Modeling Hepatitis C treatment policy.

Chronic infection with Hepatitis C virus (HCV) results in cirrhosis, liver cancer and death. As the nation's largest provider of care for HCV, US Veterans Health Administration (VHA) invests extensive resources in the diagnosis and treatment of the disease. This report documents modeling and analysis of HCV treatment dynamics performed for the VHA aimed at improving service delivery efficiency. System dynamics modeling of disease treatment demonstrated the benefits of early detection and the role of comorbidities in disease progress and patient mortality. Preliminary modeling showed that adherence to rigorous treatment protocols is a primary determinant of treatment success. In depth meta-analysis revealed correlations of adherence and various psycho-social factors. This initial meta-analysis indicates areas where substantial improvement in patient outcomes can potentially result from VA programs which incorporate these factors into their design.

Historical activities at facilities producing nuclear materials for weapons released radioactivity into the air and water. Past studies in the United States have evaluated the release, atmospheric transport and environmental accumulation of 131I from the nuclear facilities at Hanford in Washington State and the resulting dose to members of the public (Farris et al. 1994). A multi-year dose reconstruction effort (Mokrov et al. 2004) is also being conducted to produce representative dose estimates for members of the public living near Mayak, Russia, from atmospheric releases of 131I at the facilities of the Mayak Production Association. The approach to calculating individual doses to members of the public from historical releases of airborne 131I has the following general steps: • Construct estimates of releases 131I to the air from production facilities. • Model the transport of 131I in the air and subsequent deposition on the ground and vegetation. • Model the accumulation of 131I in soil, water and food products (environmental media). • Calculate the dose for an individual by matching the appropriate lifestyle and consumption data for the individual to the concentrations of 131I in environmental media at their residence location. The Advanced Energy Retrofit Guide for Healthcare Facilities is part of a series of retrofit guides commissioned by the U.S. Department of Energy. By presenting general project planning guidance as well as detailed descriptions and financial payback metrics for the most important and relevant energy efficiency measures (EEMs), the guides provide a practical roadmap for effectively planning and implementing performance improvements in existing buildings. The Advanced Energy Retrofit Guides (AERGs) are intended to address key segments of the U.S. commercial building stock: retail stores, office buildings, K-12 schools, grocery stores, and healthcare facilities. The guides' general project planning considerations are applicable nationwide; the energy and cost savings estimates for recommended EEMs were developed based on energy simulations and cost estimates for an example hospital tailored to five distinct climate regions. These results can be extrapolated to other U.S. climate zones. Analysis is presented for individual EEMs, and for packages of recommended EEMs for two project types: existing building commissioning projects that apply low-cost and no-cost measures, and whole-building retrofits involving more capital-intensive measures.

Plug-in Hybrid Initiative

Our main project objective was to implement Plug-in Electric Vehicles (PEV) and charging infrastructure into our electric distribution service territory and help reduce barriers in the process. Our research demonstrated the desire for some to be early adopters of electric vehicles and the effects lack of education plays on others. The response of early adopters was tremendous: with the initial launch of our program we had nearly 60 residential customers interested in taking part in our program. However, our program only allowed for 15 residential participants. Our program provided assistance towards purchasing a PEV and installation of Electric Vehicle Supply Equipment (EVSE). The residential participants have all come to love their PEVs and are more than enthusiastic about promoting the many benefits of driving electric.

This report fulfills the M2 milestone M2FT-13PN0912022, “Stranded Sites De-Inventorying Report.” In January 2013, the U.S. Department of Energy (DOE) issued the Strategy for the Management and Disposal of Used Nuclear Fuel and High-Level Radioactive Waste (DOE 2013). Among the elements contained in this strategy is an initial focus on accepting used nuclear fuel from shutdown reactor sites. This focus is consistent with the recommendations of the Blue Ribbon Commission on America’s Nuclear Future, which identified removal of stranded used nuclear fuel at shutdown sites as a priority so that these sites may be completely decommissioned and put to other beneficial uses (BRC 2012). Shutdown sites are defined as those commercial nuclear power reactor sites where the nuclear power reactors have been shut down and the site has been decommissioned or is undergoing decommissioning. In this report, a preliminary evaluation of removing used nuclear fuel from 12 shutdown sites was conducted. The shutdown sites were Maine Yankee, Yankee Rowe, Connecticut Yankee, Humboldt Bay, Big Rock Point, Rancho Seco, Trojan, La Crosse, Zion, Crystal River, Kewaunee, and San Onofre. Offshore wind energy is a valuable resource that can provide a significant boost to the US renewable energy portfolio. A current constraint to the development of offshore wind farms is the potential for interference to be caused by large wind farms on existing electronic and acoustical equipment such as radar and sonar systems for surveillance, navigation and communications. The US Department of Energy funded this study as an objective assessment of possible interference to various types of equipment operating in the marine environment where offshore wind farms could be installed. The objective of this project was to conduct a baseline evaluation of electromagnetic and acoustical challenges to sea surface, subsurface and airborne electronic systems presented by offshore wind farms. To accomplish this goal, the following tasks were carried out: (1) survey electronic systems that can potentially be impacted by large offshore wind farms, and identify impact assessment studies and research and development activities both within and outside the US, (2) engage key stakeholders to identify their possible concerns and operating requirements, (3) conduct first-principle modeling on the interactions of electromagnetic signals with, and the radiation of underwater acoustic signals from, offshore wind farms to evaluate the effect of such interactions …

