VA Health Care: Reliability of Reported Outpatient Medical Appointment Wait Times and Scheduling Oversight Need Improvement

A letter report issued by the Government Accountability Office with an abstract that begins "Outpatient medical appointment wait times reported by the Veterans Health Administration (VHA), within the Department of Veterans Affairs (VA), are unreliable. Wait times for outpatient medical appointments--referred to as medical appointments--are calculated as the number of days elapsed from the desired date, which is defined as the date on which the patient or health care provider wants the patient to be seen. The reliability of reported wait time performance measures is dependent on the consistency with which schedulers record the desired date in the scheduling system in accordance with VHA's scheduling policy. However, VHA's scheduling policy and training documents for recording desired date are unclear and do not ensure consistent use of the desired date. Some schedulers at Veterans Affairs medical centers (VAMC) that GAO visited did not record the desired date correctly. For example, three schedulers changed the desired date based on appointment availability; this would have resulted in a reported wait time that was shorter than the patient actually experienced. Veterans' Disability Benefits: Timely Processing Remains a Daunting Challenge

A letter report issued by the Government Accountability Office with an abstract that begins "A number of factors—both external and internal to the Veterans Benefits Administration (VBA)—have contributed to the increase in processing timeframes and subsequent growth in the backlog of veterans’ disability compensation claims. As the population of new veterans has swelled in recent years, the annual number of claims received by VBA has gone up. Compared to the past, these claims have a higher number of disabling conditions, and some of these conditions, such as traumatic brain injuries, make their assessment complex. Moreover, due to new regulations that have established eligibility for benefits for new diseases associated with Agent Orange exposure, VBA adjudicated 260,000 previously denied and new claims. Beyond these external factors, issues with the design and implementation of the compensation program have contributed to timeliness challenges. For example, the law requires the Department of Veterans Affairs (VA) to assist veterans in obtaining records that support their claim. However, VBA officials said that lengthy timeframes in obtaining military records—particularly for members of the National Guard and Reserve—and Social Security Administration (SSA) medical records impact VA’s duty to assist, possibly delaying a decision on a veteran’s disability claim. …

This is one of two products we are issuing in response to congressional October 13, 2009 request that we evaluate the Department of Defense's (DOD) plans for implementing EPAA. We provided congressional staff a draft copy of this briefing in a meeting with them on September 22, 2010. We do not make any recommendations in the briefing. We will issue a final report on broader issues of European missile defense that will include the material in the briefing. That final product will have recommendations, as appropriate. For the briefing we sought to answer two questions: (1) What key acquisition planning and management practices are in place for EPAA? (2) Are there near-term development risks for EPAA?"

Godiva and Juliet Diagnostics CED-1 (IER-176)

A suite of diagnostics are being proposed for use in the Juliet experiment (IER-128). In order to calibrate and test the diagnostics prior to use, the LLNL calibration facility and Godiva pulsed reactor will be used to provide intense sources of neutrons and gammas. Due to the similarities of the Godiva and Juliet radiation fields, the diagnostics being developed and tested for Juliet can also play an on-going role in diagnostics for Godiva as well as, perhaps, other critical assembly experiments. Similar work is also being conducted for IER-147 for the purpose of characterizing the Godiva radiation field in support of an upcoming international nuclear accident dosimetry exercise. Diagnostics developed and fielded under IER-147 can provide valuable data with respect to the neutron and gamma energy spectrums in the vicinity of Godiva which is relevant to the calibration of Juliet diagnostics.

FINAL SCIENTIFIC/TECHNICAL REPORT

Dynalene Inc has developed and patented a fuel cell coolant with the help of DOE SBIR Phase I and Phase II funding (Project DE-FG02-04ER83884). However, this coolant could only be produced in lab scale (500 ml to 2 L) due to problems in the optimization and scale-up of a nanoparticle ingredient. This project optimized the nanoparticle production process in 10 L and 100 L reactors (which translates to about 5000 gallons of coolant), optimized the filtration process for the nanoparticles, and develop a high throughput production as well as quality control method for the final coolant formulation. Scale-up of nanoparticle synthesis (using emulsion polymerization) is an extremely challenging task. Dynalene researchers, in collaboration with a university partner, identified all the parameters affecting the size, charge density and coagulation characteristics of the nanoparticles and then optimized these parameters to achieve the goals and the objectives of this project. Nanoparticle synthesis was demonstrated to be reproducible in the 10 L and 100 L scales.

The Magellan Final Report on Cloud Computing

The goal of Magellan, a project funded through the U.S. Department of Energy (DOE) Office of Advanced Scientific Computing Research (ASCR), was to investigate the potential role of cloud computing in addressing the computing needs for the DOE Office of Science (SC), particularly related to serving the needs of mid- range computing and future data-intensive computing workloads. A set of research questions was formed to probe various aspects of cloud computing from performance, usability, and cost. To address these questions, a distributed testbed infrastructure was deployed at the Argonne Leadership Computing Facility (ALCF) and the National Energy Research Scientific Computing Center (NERSC). The testbed was designed to be flexible and capable enough to explore a variety of computing models and hardware design points in order to understand the impact for various scientific applications. During the project, the testbed also served as a valuable resource to application scientists. Applications from a diverse set of projects such as MG-RAST (a metagenomics analysis server), the Joint Genome Institute, the STAR experiment at the Relativistic Heavy Ion Collider, and the Laser Interferometer Gravitational Wave Observatory (LIGO), were used by the Magellan project for benchmarking within the cloud, but the project teams were also able …

Leakage testing of a representative sample of the safety-significant isolation valves for Double Valve Isolation (DVI) in an environment that simulates the abrasive characteristics of the Hanford Tank Farms Waste Transfer System during waste feed delivery to the Waste Treatment and Immobilization Plant (WTP) is to be conducted. The testing will consist of periodic leak performed on the DVI valves after prescribed numbers of valve cycles (open and close) in a simulated environment representative of the abrasive properties of the waste and the Waste Transfer System. The valve operations include exposure to cycling conditions that include gravity drain and flush operation following slurry transfer. The simulant test will establish the performance characteristics and verify compliance with the Documented Safety Analysis.
