Investigating the Impact of Patient-Provider Communication on HIV Treatment Adherence

Today over 1.1 million people are living with HIV/AIDS in the United States; over the last 4 decades mortality rates have decreased largely made in part because of advancement in awareness and treatment options. Treatment adherence has long been considered a vital component in decreasing HIV/AIDS related mortality and has proven to reduce the risk of transmission. However not all patients take their medicine as prescribed. This research study, sponsored by The North Central Texas HIV Planning Council explored how Patient and Provider communication impacted treatment adherence. By utilizing a mixed-methods approach survey data and semi-structured interviews were used to collect insights from both Patients and Providers. Data gleaned through the interview process provided a perspective that could not be captured by using quantitative methods alone. The results from this research yielded multiple themes related to patient and provider communication with recommendations as to how The North Central Texas HIV Planning Council could address treatment adherence, such as Providers focus on Patients perceived severity based on their understanding of disease and illness; that side-effects remain a concern for patients and should not be dismissed; and finally that the word AIDS is perceived to be more stigmatized and as such organizations …

The Burner Project: Privacy and Social Control in a Networked World

As mobile phones become increasingly ubiquitous in today’s world, academic and public audiences alike are curious about the interaction between mobile technologies and social norms. To investigate this phenomenon, I examined how individuals use technology to actively manage their communication behaviors. Through a three-month research project on usage patterns of Burner, a mobile application, this thesis explores the relationships among technology, culture, and privacy. Burner is a service that equips individuals with the means to create, maintain, and/or dissolve social ties by providing temporary, disposable numbers to customers. The application offers a way to communicate without relying on a user’s personal phone number. In other words, Burner acts as a “privacy layer” for mobile phones. It also provides a valuable platform to examine how customers use the application as a strategy for communication management. This thesis represents a marriage of practice and theory: (1) As an applied enterprise, the project was constructed as a customer needs assessment intending to examine how the service was situated in the lives of its users. Doing Our Work Better, Together: An Application of Relational Coordination Theory to Explore and Shape Excellence in Trauma Care

I conducted a mixed-methods collaborative ethnography using the lens of relational coordination theory. This included a qualitative survey using an established tool to analyze the relational dimensions of multidisciplinary teamwork, participant observation, interviews, and narrative surveys. Findings were presented to clinicians in working groups for further interpretation and to facilitate co-creation of targeted interventions designed to improve team relationships and performance. I engaged a complex multidisciplinary network of ~500 care providers dispersed across seven core interdependent clinical disciplines. Initial findings highlighted the importance of each dimension of relational coordination in trauma care. Narrative survey and ethnographic findings further highlighted the centrality of team briefings and a translational simulation program in contributing positively to team culture and relational ties. A range of 16 interventions – focusing on structural, process and relational dimensions – were co-created with participants after reflecting on findings and are now being implemented and evaluated by various trauma care providers. Relational coordination theory is a valuable way to conceptualize the coordination of trauma care. Collaborative reflection on quantitative and narrative data through this lens can be used as a community-based quality improvement tool.

Understanding the RCS Way: A Study of Organizational Culture

The quality of workplace morale can positively and negatively affect employee engagement. Engaged employees are more likely to participate in active communication with one another. They are also more likely to adopt the organizational goals and work towards creating a productive work environment. Communication and engagement build trust. Established trust between employees and executive leadership is a delicate relationship to maintain, though it is necessary when contending with periods of stress. The organization profiled below experienced the stressor of needing to grow without alienating those afraid of change or used to operating in one predictable direction. There was a desire to generate engagement, build trust and make room for employee-directed change. This desire left the organization open to exploring its culture and its impact on employee engagement.

Facilitating Positive Relationships between Patients and Foreign Born Providers in South Central Pennsylvania

Foreign-born providers make up over a quarter of the physician workforce nationally. Patients in south central Pennsylvania are primarily white with limited interaction with foreigners which can produce barriers to communication and trust. This study proposes practical steps for building positive relationships between patients and their foreign-born providers. Ethnographic methods were used to interview and survey patients and providers about the relationships between foreign-born providers and patients, primarily in the Summit Health system. The results of the study provide a framework of how trust is built between patients and providers in general, suggest additional actions for foreign-born providers, and propose ways patients can do their part to achieve a positive relationship with their provider. While much of the literature on cultural competence is in the context of patients who are from minority ethnicities, this study adds to the body of research by also considering the providers as part of minority groups.

