A letter report issued by the Government Accountability Office with an abstract that begins "The Medicare Improvements for Patients and Providers Act of 2008 directed the Secretary of Health and Human Services to develop a program to give physicians confidential feedback on the Medicare resources used to provide care to Medicare beneficiaries. GAO was asked to evaluate the per capita methodology for profiling physicians--a method which measures a patient's resource use over a fixed period of time and attributes that resource use to physicians--in order to assist the Centers for Medicare & Medicaid Services (CMS) with the development of a physician feedback approach. In response, this report examines (1) the extent to which physicians in selected specialties show stable practice patterns and how beneficiary utilization of services varies by physician resource use level; (2) factors to consider in developing feedback reports on physicians' performance, including per capita resource use; and (3) the extent to which feedback reports may influence physician behavior. GAO focused on four medical specialties and four metropolitan areas chosen for their geographic diversity and range in average Medicare spending per beneficiary. A letter report issued by the Government Accountability Office with an abstract that begins "The Department of Defense (DOD) has spent a total of over $115 billion since the mid-1980s to develop a Ballistic Missile Defense System (BMDS) comprised of land, air, and sea-based elements--such as missiles and radars--working together as an integrated system. Since the cost to operate and support a weapon system usually accounts for most of a system's lifetime costs, the resources needed to fund BMDS could be significant as DOD fields an increasing number of BMDS elements. In 2005, DOD began planning to transition responsibility for supporting BMDS elements from the Missile Defense Agency (MDA) to the services. GAO was asked to assess the extent to which DOD has (1) planned to support BMDS elements over the long-term, and (2) identified long-term operation and support costs. To do so, GAO analyzed 7 BMDS elements that will be fielded by 2015, compared DOD's plans and cost estimates to DOD and GAO key principles, and assessed the extent to which MDA and the services have agreed on responsibilities for supporting and funding BMDS elements."

A letter report issued by the Government Accountability Office with an abstract that begins "Between fiscal years 2010 and 2012, the Department of Homeland Security's (DHS) border and maritime research and development (R&D) components reported producing 97 R&D deliverables at an estimated cost of $177 million. The type of border and maritime R&D deliverables produced by DHS's Science and Technology (S&T) Directorate, the Coast Guard, and the Domestic Nuclear Detection Office (DNDO) varied, and R&D customers we met with reported mixed views on the impact of the R&D deliverables they received. These deliverables were wide-ranging in their cost and scale, and included knowledge products and reports, technology prototypes, and software (as shown in the figure below). The Coast Guard and DNDO reported having processes in place to collect and evaluate feedback from its customers regarding the results of R&D deliverables. However, S&T has not established timeframes and milestones for collecting and evaluating feedback from its customers on the extent to which the deliverables it provides to DHS components--such as US Customs and Border Protection (CBP)--are meeting its customer's needs. A letter report issued by the Government Accountability Office with an abstract that begins "The volume, but not the energy content, of hydrocarbon fuels, such as gasoline and diesel, varies in response to changes in temperature. Thus, because of expansion, the energy content per gallon of 90 degree fuel is less than that of 60 degree fuel. States and localities adopt and enforce weights and measures regulations, often using the model regulatory standards published by the National Institute of Standards and Technology (NIST). Although technology now exists to compensate for the effects of temperature on gas volume, the costs of doing so at the retail level have become the subject of much debate among weights and measures officials, consumer groups, and representatives of the petroleum and fuel marketing industries. GAO was asked to provide information on (1) the views of U.S. stakeholders on the costs to implement automatic temperature compensation, (2) the views of U.S. stakeholders on who would bear these costs, and (3) the reasons some state and national governments have adopted or rejected automatic temperature compensation. A letter report issued by the Government Accountability Office with an abstract that begins "During the late 1990s, many petroleum companies merged to stay profitable while crude oil prices were low, and in recent years mergers have continued. Congress and others have concerns about the impact mergers might be having on competition in U.S. petroleum markets. The Federal Trade Commission (FTC) has the authority to maintain competition in the petroleum industry and reviews proposed mergers to determine whether they are likely to diminish competition or increase prices, among other things. GAO was asked to examine (1) mergers in the U.S. petroleum industry and changes in market concentration since 2000 and (2) the steps FTC uses to maintain competition in the U.S. petroleum industry, and the roles other federal and state agencies play in monitoring petroleum industry markets. In conducting this study, GAO worked with petroleum industry experts to delineate regional markets and to develop estimates of refinery gasoline production capacity in order to calculate market concentration. A letter report issued by the Government Accountability Office with an abstract that begins "Since fiscal year 2008, U.S. agencies allocated over $1.2 billion in funding for Central America Regional Security Initiative (CARSI) activities and non-CARSI funding that supports CARSI goals. As of June 1, 2013, the Department of State (State) and the United States Agency for International Development (USAID) obligated at least $463 million of the close to $495 million in allocated funding for CARSI activities, and disbursed at least $189 million to provide partner countries with equipment, technical assistance, and training to improve interdiction and disrupt criminal networks. Moreover, as of March 31, 2013, U.S. agencies estimated that they had allocated approximately $708 million in non-CARSI funding that supports CARSI goals, but data on disbursements were not readily available. U.S. agencies, including State, the Department of Defense (DOD), and the Department of Justice, use this funding to provide equipment, technical assistance, and training, as well as infrastructure and investigation assistance to partner countries. For example, DOD allocated $25 million in funding to help Guatemala establish an interagency border unit to combat drug trafficking."

