Medicare Physician Services: Use of Services Increasing Nationwide and Relatively Few Beneficiaries Report Major Access Problems

A letter report issued by the Government Accountability Office with an abstract that begins "Congress, policy analysts, and groups representing physicians have periodically raised concerns that Medicare's efforts to control spending on physician services by limiting annual updates to physician fees could have an adverse impact on beneficiaries' access to physician services. These concerns were heightened in 2002 when Medicare's formula for setting physician fees required a 5.4 percent reduction in fees to help moderate rapid spending increases. From 2003 to 2006, fees have not grown as rapidly as the estimated cost to physicians of providing services, and concerns about access have remained. The Medicare Prescription Drug, Improvement, and Modernization Act of 2003 requires GAO to study access to physician services by beneficiaries in the traditional fee-for-service (FFS) program. This report focuses on (1) trends and patterns in beneficiaries' perceptions of the availability of physician services from 2000 through 2004, (2) trends in beneficiaries' utilization of physician services from 2000 through 2005, and (3) indicators of physician supply and willingness to serve Medicare beneficiaries from 2000 through 2005. Exterior complex scaling provides a practical path forfirst-principles studies of atomic and molecular ionizationproblemssince it avoids explicit enforcement of asymptotic boundary conditionsfor 3-body Coulomb breakup. We have used the method of exterior complexscaling, implemented with both the discrete variable representation andB-splines, to obtain the first-order wave function for molecular hydrogencorresponding to a single photon having been absorbed by a correlatedinitial state. These wave functions are used to construct convergedtriple differential cross sections for double photoionization of alignedH2 molecules.

Development of a performance-based industrial energy efficiency indicator for cement manufacturing plants.

Organizations that implement strategic energy management programs have the potential to achieve sustained energy savings if the programs are carried out properly. A key opportunity for achieving energy savings that plant managers can take is to determine an appropriate level of energy performance by comparing the plant performance with that of similar plants in the same industry. Manufacturing plants can set energy efficiency targets by using performance-based indicators. The U.S. Environmental Protection Agency (EPA), through its ENERGY STAR{reg_sign} program, has been developing plant energy performance indicators (EPIs) to encourage a variety of U.S. industries to use energy more efficiently. This report describes work with the cement manufacturing industry to provide a plant-level indicator of energy efficiency for assembly plants that produce a variety of products, including Portland cement and other specialty cement products, in the United States. Consideration is given to the role that performance-based indicators play in motivating change; the steps needed to develop indicators, including interacting with an industry to secure adequate data for an indicator; and the actual application and use of an indicator when complete. X-ray survival characteristics and genetic analysis for nineSaccharomyces deletion mutants that affect radiation sensitivity

We examine ionizing radiation (IR) sensitivity and epistasisrelationships of several Saccharomyces mutants affectingpost-translational modifications of histones H2B and H3. Mutantsbre1delta, lge1delta, and rtf1delta, defective in histone H2B lysine 123ubiquitination, show IR sensitivity equivalent to that of the dot1deltamutant that we reported on earlier, consistent with published findingsthat Dot1p requires H2B K123 ubiquitination to fully methylate histone H3K79. This implicates progressive K79 methylation rather thanmono-methylation in IR resistance. The set2delta mutant, defective in H3K36 methylation, shows mild IR sensitivity whereas mutants that abolishH3 K4 methylation resemble wild type. The dot1delta, bre1delta, andlge1delta mutants show epistasis for IR sensitivity. The paf1deltamutant, also reportedly defective in H2B K123 ubiquitination, confers nosensitivity. The rad6delta, rad51null, rad50delta, and rad9deltamutations are epistatic to bre1? and dot1delta, but rad18delta andrad5delta show additivity with bre1delta, dot1delta, and each other. Thebre1delta rad18delta double mutant resembles rad6delta in sensitivity;thus the role of Rad6p in ubiquitinating H2B accounts for its extrasensitivity compared to rad18delta. We conclude that IR resistanceconferred by BRE1 and DOT1 is mediated through homologous recombinationalrepair, not postreplication repair, and confirm findings of a G1checkpoint role for the RAD6/BRE1/DOT1 pathway.

Improving Energy Efficiency Via Optimized Charge Motion and Slurry Flow in Plant Scale Sag Mills

A research team from the University of Utah is working to make inroads into saving energy in these SAG mills. In 2003, Industries of the Future Program of the Department of Energy tasked the University of Utah team to build a partnership between the University and the mining industry for the specific purpose of reducing energy consumption in SAG mills. A partnership was formed with Cortez Gold Mines, Outokumpu Technology, Kennecott Utah Copper Corporation, and Process Engineering Resources Inc. At Cortez Gold Operations the shell and pulp lifters of the semiautogenous grinding mill was redesigned. The redesigned shell lifter has been in operation for over three years and the redesigned pulp lifter has been in operation for over nine months now. This report summarizes the dramatic reductions in energy consumption. Even though the energy reductions are very large, it is safe to say that a 20% minimum reduction would be achieved in any future installations of this technology.