Developing Policy for a Tech Program Based on Understanding Organizational Practices

This thesis contributes to research that informs the studies of organizational management and organizational anthropology. It examines the internal hierarchy and organizational practices of a Tech Company and describes how findings contributed to policy recommendations aimed towards supporting a “guild” model for organizational success. The data collecting and research were undertaken while working as an employee of the Tech Program and subsequent analysis continued past the end of that phase of work. Methods included semi-structured interviews which captured the sentiments and understandings of employees within the organization, and a questionnaire which revealed sentiments and experiences from former employees. These were buttressed with participant observation engaged through a participatory action research methodology. Findings add to the work directed towards understanding the effect of Founder’s Syndrome within organizations. Additionally, this thesis contributes to a growing body of research centered on best practices for fostering positive organizational growth by creating lines of communication from front-line employees to management level employers.

Giving Voice to Multiple Sclerosis: A Patient and Provider Investigation

With the advent of the telephonic and technological healthcare revolution, pharmaceutical corporations seek to improve patient compliance and quality of life by contracting with services providers. As an employee of one such provider, working for more than three years on a medication for the neurologically degenerative disease, multiple sclerosis, this investigation utilizes a mixed methodological approach. In order to improve and diversify the clinical services provided to patients, I was contracted as a consultant. I interviewed phone and PRN nurses, developed and released a PRN survey, and interviewed patients living in the Dallas/Fort Worth, Texas area. The combined experiences and expertise of the three groups who participated would serve to inform and develop new programs and services for patients with differing disease states. The research resulted in a re-imagining of the social networking theory of health, as well as the works of Pierre Bourdieu and Michel Foucault, to serve the evolving tele-health and technologically based healthcare workplace.

Using Ethics to Teach Social Emotional Learning to At-Risk Youth: Recontextualizing Content and Determining Efficacy

At the Northwest Regional Learning Center (NRLC), an alternative high school in Arlington, Washington serving only at-risk youth, a new ethics course was conducted to assist students with their social-emotional learning development (SEL) and provide NRLC staff with greater insight into the lived experiences of students. Through semi-structured interviews, longitudinal ethical position surveys, and in-class observational ethnographic notes, this study presents shifts in student ethical positions over time as students engaged in this new course. By drawing from the knowledge at-risk students bring to school and focusing on behaviorism, progressive teaching theory, and constructivism, this course promoted open, student-led discussion that helped establish and build critical thinking skills, learn about perspectives in relation to others, and analyze various ethical positions. Through learning more about the lived experiences of their students, teachers at NRLC were able to contextualize and accommodate individual student behaviors, needs, and beliefs over their high-school experience. Drawing from student beliefs and experiences, the new course content was largely created by the students, providing at-risk youth an environment to openly share their beliefs while directly relating course content to their lives outside of school. Building a Vegan Community of Practice: An Outreach Analysis for Vegan Society of PEACE, Houston, Texas

This research is focused on a group of vegan and vegan-curious individuals who are creating, building and maintaining a vegan community of practice in Houston, Texas. Through ethnographic methods, including participant observation, in-depth semi-structured interviews, surveys, quantitative analysis, and use of geographic information systems (GIS), this thesis considers motivations, group hierarchies, core and peripheral membership, practices, beliefs and construction of identity within the vegan community of practice. Further, concepts from the anthropology of religion are utilized in discourse analysis around conversion to ethical veganism, preaching, and religious-ethical beliefs around enlightenment and the principle of ahimsa. Utilizing subcultural studies and social movement theory, this thesis also shows how the vegan community of practice fits into vegan subcultures and the greater vegan lifestyle movement. Finally, as an applied project, deliverables to the client Vegan Society of PEACE includes both personal and structural barriers to veganism which are understood with respect to a race-conscious approach to veganism, and with special consideration given to the capitalist commodification of animals. Suggestions are given and strategies for growth of the community are highlighted at the end of this paper.