Characterization Data Package for Containerized Sludge Samples Collected from Engineered Container SCS-CON-210

This data package contains the K Basin sludge characterization results obtained by Pacific Northwest National Laboratory during processing and analysis of four sludge core samples collected from Engineered Container SCS-CON-210 in 2010 as requested by CH2M Hill Plateau Remediation Company. Sample processing requirements, analytes of interest, detection limits, and quality control sample requirements are defined in the KBC-33786, Rev. 2. The core processing scope included reconstitution of a sludge core sample distributed among four to six 4-L polypropylene bottles into a single container. The reconstituted core sample was then mixed and subsampled to support a variety of characterization activities. Additional core sludge subsamples were combined to prepare a container composite. The container composite was fractionated by wet sieving through a 2,000 micron mesh and a 500-micron mesh sieve. Each sieve fraction was sampled to support a suite of analyses. The core composite analysis scope included density determination, radioisotope analysis, and metals analysis, including the Waste Isolation Pilot Plant Hazardous Waste Facility Permit metals (with the exception of mercury). The container composite analysis included most of the core composite analysis scope plus particle size distribution, particle density, rheology, and crystalline phase identification. Radioactive Demonstration Of Mineralized Waste Forms Made From Hanford Low Activity Waste (Tank SX-105, Tank AN-103, And AZ-101/102) By Fluidized Bed Steam Reformation (FBSR)

Fluidized Bed Steam Reforming (FBSR) is a robust technology for the immobilization of a wide variety of radioactive wastes. Applications have been tested at the pilot scale for the high sodium, sulfate, halide, organic and nitrate wastes at the Hanford site, the Idaho National Laboratory (INL), and the Savannah River Site (SRS). Due to the moderate processing temperatures, halides, sulfates, and technetium are retained in mineral phases of the feldspathoid family (nepheline, sodalite, nosean, carnegieite, etc). The feldspathoid minerals bind the contaminants such as Tc-99 in cage (sodalite, nosean) or ring (nepheline) structures to surrounding aluminosilicate tetrahedra in the feldspathoid structures. The granular FBSR mineral waste form that is produced has a comparable durability to LAW glass based on the short term PCT testing in this study, the INL studies, SPFT and PUF testing from previous studies as given in the columns in Table 1-3 that represent the various durability tests. Monolithing of the granular product was shown to be feasible in a separate study. Macro-encapsulating the granular product provides a decrease in leaching compared to the FBSR granular product when the geopolymer is correctly formulated.

The West Valley Demonstration Project (WVDP) Annual Site Environmental Report (ASER) for Calendar Year 2012. The report, prepared for the U.S. Department of Energy West Valley Demonstration Project office (DOE-WVDP), summarizes the environmental protection program at the WVDP for calendar year (CY) 2012. Monitoring and surveillance of the facilities used by the DOE are conducted to verify protection of public health and safety and the environment. The report is a key component of DOE’s effort to keep the public informed of environmental conditions at the WVDP. The quality assurance protocols applied to the environmental monitoring program ensure the validity and accuracy of the monitoring data. In addition to demonstrating compliance with environmental laws, regulations, and directives, evaluation of data collected in 2012 continued to indicate that WVDP activities pose no threat to public health or safety, or to the environment.

The Underground Test Area (UGTA) Corrective Action Unit (CAU) 97, Yucca Flat/Climax Mine, in the northeast part of the Nevada National Security Site (NNSS) requires environmental corrective action activities to assess contamination resulting from underground nuclear testing. These activities are necessary to comply with the UGTA corrective action strategy (referred to as the UGTA strategy). The corrective action investigation phase of the UGTA strategy requires the development of groundwater flow and contaminant transport models whose purpose is to identify the lateral and vertical extent of contaminant migration over the next 1,000 years. In particular, the goal is to calculate the contaminant boundary, which is defined as a probabilistic model-forecast perimeter and a lower hydrostratigraphic unit (HSU) boundary that delineate the possible extent of radionuclide-contaminated groundwater from underground nuclear testing. Because of structural uncertainty in the contaminant boundary, a range of potential contaminant boundaries was forecast, resulting in an ensemble of contaminant boundaries. The contaminant boundary extent is determined by the volume of groundwater that has at least a 5 percent chance of exceeding the radiological standards of the Safe Drinking Water Act (SDWA) (CFR, 2012).