Double Photoionization of Aligned Molecular Hydrogen

We present converged, completely ab initio calculations ofthe triple differential cross sections for double photoionization ofaligned H2 molecules for a photon energy of 75.0 eV. The method ofexterior complex scaling, implemented with both the discrete variablerepresentation and B-splines, is used to solve the Schroedinger equationfor a correlated continuum wave function corresponding to a single photonhaving been absorbed by a correlated initial state. Results for a fixedinternuclear distance are compared with recent experiments and show thatintegration over experimental angular and energy resolutions is necessaryto produce good qualitative agreement, but does not eliminate somediscrepancies. Limitations of current experimental resolution are shownto sometimes obscure interesting details of the crosssection.

The Americans with Disabilities Act and Emergency Preparedness and Response

The Americans with Disabilities Act provides broad nondiscrimination protection for individuals with disabilities in employment, public services, and public accommodation and services operated by private entities. Although the ADA does not include provisions specifically discussing its application to disasters such as Hurricane Katrina, its nondiscrimination provisions are applicable to emergency preparedness and responses to disasters. For example, this would mean that emergency planning should include individuals with disabilities and that emergency shelters should be accessible to individuals with disabilities. In order to further the goals of the ADA, President Bush issues an Executive Order on Jluy 22nd, 2004relating to emergency preparedness for individuals with disabilities and establishing the Interagency Coordinating Council on Emergency Preparedness and Individuals with Disabilities. The Department of Homeland Security issued a Nationwide Plan Review Phase 2 Report, which includes a discussion of people with disabilities and emergency planning and readiness. The National Council on Disability has also issued a recommendation on emergency preparation and disaster relief relating to individuals with disabilities. Legislation has also been introduced to address the needs of individuals with disabilities in emergency planning and relief.

The WEI6K, a 6-kW 7-m Small Wind Turbine: Final Technical Report

This project was selected by the U.S. Department of Energy under a DOE solicitation “Low Wind Speed Technology for Small Turbine Development.” The objective of this project has been to design a new small wind turbine with improved cost, reliability and performance in grid-connected residential and small business applications, in order to achieve the overall DOE goal of cost effectiveness in Class 3 wind resources that can now be achieved in Class 5 resources. The scope of work for this project has been to complete the preliminary design of an improved small wind turbine, including preliminary loads and strength analyses; analysis and design of all major components; systems integration and structural dynamic analysis; estimation of life-cycle cost of energy; and design documentation and review. The project did not entail hardware fabrication or testing. The WEI6K Turbine resulting from this project is an upwind horizontal-axis wind turbine rated at 6 kW. It features a 3-blade 7-m diameter rotor. The generator is a direct-drive permanent magnet synchronous machine generating 3-phase power at 240 VAC. The Optical Lightpipe as a High-Bandwidth Fusion Diagnostic

A recent series of experiments at the University of Rochester Laboratory for Laser Energetics OMEGA facility studied the feasibility of using radiation-to-light converters and high bandwidth optical signal transmission to remote recording devices as an alternate nuclear diagnostic method. A prototype system included a radiation-to-light converter, a multiple-section light pipe consisting of stainless steel tubes with polished interiors and turning mirrors, and a streak camera or photomultiplier/digitizer combination for signal recording. Several different radiation-to-light converters (scintillators, glasses, plastics, and pressurized CO{sub 2}) performed well and produced predictable optical emissions. The lightpipe transmitted high-bandwidth optical signals to the recording stations. Data were recorded with the streak camera, the photomultiplier/digitizer, and with both recorders simultaneously.

Time-resolved Soft X-Ray Imaging (SXRI) diagnostic for use at the NIF and OMEGA lasers (version 2)

The soft x-ray imager (SXRI) built for the first experiments at the National Ignition Facility (NIF) has four soft x-ray channels and one hard x-ray channel. The SXRI is a snout that mounts to a four strip gated imager. This produces four soft x-ray images per strip, which can be separated in time by {approx}60psec. Each soft x-ray channel consists of a mirror plus a filter. The diagnostic was used to study x-ray burnthrough of hot hohlraum targets at the NIF and OMEGA lasers. The SXRI snout design and issues involved in selecting the desired soft x-ray channels are discussed.