Exploratory User Research for a Website that Provides Resources for Educators of American Indian Students in Higher Education

Several studies have indicated that American Indian students in the United States higher education system confront unique challenges that derive from a legacy of colonialism and assimilationist policies (Huff 1997). Several scholars, American Indian and non-Native alike, have explored the effects of this history upon students in higher education (Brayboy 2004; Guillory and Wolverton 2008; Waterman and Lindley 2013). Very few, however, have explored the role of the educators of American Indian students, and most of the literature focuses on K-12 educational settings (McCarty and Lee 2014; Yong and Hoffman 2014). This thesis examines exploratory user research conducted to generate a foundational understanding of educators of American Indian students in higher education. Utilizing methods from design anthropology and user experience, semi-structured interviews and think-aloud sessions were conducted, almost exclusively virtually, for 17 participants. This research was conducted for a client, Fire & Associates, as part of the applied thesis process. Findings revealed a complex web of needs for educators of American Indian students in higher education related to teaching diverse students, the use of media and technology in the classroom, and the process of networking among other educators. The Product is People: An Investigation of Missile Combat Crew Perceptions Surrounding Standardized Training Curriculum

Missile Combat Crew members are officers in the United States Air Force responsible for operating nuclear-tipped intercontinental ballistic missiles. They undergo on-the-job training as part of the curriculum necessary to progress in their careers and achieve higher levels of job responsibility. The curriculum they use is created and maintained by 20th Air Force Test and Training Section. This product is known as the Missile Combat Crew Commander Upgrade program, and it has received criticisms from stakeholders who use it for being out of date and failing to capture the necessary topics for ensuring adequate on the job training is being conducted. This project seeks to examine these critiques, break down the curriculum produced by 20th AF into stages (creation, implementation, and feedback) for evaluation, uses principles of user-oriented design drawing on design anthropology to suggest alternative methods for curriculum creation, and utilizes the results of a diagnostic survey to provide data-driven recommendations to 20th AF for future rewrites of their product based on feedback from the crew members who use their product in the field.

Understanding Affluence through the Lens of Technology: An Ethnographic Study toward Building an Anthropology Practice in Advertising

This thesis describes a pilot study for a new cultural anthropology initiative at Team One, a US-based premium and luxury brand advertising agency. In this study, I explore the role and meaning of technology among a population of affluent individuals in Southern California through diaries and ethnographic interviews conducted in their homes. Using schema theory and design anthropology to inform my theoretical approach, I discuss socioeconomic and cultural factors that shape these participants' notions of affluence and influence their presentation of self through an examination of their technology and proudest possessions. I put forward a theory of conspicuous achievement as a way to describe how the affluent use technology to espouse a merit-based model of affluence. Through this model of affluence, participants strive to align themselves to the virtuous middle-class while ascribing moral value to their consumption practices. Lastly, I provide a typology of meaningful technology artifacts in the affluent home that describes the roles of their most used tech devices and how each type supports conspicuous achievement.

The aim of this thesis is to investigate the current status of job descriptions at a Public School System, in [City], [State], USA, and to make recommendations for improvement in terms of job description content, format, and the creation and updated processes. This work covers job descriptions among classified employees and does not cover job descriptions for instructional staff (teachers, principals, etc.) or Executive Administrative Staff. The work begins by introducing the reader to the client and the current status of the client's job descriptions. The demographics of the subject population are discussed as well as the research methodologies. Findings are presented in light of research data and analyzed using Social Identity Theory, as well as business leadership principals. Finally deliverables are provided and recommendations are made. Advancing a Community's Conversations About and Engagement with Climate Change

The goal of this project completed for the Greater Northfield Sustainability Collaborative (GNSC) was to understand how Northfield, Minnesota citizens are experiencing climate change. Thirty individuals were interviewed to find out what they know about climate change, what actions they are taking, what they think the solutions are to the problems, and what barriers they have to more fully engaging with climate change issues. The interview results are intended to promote and advance the community's discussion on climate change via social learning and community engagement activities such as town hall forums and community surveys. These activities encourage citizens in the community to have direct input into the development of the community's climate action plan (CAP). Analysis of the interviews showed that the interviewees are witnessing climate change, that most are taking at least some action such as recycling or lowering thermostats, that they can name barriers to their own inaction, that they say communication about climate change remains confusing and is not widespread in Northfield, and that they are able to provide numerous suggestions for what the local and broader leadership should be doing. High Driver Turnover among Large Long-Haul Motor Carriers: Causes and Consequences