For example, through the Visa Security Program, DHS has deployed personnel abroad to help prevent the issuance of visas to people who might pose a threat. As of May 2013, DHS has stationed about 1,800 employees in almost 80 countries to conduct these and other activities. In addition, DHS has delivered training and technical assistance in areas such as border and aviation security to officials from about 180 countries to enhance partner nations' security capacities."

A letter report issued by the Government Accountability Office with an abstract that begins "Increasing combat demands and fiscal constraints make it critical for the Department of Defense (DOD) to ensure that its weapon system investments not only meet the needs of the warfighter, but make the most efficient use of available resources. GAO's past work has shown that achieving this balance has been a challenge and weapon programs have often experienced cost growth and delayed delivery to the warfighter. In 2003, DOD implemented the Joint Capabilities Integration and Development System (JCIDS) to prioritize and ensure that the warfighter's most essential needs are met. In response to Senate Report 109-69, GAO reported in March 2007 that DOD lacks an effective approach to balance its weapon system investments with available resources. This follow-on report focuses on (1) whether the JCIDS process has achieved its objective to prioritize joint warfighting needs and (2) factors that have affected DOD's ability to effectively implement JCIDS. To conduct its work, GAO reviewed JCIDS guidance and capability documents and budgetary and programming data on major weapon systems, and interviewed DOD officials."

A letter report issued by the Government Accountability Office with an abstract that begins "Terrorists using unconventional weapons, also known as nuclear, biological, chemical, or radiological (NBCR) weapons, could cause devastating losses. The Terrorism Risk Insurance Act (TRIA) of 2002, as well as the extension passed in 2005, will cover losses from a certified act of terrorism, irrespective of the weapon used, if those types of losses are included in the coverage. Because of a lack of information about the willingness of insurers to cover NBCR risks and uncertainties about the extent to which these risks can be and are being insured by private insurers across various lines of insurance, GAO was asked to study these issues. This report discusses (1) commonly accepted principles of insurability and whether NBCR risks are measurable and predictable, and (2) whether private insurers currently are exposed to NBCR risks and the challenges they face in pricing such risks. GAO collected information from and met with some of the largest insurers in each line of insurance, associations representing a broader cross section of the industry and state insurance regulators. A letter report issued by the Government Accountability Office with an abstract that begins "The Department of Defense (DOD) has improved its methodology for estimating and comparing the full cost to the taxpayer of work performed by military and civilian personnel and contractor support, but the methodology continues to have certain limitations. Best practices state that cost estimating rules should include a common set of standards that minimize conflicts in definitions, but DOD's methodology does not provide guidance for certain costs. For instance, its estimate of service training costs divides total training funding by the number of servicemembers. Using this method yields an average training cost of $6,490 per servicemember in the Army for fiscal year 2012. However, Army data show that training for a general aviation officer can be as high as $93,600 a year, while the training for an enlisted infantryman can be as low as about $4,600 a year. DOD's Cost Assessment and Program Evaluation (CAPE) office has not provided more specific direction on training costs, although some officials have requested it.
