A letter report issued by the General Accounting Office with an abstract that begins "The Centers for Medicare & Medicaid Services (CMS) recently implemented a Medicare ambulance fee schedule in which providers are paid a base payment per trip plus a mileage payment. An adjustment is made to the mileage rate for rural trips to account for higher costs. CMS has stated that this rural adjustment may not sufficiently target providers serving sparsely populated rural areas. The Medicare, Medicaid, and SCHIP Benefits Improvement and Protection Act of 2000 (BIPA) directed GAO to examine rural ambulance costs. GAO identified factors that affect ambulance costs per trip, examined how these factors varied across geographic areas, and analyzed whether Medicare payments account for geographic cost differences. GAO used survey data on ambulance providers and Medicare claims data."

A letter report issued by the General Accounting Office with an abstract that begins "The National Defense Authorization Act for Fiscal Year 2003 directed the Department of Defense (DOD) to develop an enterprise architecture and a transition plan that meets certain requirements. The act also directed DOD to have a process for controlling its system investments. As required by the act, GAO assessed DOD's actions to comply with the act's requirements and recently issued a report to congressional defense committees. This report provides further details of GAO's assessment results regarding (1) the extent to which DOD's actions complied with the requirements of the act and (2) DOD's plans for further development and implementation of its architecture."

A letter report issued by the General Accounting Office with an abstract that begins "The Department of Homeland Security (DHS) plans to establish a program to strengthen management of the pre-entry, entry, status, and exit of foreign nationals who travel to the United States. The goals of the program, known as the United States Visitor and Immigrant Status Indicator Technology (US-VISIT), are to facilitate legitimate trade and travel, enhance national security, and adhere to U.S. privacy laws and policies. By congressional mandate, DHS is to develop and submit for approval an expenditure plan for US-VISIT that satisfies certain conditions, including being reviewed by GAO. GAO was asked to determine, among other things, whether the plan satisfies these conditions and to provide observations about the plan and DHS's management of the program."

A letter report issued by the General Accounting Office with an abstract that begins "Each year federal agencies issue numerous proposed or final rules or take other regulatory actions that may potentially affect the use of private property. Some of these actions may result in the property owner being owed just compensation under the Fifth Amendment. In 1988 the President issued Executive Order 12630 on property rights to ensure that government actions affecting the use of private property are undertaken on a well-reasoned basis with due regard for the potential financial impacts imposed on the government. GAO was asked to provide information on the compliance of the Department of Justice and four agencies--the Department of Agriculture, the Army Corps of Engineers, the Environmental Protection Agency, and the Department of the Interior--with the executive order. Specifically, GAO examined the extent to which (1) Justice has updated its guidelines for the order to reflect changes in case law and issued supplemental guidelines for the four agencies, (2) the four agencies have complied with the specific provisions of the executive order, and (3) just compensation awards have been assessed against the four agencies in recent years. The Worker Adjustment and Retraining Notification (WARN) Act requires advance notice of plant closures and mass layoffs. The report discusses (1) the extent to which plant closures and mass layoffs were subject to WARN's requirements, (2) the extent to which employers with mass layoffs and plant closures provided notice, and (3) what issues employers and employees face when assessing the applicability of WARN to their circumstances."

A letter report issued by the General Accounting Office with an abstract that begins "Following the 1990-91 Persian Gulf War, many servicemembers experienced health problems that they attributed to their military service in the Persian Gulf. However, a lack of servicemember health and deployment data hampered subsequent investigations into the nature and causes of these illnesses. Public Law 105-85, enacted in November 1997, required the Department of Defense (DOD) to establish a system to assess the medical condition of service members before and after deployments. GAO was asked to determine whether (1) the military services met DOD's force health protection and surveillance requirements for servicemembers deploying in support of Operation Enduring Freedom (OEF) in Central Asia and Operation Joint Guardian (OJG) in Kosovo and (2) DOD has corrected problems related to the accuracy and completeness of databases reflecting which servicemembers were deployed to certain locations."

A letter report issued by the General Accounting Office with an abstract that begins "Critical Access Hospitals (CAHs) are small rural hospitals that receive payment for their reasonable costs of providing inpatient and outpatient services to Medicare beneficiaries, rather than being paid fixed amounts under Medicare's prospective payment systems. Between fiscal years 1997 and 2002, 681 hospitals have become CAHs. In the Medicare, Medicaid and SCHIP Benefits Improvement and Protection Act of 2000, GAO was directed to examine requirements for CAH eligibility, including the ban on inpatient psychiatric or rehabilitation distinct part units (DPUs) and limit on patient census, and to make recommendations on related program changes."

A letter report issued by the General Accounting Office with an abstract that begins "The Department of Defense (DOD) must convince more than 200,000 people each year to join the military. To assist in recruiting, the military services advertise on television, on radio, and in print and participate in other promotional activities. In the late 1990s, some of the services missed their overall recruiting goals. In response, DOD added recruiting resources by increasing its advertising, number of recruiters, and financial incentives. By fiscal year 2003, DOD's total recruiting budget was approaching $4 billion annually. At the request of Congress, GAO determined the changes in DOD's advertising programs and funding trends since the late 1990s and assessed the adequacy of measures used by DOD to evaluate the effectiveness of its advertising. GAO recommends that DOD set clear, measurable advertising"

This document is the final report for the Cooperative Research and Development Agreement (CRADA) between UT-Battelle and Prime Photonics, Inc. The purpose of this CRADA was to improve the properties of single crystal sapphire optical fibers for sensor applications. A reactive coating process was developed to form a magnesium aluminate spinel cladding on sapphire optical fibers. The resulting clad fiber had a numerical aperture, NA, of 0.09 as compared with 0.83 for the unclad fiber, dramatically enhancing its usefulness for sensor applications. Because the process allows one to control the diameter of the sapphire core within the fiber, it may be possible using this technology to develop waveguides that approach single-mode transmission character.