My thesis provides evidence supporting a theory asserting that the high level of competition that exists between motor carriers operating within long-haul trucking is the most significant factor contributing to the continuously high driver turnover rates affecting the entire logistics industry. I explore how long-haul truck drivers internalize the conflict between their identity and the aggressively competitive environment within which they work. Social science authors, industry reports, and truck driver feedback from my own ethnographic study are analyzed for contexts in order to explore the current operating definition of success for motor carriers in both monetary and human terms.

Aging Texas Well: An Assessment of Denton's Aging-Friendliness

The purpose of this research was to conduct a needs assessment for the city of Denton, Texas to learn how residents view Denton's aging-friendliness. The research design was based on the Texas Department of Aging and Disability Services' Aging Texas Well Toolkit and was funded by a two year grant from that agency. Both qualitative and quantitative methods were used to gather data on six community indicators: demographics, housing, transportation, health care (including mental health and substance abuse services), recreation, and community supports and services. Input from city residents was gathered through focus groups, followed by a survey of the broader community in the city to validate and prioritize the needs identified. The research found gaps in Denton's aging-friendliness. Denton residents feel that although there are some services for the aging in the area, other services are lacking. The top needs identified by residents were a single point of contact for, and better communication about, resources currently available, as well as a need for increased transportation options.

Social Distancing and Social Barriers: The Impact of the Pandemic on Dallas Youth

As stay-at-home mandates were put in place to curtail the spread of COVID-19, the extent to which today's youth has been affected by such efforts has gone largely under examined. Through a collaborative qualitative study with Big Thought, a Dallas-based nonprofit geared towards empowering youth, we sought to answer how the social interactions and socioemotional wellbeing of their 2021 summer program participants were impacted, as well as how Big Thought was able to exhibit organizational resilience. Methods used for this study included digital and in-person ethnography, interviewing, and interactive media projects. Findings showcased noticeable adverse effects to the socioemotional wellbeing of youth (particularly among older cohorts), shifts in communication, gaps in learned practices of socialization, and coping through digital device use. Despite Big Thought's ability to display organization resilience, there is a clear need for additional concerted efforts to be practiced in reacclimating and guiding youth back into social environments and providing them with the resources and support to get there.

Exploring Ahope Client Satisfaction and Attitudes

I led a participatory action research (PAR) project with the staff and homeless clients of Ahope Day Center in Asheville, North Carolina, which was meant to evaluate client satisfaction with services and attitudes about certain issues. Project is led by an inquiry group consisting of members of Ahope staff and Ahope clients. The project is a co-designed, co-implemented, and co-interpreted mixed-method evaluation of Ahope's services, client attitudes about education and the environment, client adaptive strategies, optimism levels, and a mapping of client daily routines. The data was collected through participant observation, document analysis, surveys, a listing activity, and informal interviews. Documents were coded using grounded theory and themes emerged related to the value of the intangibles of security, community, and ‘being seen' at Ahope while some suggestions were also made. Findings included client attitudes indicating the importance of the environment and education to clients, high optimism levels among clients, and a number of suggestions for the improvement of Ahope's services.

The Power of Place: A Qualitative Evaluation of Stream Monitoring Data Usage by Decision-Makers in Dane County, Wisconsin

Has years of citizen-based nutrient sampling and baseline water quality monitoring efforts only had a "trivial" impact on natural resource management decisions? This thesis will explore this and other findings from a qualitative evaluation of the Rock River Coalition (RRC) citizen-based stream monitoring project in Dane County, Wisconsin, USA. These findings are the culmination of 47 semi-structured interviews with decision-makers from seven client-identified categories and participant observations of board meetings and other watershed groups. Interview questions focused on current strategies of data design and dissemination with the goal of constructing a clearer picture of existing data usage by Dane County decision-makers. In the wider picture of citizen science and community-based research, this case study aims to highlight barriers to data use and potential solutions. The results of this case study were understood through four key frames: (1) Bourdieu's concept of symbolic capital, (2) Barzilai‐Nahon's theory of network gatekeeping, (3) Newman et al.'s framework for leveraging the power of place, and (4) a Foucauldian approach to the production of scientific knowledge. An Ethnography of Direct-to-Consumer Genomics [DTCG]: Design Anthropology Insights for the Product Management of a Disruptive Innovation

