programs aimed at building this broader capacity to detect a variety of infectious diseases. This report describes (1) the obligations, goals, and activities of these programs and (2) the U.S. agencies' monitoring of the programs' progress. To address these objectives, GAO reviewed budgets and other funding documents, examined strategic plans and program monitoring and progress reports, and interviewed U.S. agency officials. GAO did not review capacity-building efforts in programs that focus on specific diseases, namely polio, tuberculosis, malaria, avian influenza, or HIV/AIDS. GAO is not making any recommendations.