During the first 18 months of this project, four experimental subtasks were to have begun but only one of these was to have been completed. Additionally, five modeling subtasks were scheduled to begin, four of which were to have been completed. We are on schedule for all but one of these subtasks. All four experimental tasks are progressing on schedule. Initial durability tests were completed. The conclusions drawn from this first round of durability tests are being used to design the next set of tests. Initial baseline engine data were acquired and showed that the engine selected for this task behaves as hoped. However, the dyno controller is inadequate. The engine will be moved to another dyno during the near future. The modeling tasks are also progressing well. A model for the dynamic response of the ignition circuit was developed and validated. Two technical papers resulting from this model were submitted for publication. Development of a model for the railplug ignition process was begun but was not scheduled for completion. Progress on this task consisted of two subtasks. First, a railplug circuit model was also developed and validated. Second, a model was developed for the physics that govern railplug performance. …

Gas Puff Imaging (GPI) experiments are designed to isolate the structure of plasma turbulence in the plane perpendicular to the magnetic field. Three-dimensional aspects of this diagnostic technique as used on the National Spherical Torus eXperiment (NSTX) are examined via Monte Carlo neutral transport simulations. The radial width of the simulated GPI images are in rough agreement with observations. However, the simulated emission clouds are angled approximately 15 degrees with respect to the experimental images. The simulations indicate that the finite extent of the gas puff along the viewing direction does not significantly degrade the radial resolution of the diagnostic. These simulations also yield effective neutral density data that can be used in an approximate attempt to infer two-dimensional electron density and temperature profiles from the experimental images.

We have started to examine the reactor potential of quasi-axisymmetric (QA) stellarators with an integrated approach that includes systems evaluation, engineering considerations, and plasma and coil optimizations. In this paper, we summarize the progress made so far in developing QA configurations with reduced alpha losses while retaining good MHD stability properties. The minimization of alpha losses is achieved by directly targeting the collisionless orbits to prolong the average resident times. Configurations with an overall energy loss rate of {approx}10% or less, including collisional contributions, have been found. To allow remotely maintaining coils and machine components in a reactor environment, there is a desire to simplify to the extent possible the coil design. To this end, finding a configuration that is optimized not only for the alpha confinement and MHD stability but also for the good coil and reactor performance, remains to be a challenging task.

A letter report issued by the General Accounting Office with an abstract that begins "Scientists at the Plum Island Animal Disease Center are responsible for protecting the nation against animal diseases that could be accidentally or deliberately introduced into the country. Questions about the security of Plum Island arose after the 2001 terrorist attacks and when employees of the contractor hired to operate and maintain the Plum Island facilities went on strike in August 2002. GAO reviewed (1) the adequacy of security at Plum Island and (2) how well the contractor performed during the strike. The Department of Homeland Security (DHS) assumed the administration of Plum Island from the Department of Agriculture (USDA) on June 1, 2003. While DHS is now responsible for Plum Island, USDA is continuing its research and diagnostic programs."

Proposed NIF ICF capsules are sensitive to asymmetries resulting from radiation drive and capsule imperfections. Capsule imperfections are amplified by the Rayleigh-Taylor and Richtmeyer-Meshkoff instabilities, and pose a risk at mode numbers in the range of 20 to 100. Radiation drive asymmetries are also amplified by these hydrodynamic instabilities, and pose a danger at modes below 10. Previous work has mostly concentrated on quantifying the sensitivity to capsule imperfections. Here we concentrate on characterizing the sensitivity to drive asymmetries.

The National Museum of the Pacific War presents an oral interview with Arnold Spielberg. Spielberg was born in Cincinnati, Ohio on 6 February 1917. His mother and father migrated from the Ukraine. He joined the United States Army and based on his experience as a “Ham” radio operator and knowledge of Morse code, he was assigned to the 442nd Signal Corps. Soon afterwards, he was assigned to the United States Army Air Forces and went aboard the SS Santa Paul in May 1942 bound for Karachi, India. After a short stint in supply, Spielberg was transferred to the 490th Bombardment Group where he set up a communications center at Ondal, India. The unit then moved to Bishnapur, India where he became the communications chief. During this time he was instrumental in setting up a rhombic directional antenna to improve communication with New Delhi. He describes instances where he went directly to the manufacturer for parts that were badly needed. Following his tour in India, Spielberg was assigned to research and development at Wright Field where he stayed until he was discharged.

The National Museum of the Pacific War presents an oral interview with Mose Davie. Davie was born in Clarksville, Tennessee on 12 July 1918. He graduated from high school in 1938 and attended Tennessee State University majoring in engineering. In 1941 he was drafted into the United States Army under the Selective Training and Service Act. Following basic training he was assigned to the 382nd Engineer Construction Battalion. He tells of the segregated unit boarding the USS Hermitage (AP-54) bound for Bombay, India. Once in India, the battalion was assigned the job of building the Ledo Road. Working in jungles, swamps and over mountains and rivers, malaria was prevalent and many of the men died. He relates how food and supplies were brought in by C-46 and C-47 transport planes that flew over The Hump and of being subjected to periodic Japanese air raids over the construction camps. Upon completion of the Ledo Road, the battalion returned to India. Davie also attended USO shows that featured Joe E. Brown and Hank Greenberg. Following the surrender of Japan, Davie returned to the United States and received his discharge in 1945.