Direct-to-consumer genomics (DTCG) health testing offers great promise to humanity, however to date adoption has lagged as a result of consumer awareness, understanding, and previous government regulations restricting DTCG companies from providing information on an individual's genetic predispositions. But in 2017 the broader DTCG market which also includes genealogical testing demonstrated exponential growth, implying that DTCG is starting to diffuse as an innovation. To better understand the sociocultural forces affecting diffusion, adoption, and satisfaction, qualitative ethnographic research was conducted with DTCG genealogy and health consumers. The data was qualitatively analyzed using thematic analysis to understand the similarities and differences in beliefs, attitudes, intentions, and mediating factors that have influenced consumers. Design anthropology theory and methods were used to produce ethnographically informed insights. The insights were then translated into actionable product management and business strategy recommendations.

It's Going to be Different, but It's Going to be Okay: Caregiver Perspectives on Autism, Culture and Accessing Care

Through ethnography influenced by public health and anthropological theory, I explored the cultural perceptions of autism among eight caregivers whose children received services from a local Dallas-Fort Worth autism treatment organization. Participant observations and semi-structured interviews with caregivers and program employees provided a rich and nuanced view into the state of care currently available in the DFW area while also highlighting areas for improvement. This research will be used to not only identify the barriers faced by North Texas Families while seeking out care,but also the strategies the organization uses when connecting with families from different backgrounds.

An Assessment of a Hospice and Palliative Care Partnership Program

This project attempts to describe how a hospice and palliative care partnership program works. Through the assessment of one such program, the researcher sought to find out the essential components of the partnership including how the two partner organizations interact and work together. Data was collected using various methods: document review of organization documents such as newsletters, annual or quarterly reports, brochures and other available literature e.g. materials on organizations’ website and on social media; in-depth interviews with stakeholders of both organizations that included staff and board members; observation of staff working; and participant observation during organization events. The findings of the research shows that in order for organizations to have an effective partnership program in place, both partners need to have strong leadership in place, possess a willingness to learn from each other, maintain regular communication, and visit each other regularly. With this in place, several outcomes of the program are likely such as: increasing advocacy for hospice and palliative care, increasing visibility of the organizations both nationally and internationally, and provides an opportunity for organizations to network with other organizations in their locality in order to achieve partnership objectives. Subjective Efficiencies: Water Use, Management and Governance in the North Platte Natural Resources District

The North Platte Natural Resources District (NPNRD) is one of 23 quasi-governmental organizations in the state of Nebraska that are organized by river basin and are responsible for the management of groundwater. Conversely, the state's surface water is governed by the Nebraska Department of Natural Resources under the system of prior appropriation. This study uses Foucauldian neoliberal governmentality and a contrasting theory of 'meandering' to explore the conflicting beliefs, perceptions and values that form the foundations of different notions of 'efficiency' as it pertains to water use and management in NPNRD while a political ecology lens is used to situate local perceptions within the regional context of the Platte River Basin. Study findings ultimately point to the remaining 'disintegration' of water governance despite the state's efforts to create legislation that seeks to merge ground and surface water management in practice.

There is limited anthropological research on inclusive play and universally designed playgrounds and this study aims to make some contribution in this field. FTLOTL is a non-profit organization located in Dallas, Texas, dedicated to White Rock Lake Park's maintenance. In 2014, FTLOTL became of the view that the park's current playgrounds lacked accessibility for differently-abled children. Therefore, FTLOTL decided to undertake a renovation project of Flag Pole Hill playground to incorporate inclusiveness and diversity in the playground design. The overarching objective of this exploratory, ethnographic needs assessment was to provide insights for an inclusive playground using universal design for families with special needs children. Fourteen parents, each with at least one child having physical, social, or intellectual disabilities in the Dallas/Fort Worth (DFW) Texas metroplex were interviewed. The interviews were semi structured, each lasting for about an hour and were digitally recorded. Later these audio recordings were transcribed verbatim and then coded using Dedoose.
