Traumatic Brain Injury: Better DOD and VA Oversight Can Help Ensure More Accurate, Consistent, and Timely Decisions for the Traumatic Injury Insurance Program
A letter report issued by the Government Accountability Office with an abstract that begins "In 2005, Congress created a traumatic injury insurance benefit program, known as TSGLI, to help servicemembers with traumatic brain injury and other serious injuries with the financial burdens that they and their families face. The Department of Veterans Affairs (VA) administers the program, in collaboration with the Department of Defense (DOD), while the branches of service are responsible for deciding servicemembers' claims. GAO examined (1) the TSGLI approval rate for traumatic brain injury claimants, and whether DOD and VA have assurance that claims are processed accurately, consistently, and in a timely manner and (2) any challenges servicemembers with traumatic brain injury may have faced in accessing TSGLI benefits, and the extent to which DOD and VA have taken steps to address such challenges. GAO analyzed program data and interviewed DOD and VA officials, servicemembers, and medical professionals."

Defense Acquisitions: Risks Posed by DOD's New Space Systems Acquisition Policy

Secure Border Initiative Fence Construction Costs
Correspondence issued by the Government Accountability Office with an abstract that begins "Much of the United States' 6,000 miles of international borders with Canada and Mexico remains vulnerable to illegal entry of aliens, criminals, and cargo. The Department of Homeland Security (DHS) apprehends hundreds of thousands of people and seizes large volumes of cargo entering the country illegally each year; however, several hundreds of thousands of individuals and an unknown volume of contraband also enter the United States illegally and undetected. DHS's U.S. Customs and Border Protection (CBP) is the agency responsible for securing the nation's borders along and between ports of entry. In November 2005, DHS announced the launch of the Secure Border Initiative (SBI), a multiyear, multibillion-dollar program aimed at securing U.S. borders and reducing illegal immigration. CBP's SBI program office is responsible for managing the SBI program and for developing a comprehensive border protection system. This system has two main components: SBInet, which employs radars, sensors, and cameras to detect, identify, and classify the threat level associated with an illegal entry into the United States between the ports of entry, and SBI tactical infrastructure (TI), fencing, roads, and lighting intended to enhance U.S. Border Patrol agents' ability …

Hurricanes Katrina and Rita Disaster Relief: Prevention Is the Key to Minimizing Fraud, Waste, and Abuse in Recovery Efforts
Testimony issued by the Government Accountability Office with an abstract that begins "Hurricanes Katrina and Rita destroyed homes and displaced millions of individuals. While federal and state governments continue to respond to this disaster, GAO has identified significant control weaknesses--specifically in the Federal Emergency Management Agency (FEMA)'s Individuals and Households Program (IHP) and in Department of Homeland Security (DHS)'s purchase card program--resulting in significant fraud, waste, and abuse. In response to the numerous recommendations GAO made, DHS and FEMA have reported on numerous actions taken to address our recommendations. Lessons learned from GAO's prior work can serve as a framework for an effective fraud prevention system for federal and state governments as they consider spending billions more on disaster recovery. These lessons are particularly important because funding that is lost to fraud, waste, and abuse reduces the amount of money that could be delivered to victims in need. Today's testimony will (1) describe key findings from past GAO work and (2) use the results from that work and GAO's other experiences to discuss the importance of an effective fraud, waste and abuse prevention program."

State and Local Government Retiree Benefits: Current Funded Status of Pension and Health Benefits
A letter report issued by the Government Accountability Office with an abstract that begins "Pension and other retiree benefits for state and local government employees represent liabilities for state and local governments and ultimately a burden for state and local taxpayers. Since 1986, accounting standards have required state and local governments to report their unfunded pension liabilities. Recently, however, standards changed and now call for governments also to report retiree health liabilities. The extent of these liabilities nationwide is not yet known, but some predict they will be very large, possibly exceeding a trillion dollars in present value terms. The federal government has an interest in assuring that all Americans have a secure retirement, as reflected in the federal tax deferral for contributions to both public and private pension plans. Consequently, the GAO was asked to examine: 1) the key measures of the funded status of retiree benefits and 2) the current funded status of retiree benefits. GAO analyzed data on public pensions, reviewed current literature, and interviewed a range of experts on public retiree benefits, actuarial science, and accounting."

Dietary Supplements: FDA Should Take Further Actions to Improve Oversight and Consumer Understanding
A letter report issued by the Government Accountability Office with an abstract that begins "Dietary supplements and foods with added dietary ingredients, such as vitamins and herbs, constitute multibillion dollar industries. Past reports on the Food and Drug Administration's (FDA) regulation of these products raised concerns about product safety and the availability of reliable information. Since then, FDA published draft guidance on requirements for reporting adverse events--which are harmful effects or illnesses--and Current Good Manufacturing Practice regulations for dietary supplements. GAO was asked to examine FDA's (1) actions to respond to the new serious adverse event reporting requirements, (2) ability to identify and act on concerns about the safety of dietary supplements, (3) ability to identify and act on concerns about the safety of foods with added dietary ingredients, and (4) actions to ensure that consumers have useful information about the safety and efficacy of supplements."