TECHNICAL ASSESSMENT OF BULK VITRIFICATION PROCESS & PRODUCT FOR TANK WASTE TREATMENT AT THE DEPARTMENT OF ENERGY HANFORD SITE

At the U.S. Department of Energy (DOE) Hanford Site, the Waste Treatment Plant (WTP) is being constructed to immobilize both high-level waste (IUW) for disposal in a national repository and low-activity waste (LAW) for onsite, near-surface disposal. The schedule-controlling step for the WTP Project is vitrification of the large volume of LAW, current capacity of the WTP (as planned) would require 50 years to treat the Hanford tank waste, if the entire LAW volume were to be processed through the WTP. To reduce the time and cost for treatment of Hanford Tank Waste, and as required by the Tank Waste Remediation System Environmental Impact Statement Record of Decision and the Hanford Federal Facility Consent Agreement (Tn-Party Agreement), DOE plans to supplement the LAW treatment capacity of the WTP. Since 2002, DOE, in cooperation with the Environmental Protection Agency and State of Washington Department of Ecology has been evaluating technologies that could provide safe and effective supplemental treatment of LAW. Current efforts at Hanford are intended to provide additional information to aid a joint agency decision on which technology will be used to supplement the WTP. Fringe-free, Background-free, Collinear Third Harmonic Generation FROG Measurements for Multiphoton Microscopy

Collinear pulse measurement tools useful at the full numerical aperture (NA) of multiphoton microscope objectives are a necessity for a quantitative characterization of the femtosecond pulses focused by these systems. In this letter, we demonstrate a simple new technique, for characterizing the pulse at the focus in a multiphoton microscope. This technique, a background-free, fringe-free, form of frequency-resolved optical gating, uses the third harmonic signal generated from a glass coverslip. Here it is used to characterize 100 fs pulses (typical values for a multiphoton microscope) at the focus of a 0.65 NA objective.

In 2003, Industries of the Future Program of the Department of Energy tasked the University of Utah team to build a partnership between the University and the mining industry for the specific purpose of reducing energy consumption in SAG mills. A partnership was formed with Cortez Gold Mines, Kennecott Utah Copper Corporation, Process Engineering Resources Inc. and Outokumpu Technology. In the current project, Cortez Gold Mines played a key role in facilitating the 26-ft SAG mill at Cortez as a test mill for this study. According to plant personnel, there were a number of unscheduled shut downs to repair broken liners and the mill throughput fluctuated depending on ore type. The University team had two softwares, Millsoft and FlowMod to tackle the problem. Millsoft is capable of simulating the motion of charge in the mill. FlowMod calculates the slurry flow through the grate and pulp lifters. Based on this data the two models were fine-tuned to fit the Cortez SAG will. The All-wavelength Extended Groth Strip International Survey (AEGIS) Data Sets

In this the first of a series of ''Letters'', we present a description of the panchromatic data sets that have been acquired in the Extended Groth Strip region of the sky. Our survey, the All-wavelength Extended Groth Strip International Survey (AEGIS), is intended to study the physical properties and evolutionary processes of galaxies at z {approx} 1. It includes the following deep, wide-field imaging data sets: Chandra/ACIS{sup 30} X-ray (0.5-10 keV), GALEX{sup 31} ultraviolet (1200-2500 A), CFHT/MegaCam Legacy Survey{sup 32} optical (3600-9000 {angstrom}), CFHT/CFH12K optical (4500-9000 {angstrom}), Hubble Space Telescope/ACS{sup 33} optical (4400-8500 {angstrom}), Palomar/WIRC{sup 34} near-infrared (1.2-2.2 {micro}m), Spitzer/IRAC{sup 35} mid-infrared (3.6-8.0 {micro}m), Spitzer/MIPS far-infrared (24-70 {micro}m), and VLA{sup 36} radio continuum (6-20 cm). In addition, this region of the sky has been targeted for extensive spectroscopy using the DEIMOS spectrograph on the Keck II 10 m telescope{sup 37}. Our survey is compared to other large multiwavelength surveys in terms of depth and sky coverage.

The purpose of this calculation is to determine the structural response of a Naval SNF (Spent Nuclear Fuel) Long Waste Package (WP) subjected to 2 m-vertical drop on unyielding surface (US). The scope of this document is limited to reporting the calculation results in terms of maximum stress intensities. This calculation is associated with the waste package design; calculation is performed by the Waste Package Design group. AP-3.12Q, Revision 0, ICN 0, Calculations, is used to perform the calculation and develop the document. The finite element calculation is performed by using the commercially available ANSYS Version (V) 5.4 finite element code. The result of this calculation is provided in terms of maximum stress intensities.