Nuclear Detection: Domestic Nuclear Detection Office Should Improve Planning to Better Address Gaps and Vulnerabilities
A letter report issued by the Government Accountability Office with an abstract that begins "In April 2005, the Domestic Nuclear Detection Office (DNDO) was established within the Department of Homeland Security (DHS) to enhance and coordinate federal, state, and local efforts to combat nuclear smuggling domestically and overseas. DNDO was directed to develop, in coordination with the departments of Defense (DOD), Energy (DOE), and State (State), a global strategy for nuclear detection--a system of radiation detection equipment and interdiction activities domestically and abroad. GAO was asked to examine (1) DNDO's progress in developing programs to address critical gaps in preventing nuclear smuggling domestically, (2) DNDO's role in supporting other agencies' efforts to combat nuclear smuggling overseas, and (3) the amount budgeted by DHS, DOD, DOE, and State for programs that constitute the global nuclear detection strategy. To do so, GAO analyzed agency documents; interviewed agency, state, and local officials; and visited select pilot program locations."

Pension and Welfare Benefits Administration: Opportunities Exist for Improving Management of the Enforcement Program
A letter report issued by the General Accounting Office with an abstract that begins "The Pension and Welfare Benefits Administration (PWBA) safeguards the economic interests of more than 150 million people in six million employee benefit plans. To enforce the provisions of the Employee Retirement Income Security Act of 1974 (ERISA), PWBA investigates employee benefit plans, conducts public education, and has begun a new voluntary correction program. Through its plan investigations, PWBA detects and corrects violations and has a deterrent presence that will prevent future violations. The Office of Enforcement prescribes areas for its regions to investigate. Regional offices have considerable flexibility in implementing PWBA's enforcement strategy by focusing most of their investigations on local issues. Although PWBA has taken steps to strengthen its enforcement activities, GAO found weaknesses in PWBA's enforcement strategy management and investigative process, its overall human capital management, and its measures for addressing program performance."

Medical Devices: Challenges for FDA in Conducting Manufacturer Inspections
Testimony issued by the Government Accountability Office with an abstract that begins "As part of the Food and Drug Administration's (FDA) oversight of the safety and effectiveness of medical devices marketed in the United States, it inspects domestic and foreign establishments where these devices are manufactured. To help FDA address shortcomings in its inspection program, the Medical Device User Fee and Modernization Act of 2002 required FDA to accredit third parties to inspect certain establishments. In response, FDA has implemented two such voluntary programs. GAO previously reported on the status of one of these programs, citing concerns regarding its implementation and factors that may influence manufacturers' participation. (Medical Devices: Status of FDA's Program for Inspections by Accredited Organizations, GAO-07-157, January 2007.) This statement (1) assesses FDA's management of inspections of establishments--particularly those in foreign countries--manufacturing devices for the U.S. market, and (2) provides the status of FDA's programs for third-party inspections of medical device manufacturing establishments. GAO interviewed FDA officials; reviewed pertinent statutes, regulations, guidance, and reports; and analyzed information from FDA databases. GAO also updated its previous work on FDA's programs for inspections by accredited third parties."

2000 Census: Coverage Measurement Programs' Results, Costs, and Lessons Learned
A letter report issued by the General Accounting Office with an abstract that begins "To help measure the quality of the 2000 Census and to possibly adjust for any errors, the U.S. Census Bureau (Bureau) conducted the Accuracy and Coverage Evaluation (A.C.E.) program. However, after obligating around $207 million for A.C.E. and its predecessor program, Integrated Coverage Measurement (I.C.M.), from fiscal years 1996 through 2001, the Bureau did not use either program to adjust the census numbers. Concerned about the amount of money the Bureau spent on I.C.M. and A.C.E. programs and what was produced in return, the subcommittee asked us to review the objectives and results of the programs, the costs of consultants, and how best to track future coverage measurement activities."

Small Business Administration: Response to September 11 Victims and Performance Measures for Disaster Lending

VA Drug Formulary: Better Oversight Is Required, but Veterans Are Getting Needed Drugs
A letter report issued by the General Accounting Office with an abstract that begins "During the last three years, the Department of Veterans Affairs (VA) has made significant progress in establishing its national drug formulary, which has generally met with the prescriber acceptance. Most veterans are receiving the drugs the need and rarely register complaints about prescription drugs. However, VA has not been sufficient to ensure that the Veterans Integrated Service Networks (VISN) and medical centers comply with formulary policies and that the flexibility given to them does not compromise VA's goal of formulary standardization. Contrary to VA formulary policy, some facilities omitted national formulary drugs or modified the closest drug classes. Although a limited number of drugs to supplement the national formulary is permitted, formulary differences among facilities are likely to become more pronounced, as more drugs are added by VISNs, decreasing formulary standardization. VA recognizes the trade-off between local flexibility and standardization, but it lacks criteria for determining the appropriateness of adding drugs to supplement the national formulary and therefore may not be able to determine whether the decrease in standardization is acceptable."

International Environment: U.S. Actions to Fulfill Committments Under Five Key Agreements
A letter report issued by the General Accounting Office with an abstract that begins "Environmental problems do not respect national boundaries. These problems include (1) climate change, (2) drought and the expansion of degraded land, (3) environmental cooperation among the countries of North America, (4) illegal trade in endangered species, and (5) substances that deplete the earth's protective ozone layer. To address such problems, the United States and other nations have entered into numerous international environmental agreements. In implementing these agreements, the parties typically commit to establish domestic programs and report periodically on their progress. Developed nations like the United States may also pledge to provide funds to assist developing nations. GAO was asked to examine (1) U.S. actions to fulfill its commitments under five international agreements identified by the requesters, (2) the means used to track these actions, and (3) the results of others' evaluations of these actions for the selected agreements."
